术后持续性疼痛大鼠脊髓、DRG中Epac1表达的变化

【摘要】Objective: To investigate the expression changes of exchange protein directly activated by cyclic adenosine monophospate(Epac1) in rats with persistent postoperative pain. Methods: Fifteen male adult SD rats aged 2-3 months weighting 200-250 g were randomly divided into 3 groups(n=5 each): normal group(group naive), sham operation
group(group sham) and skin/muscle incision and retraction-induced postoperative pain group(group SMIR). Group naive: no surgery; group sham: skin and muscle were incised; group SMIR: skin and superficial muscle of the medial thigh were incised and a small pair of retractors inserted. This tissue was retracted for 1 h causing persistent postoperative pain. Mechanical withdrawal threshold(MWT) was measured
before(baseline) and 1, 3, 7, 14, 21 d after sham operation and SMIR. The animals were then sacrificed at the time point which the MWT is the lowest. The L3-5 spinal cord and L3-5 dorsal root ganglion(DRG) tissue were harvested. Epac1 protein expression and the number of Epac1 positive cells after 7 days were checked by Western Blot and immunofluorescence staining technique. Results: The MWT was significantly shortened at 3-21 d after SMIR compared with group naive or group sham, with 7 d as the
lowest MWT. The protein expression and number of neurons expressing Epac1 was significantly increased at 7 d after SMIR(P<0.05, P<0.01) in L3-5 spinal cord and L3-5 DRG tissue. Conclusion: Epac1 may contribute to the persistent postoperative pain. The operation methods affect the expression of Epac1 and persistent allodynia. Long time distraction may cause chronic pain.%目的:观察术后持续性疼痛大鼠鸟嘌呤核苷酸转化因子交
换蛋白(exchange protein directly activated by cyclic adenosine monophosphate, Epac1)表达的变化。

方法:随机将15只月龄2~3个月,体质量200~250 g的成年雄性SD大鼠分为对照组、假手术组、术后持续性疼痛组[皮肤肌肉牵拉模型(skin/muscle incision and retraction, SMIR)组](n=5)。

对照组:不做任何操作处理;假手术组:仅切割皮肤、肌肉;SMIR组:在假手术组操作基础上牵拉皮肤、肌肉1 h。

测定各组术后1、3、7、14、21 d时机械性刺激缩爪
阈值(mechanical withdrawal threshold, MWT);用Western Blot、免疫荧光染色技术比较各组术后第7天,L3~5脊髄、L3~5背根神经节(dorsal root ganglion, DRG)中Epac1蛋白的表达和阳性细胞数。

结果:和术前比较,SMIR
组术后3~21 d MWT显著降低(P<0.001),术后7 d下降最显著;和对照组、假
手术组比较,SMIR组术后各时间点MWT降低显著(P<0.001)、术后7 d L3~5
脊髄、L3~5DRG中Epac1蛋白的表达和阳性细胞数增加显著(P<0.05, P<0.01)。

结论:术后持续性疼痛激活大鼠Epac1信号;手术操作方法影响Epac1的表达和持续性痛觉超敏,长时间撑开手术切口易引起慢性疼痛。
