If single tires are used, the TRACK is measured (at the ground) from the center of one tire to the center of the opposite tire. If dual tires are used, the TRACK is measured from the center of the dual tires to the same point on the opposite side. All options may notbe available on all axle models:a.Driver-Controlled Differential Lock (DCDL)b.ABSc.Advanced Lube2.The use of NoSPIN “differentials” in any single rear drive axle will result in the exclusion of axleshafts from warranty considerations. Certain other carrier components will also be excluded from warranty considerations if their failure is deemed the result of a NoSPIN failure or malfunction.Depending on axle loading, the NoSPIN can cause all differential torque to be directed to one axle shaft causing overload (and potential failure). Attachment to the axle housing assembly and durability of the axle housing assembly as a result of suspension loadings on the housing, is the responsibility of the OEM. Meritor assumes responsibility of the bracket integrity and attachment only if:a.The brackets are attached by Meritor or,b.Meritor has established a prior agreement with the OEM.7.It is the responsibility of the OEM and/or the dealer to accurately convey all approved axle loadinginformation to the Body Builder if the chassis is sold as incomplete. It is also the responsibility of the final vehicle builder to insure the assigned tagged values for GAWR and GVW do not exceed those limits approvable by this vocational guideline.8.The OEM has the responsibility to determine Steering Axle Specifics (Maximum Turn Angle, TieRod Arm Selection, Steering Arm Selection, Geometry Limits, etc.). Once your practice begins the application, all other members of the practice will be locked out of the APEx Portal until the application phase is complete and ASTRO has processed your payment.Additionally, once you start the application, you will not have access to the Resources tab of the APEx Portal until access to the Self-Assessment has been granted. Before starting your application, ASTRO suggests that you download the Reaccreditation Guide and Medical Tracking Worksheet to begin your practice’s Self-Assessment preparation.Starting the ApplicationWhen you are ready to begin the application, the chosen APEx administrator will sign in to the APEx Portal and click the blue ‘Reaccreditation’ button. Click continue to confirm the start of the application.The application will pre-populate information from the previous accreditation cycle. During each step, you will have the opportunity to review the data and update it as required.Step 1 – Selecting a NetworkBefore continuing with the application, download these documents from the initial landing page:1. Facility Data Collection Form: A pdf document to send to each satellite facility (if applicable) to aid inupdating the application.2. Facility Agreement and Business Associate Agreement: Download a copy of both legal agreements andsend them to your legal team. Your legal representative should review the agreements while the application is being updated.NETWORK TYPE SELECTIONOn the pre-registration page, define your practice (network):• If you are reapplying with a single facility, select• If you are reapplying with more than one facility, selectNETWORK QUALIFICATIONSIf you select ‘Multiple Networks,’ you will be presented with the Network Qualification window. This will enable your practice to determine if the satellite location(s) are eligible for a multi-facility practice application. Select each item that applies to your multi-facility practice.After selecting Single Network or Multiple Network, and all your multi-facility qualifications (if applicable),clickNote: Your satellite facilities must meet all four qualifications, which include:1. All facilities are located within a 50-miles radius of the main facility.2. All facilities have common policies and procedures (SOP) for key evidence-indicators, including:• SOP for each treatment modality as applicable to the ROP• SOP that defines a process and timeline for individuals who are eligible, but not currently certified, to achieve certification• SOP for Culture of Safety• SOP for radiation survey requirements pre-/post-treatment for brachytherapy and unsealed radioactive sources• SOP on infection control• SOP of intradisciplinary peer review3. All facilities have a medical director, who is a radiation oncologist, responsible for each facility.4. If you need assistance regarding eligibility, including satellites outside the 50-mile radius of the main facility, please contact ********************* for further information regarding satellite eligibility.Step 2 – Selecting and Naming NetworksSELECTING YOUR NETWORKThis page will auto-populate each facility’s name and location from the previous accreditation cycle’s application.Review your facility(s) name and location, update any information and add/remove any satellites to the application (if applicable).Add a Network: Use this to add additional satellite(s) (if applicable).Edit Tool: Use this to edit the facility’s name and/or location by opening the facility’s creation window.Delete Tool: Use this to permanently delete a satellite. Only use this if the facility is no longer a part of yourmulti-facility practice. If the name or location has changed, use the edit tool.Once all facilities information is correct, clickNAMING MAIN AND SATELLITE FACILITIESEach facility in the APEx Portal must have a unique facility name. When adding or updating any facility’s name, you must use specific naming conventions. This includes facilities within the same multi-facility practice. Please refrain from using generalized names, even if that is your facility’s name. Here are some examples:When adding an additional satellite(s), you may receive an error stating that this facility already exists. This could happen if you name is too generalized or the same as another facility in the program.Step 3 – Application ReviewAPPLICATION LISTReview the information from the previous accreditation cycle for each facility listed in the application. If you are applying as a multi-facility practice, your main facility will be listed first.For facility(s) from the previous application: Click the to review the auto-populated information from the previous accreditation cycle. Any new satellites will require all information to be entered manually.Click to proceed, once all the information is correct.Step 4 – Legal AgreementsDuring the initiation of the application, the legal agreements (APEx Facility Agreement and Business Associate Agreement) should have been sent to your legal team for review. Once both have been reviewed and your legal department has authorized acceptance of the documents, you may electronically sign the agreements by clicking each box:Enter the first and last name, along with the title of the individual with the authority to sign the agreements. Enter the title of your organization (e.g., practice name, corporate name or collective name) and the details of your HIPPA officer.Step 5 – PaymentINVOICEThe final cost of accreditation maynot display in the portal if additionalfees (e.g., satellites outside the 50-mileradius) or discounts (e.g, discount ratefor 10+ practices in the APEx program)are applicable, or if you are optingfor the partial payment plan. In allinstances, your practice should requestan invoice so as not to delay yourpayment. The equipment comprised in the Stations shall be designed, constructed, maintained or used such that the Stations, when used, shall comply with the Act and Regulations and not cause interference to any other licensed or authorised stations or networks or telecommunication installation or equipment which may be lawfully owned, used or operated by any other person. The Licensee shall permit an authorised officer of IDA to inspect the equipment comprised in Stations in order to determine whether the interference is in fact caused by the said equipment.10.IDA may, at any time, withdraw the frequency(s) from or allocate a different frequency(s) lawfully in place thereof to the Licensee without assigning any reason(s) therefore.11. Unless authorised in writing by IDA, the connection of any equipment comprised inthe Stations to a Public Telecommunication Licensee’s network will not be permitted.12. The Licensee shall notify IDA of any intended change of address of the Licensee orany proposed change in any of the equipment comprised in the Stations or any proposed change in the operating radio frequency(s), the mode of transmission or usage, the location at which the Stations are installed, and before such change is effected, the Licensee shall obtain the prior written approval of IDA. The Licensee and persons authorised by the Licensee to operate the Stations shallensure that the Stations and the equipment comprised therein are not used for unlawful purposes or misused in any way. The Licensee shall ensure that the Stations, including the equipment comprisedtherein, the operating radio frequency(s) and the location at which the Stations are installed are not tampered with or modified in any manner unless IDA’s written approval has been obtained. As and when directed by IDA, the Licensee shall, at its own cost, conduct inspections on the equipment in order to satisfy IDA that the equipment has not been tampered with or modified. A failure to comply with this condition amounts to an offence under the Regulations.15. The Stations and this Licence shall be available for inspection at all reasonabletimes by any duly authorised officer of IDA. A failure to comply with this condition amounts to an offence under the Regulations.16. IDA may vary, modify, suspend and cancel any of the terms of this Licence inaccordance with the Act and the Regulations.17. When the Licensee ceases to operate the Stations or where this Licence ceases tobe valid, the Licensee shall return the licence to IDA for cancellation and IDA may require the Licensee to seal, disable or dismantle the equipment comprised in the Stations in the presence of any authorised IDA officer. HT162x Application GuidelinesD/N:AN0468EIntroductionHoltek’s HT162x series of devices are a range of LCD drivers which can drive multipledisplays and which can also directly map their internal RAM to the LCD correspondingLCD display. These devices also include serial communication, a WDT and Time Basefunctions. A tone function provides a fixed frequency output and an integrated powersaving mode greatly reduces power consumption, making the devices suitable for LCDdisplay driving product applications such as meters, home appliances, carinstrumentation and industrial instrumentation.This guideline will first introduce the HT162x series LCD driving MCUs principles ofoperation and then introduce some application methods and notes.Functional DescriptionAccording to LCD driving principles, an AC voltage needs to be applied to both COM andSEG pins to drive the LCD. If only a DC voltage is applied to an LCD segment then theliquid crystal may experience permanent damage over time. The LCD display contrast isdetermined by the RMS value of the voltage on the COM pin minus the voltage applied tothe SEG pin. This differential RMS voltage must be greater than the LCD saturationvoltage for the pixel to be on and less than the threshold voltage for the pixel to be off.The LCD driver will generate the simulation drive waveform required for LCD lighting byusing system control, and the corresponding pixel to achieve a display effect.The HT162x series will provide multiple COM and SEG output selections for different LCDdisplays. The master can carry out READ, WRITE, READ-MODIFY-WRITE operations by sending the corresponding configuration commands and also set the LCD screen display effects by setting the operating frequency, Time Base, WDT, LCD On/Off etc. The HT162x series communication modes and some functional operations will be described in the following section. HT162x Serial CommunicationThe HT162x series supports serial communication which contains three serial communication pins RD , WR , DATA, and a chip select signal CS pin. The pin functions are shown in the following table:Reading and writing data and writing command operations are carried out on the DATA pin. The RD pin is a READ clock input pin. The data is output on the DATA pin at the falling edge of the RD signal. The master can read data between the rising edge of the RD signal and the next falling edge when RD is high.The WR pin is a WRITE clock input pin. The Data, address, and command on the DATApin are written to the HT162x at the rising edge of the WR signal.When writing to the HT162x, the input clock for the write command or data is derived fromthe WR pin. The timing diagram for writing data is as follows:To read display the RAM data from the HT162x, the READ input clock is derived from theRD pin. The timing diagram for reading data is as follows:HT162x READ/WRITE OperationsBefore the HT162x operates, it needs to send an operation code to indicate what kind ofstatus is required for operation. D0~D3 is the corresponding displaydata to write display data to LCD. If the bit is "1", it will illuminate the correspondingdisplay bit, while for a "0", the corresponding bit will not be displayed.The process for continuously writing RAM data is shown as follows:DATA 1 0 1 A5A4A3A2A1ADD1D2D3DD1D2D3 D0 … Memory Address (MA) Data(MA) Data(MA+1) …A continuous write operation requires only the start address, which will automatically be incremented by one after the operation has completed. During a read process, except for different operation codes, the READ clock input is the RD pin.HT162x Timer and WDT FunctionsTimer and WDT StructureThe Time base generator and WDT share the same divided counter. The Time base generator can be enabled by the TIMER EN or WDT EN command. When the IRQ EN and WDT EN commands are enabled, the system clock will pass through WDT module after passing through the Time base generator. Once a WDT time-out occurs, the IRQ pin will remain at a logic low level until the CLR WDT or the IRQ DIS command is issued. If the TIMER EN command is enabled, the system clock will be directly output on the IRQ pin after passing through Time base generator. The IRQ pin will be disabled and remain in a floating state following an IRQ DIS command.If an external clock is selected as the system clock source, the SYS DIS command is invalid and the power down mode will not be entered. In the Power down mode, the Time base and WDT functions are disabled.HT162x Tone OutputThe Tone output function provides a fixed frequency output driving signals on the BZ and BZ pins for driving piezoelectric buzzers or for applications that require a fixed frequency.A 4kHz or 2kHz output frequency can be selected by software using the TONE 4K or TONE 2K commands. Once the system clock is disabled or the Tone output function is switched off, the BZ and BZ output will remain at a low level. The following table lists the related control commands for Tone output functions. TONE ON is the specific TONE output command for the HT1621. Other HT162x series MCUs only need to turn on the Tone output function to output the corresponding frequency by directly software configuring the TONE 4K or TONE 2K commandsHT162x LCD Driving OperationThe HT162x series provide multiple SEG and COM selections for driving different LCD display screens. The following section will take the HT1621 as an example to show how the HT1621 drives an LCD.The HT1621 is a 128 pattern (32×4) LCD driver, which can be configured with 1/2 or 1/3 Bias and 1/2, 1/3 or 1/4 Duty by software to make it suitable for multiple LCD display types. T he LCD driving clock is derived from the system clock, which can be a selected external clock, a 32768 crystal oscillator or an internal RC oscillator.The master can send the BIAS and COM commands to the HT1621 and set the BIAS and Duty of the driving waveform. LCD ON/OFF commands are used to control the on/off switching of the LCD bias generator to control the LCD display. Then set the Timer and WDTfunctions according to the actual design requirements. The IRQ output function will bedisabled by default after power on. After setting the LCD Bias and Duty values, turn onthe system oscillator using the SYS EN command. Finally, clear the LCD display RAMand then turn on the LCD bias generator. If the LCD bias generator is switched on withoutfirst clearing the LCD display RAM, the value of the display RAM will be indeterminateand garbled characters will appear.Display RAM Read/Write OperationsFor display RAM read/write operations, it is only necessary to write the first address. Users can directly read/write to the corresponding address of the display RAM data, the address is automatically incremented by one after the operation has completed. Users can continue to read/write to the display RAM data of the next address. The read/write operation flow chart is shown in the following figure.Application NotesFirst clear RAM in the initialisation processDuring initialisation, the HT162x must first clear the LCD RAM and then turn on the LCDdisplay by sending out an LCD On command. This will avoid garbled charactersappearing on the LCD after power-on. The main reasons for this are as follows.After the HT162x is powered on, the internal LCD RAM data is indeterminate. The LCDRAM data is directly mapped to the LCD driver. If the LCD RAM content is not firstcleared to zero, the LCD On command will be directly sent to turn on the LCD Biastransmitter, which will cause LCD to display garbled characters.Therefore before sending LCD On command, turn off all the display points by writing zeroto all the LCD RAM values. After clearing RAM is complete, send an LCD On command toturn on the LCD bias generator to avoid garbled characters.Power Down OrderIn practical applications, if power is separately supplied on the VLCD and VDD pins, it isrecommended to turn off the LCD driving voltage V LCD before turning off the logic supplyvoltage V DD during power-down. This will prevent the LCD display from displaying garbledcharacters due to different power-down sequences when VDD pin is powered off for thefollowing reasons:First turn off the V DD and then turn off the V LCD. V DD is the IC internal logic power supply.After V DD has been turned off, the HT162x internal LCD bias generator will stop working.COM is the scan signal whose level will stay at a certain level and whose power comesfrom V LCD. If there is a voltage on the VLCD pin, the COM and SEG pins will have adropout voltage caused by the garbled characters.It is recommended to turn off the LCD display by sending an LCD Off command beforeVDD is powered down. Both the COM and SEG pins are set high and even if there isvoltage on the VLCD pin, it will not cause garbled characters to appear on the LCD. 前言
本标准按照GB厅1.1一2009《标准化工作导则第1部分:标准的结构和编写》给出的规则起草。
本标准代替DL厅345一2010《带电设备紫外诊断技术应用导则》,与our345—2010相比,除编辑性修改以外,主要技术变化如下:
—一增加了术语和定义:离焦、计算模式(见3.3、3.4);
—增加了紫外成像仪应满足DL厅1779标准要求(见4.4);
——培加了缺陷设备放电检刹结果诊断(见6.3);
—培加了-个规范性附录(见附录B),两个资料性附录(见附录C、附录D);
—修改了紫外准确检测内容(见 5.2);
—修改了缺陷设备放电的判定方法(见 6.2);
——修改了缺陷严重程度分类及处理(见第7立);
一—修改了输变电设备和发电机电晕放电典型图(见附录F)。
标准由全国高电压试验技术和绝缘配合标准化技术委员会高电压试验技术标准化分技术委员会 (SAC厅C 163/SC 1) 归口。

PROGRAM FIELDS (cont.): law and human rights; natural resources, environmental policy, and climate change; public health policy and management; public policy analysis and public administration; substance abuse education, treatment and prevention; teaching of English as a foreign language; technology policy and management; trafficking in persons, policy and prevention; urban and regional planning. The university programs do not address themselves to the scientific or technical aspects of these fields, but rather to broad policyͲmaking and problemͲsolving issues. PLACEMENT IN UNIVERSITY PROGRAMS: Candidates who are selected for fellowships are placed in clusters by field of interest at U.S. universities specially designated to host a group of Humphrey Fellows. IIE, therefore, is not able to make placements at specific universities requested by candidates, or to award fellowships enabling candidates to attend a U.S. university on the basis that they have already been admitted. Candidates should not apply directly to U.S. institutions. APPLICATION GUIDELINES: 1. Please read the instructions for completing this application carefully before proceeding. Answer every question as completely as possible. All questions must be completed in English. 2. Please take special notice of question 13 within Step 2: Application Forms, in which the question reads "Give a 50Ͳ word summary of your proposed program plan." Please be sure to provide a succinct but substantive summary statement. Your application must contain complete and certified academic documents covering your entire period of study at universities or other postsecondary institutions, including advanced degrees. Documents must be accompanied by complete English translations. Your academic documents must consist of: x A certified official record (transcript) from each university or other postsecondary institution, listing the subjects you studied and the grades (marks) you received during each year of your enrollment. Include all postsecondary institutions you attended, even those from which you did not receive a degree or diploma. x Certified, official evidence of each postsecondary or university degree, diploma, or certificate awarded to you. To be considered official, each academic document must bear the seal of the issuing institution as well as the signature of its officials. Copies of original documents will be accepted only if they are separately certified as being authentic duplicates of originals. Certification of copies may be made by the issuing institution, by your selection committee, or by a U.S. consular official. 4. You are required to submit two letters of reference, one of which must be from your immediate supervisor in your current position. The letters of reference should be written in English or should include an English translation. Important information about TOEFL: The Test of English as a Foreign Language (TOEFL) is required by all U.S. universities. Absence of TOEFL scores could jeopardize your chance of obtaining a Fellowship. After passing a preliminary English test, you must register for the TOEFL immediately and take it as early as possible and no later than November. For information and/or test registration forms, contact the BiͲnational Education/ Fulbright Commission or U.S. Embassy in your home country. You must indicate that you want your score reports sent to the Institute of International Education (Hubert H. Humphrey Fellowship Program) Code Number 9616. You must be sure to indicate this code (9616) on the registration forms or on the answer sheets provided at the time you take the examination. TOEFL fee vouchers will be given to finalists by the BiͲnational Commission or U.S. Embassy. As soon as you receive your TOEFL score, please present it to the BiͲnational Commission or U.S. Embassy. As part of your application, you also must have a formal English language interview and submit the enclosed Report on English Proficiency (available in Step 4: Supplemental Forms).
