HER2基因过度表达与乳腺癌的治疗杨静李惠平汪有蕃(北京大学第三医院肿瘤科北京100083)一、HER2的一般研究现状HER2受体(也称c-erbB-2,HER2/neu)是分子量为185kD、具酪氨酸激酶活性的跨膜蛋白,其编码基因定位于人染色体17q21,是表皮生长因子受体家族的成员[1]。

HER2基因过度表达可导致细胞过度增殖和表型恶性转化。

研究发现有30%的乳腺癌患者HER2基因过度表达,这类患者肿瘤恶性程度高、复发和转移发生早、预后差,对某些化疗药物有抵抗,并且发现HER2基因过度表达与患者的无病生存和总的生存期有关[2]。

将肿瘤的生物标志物作为肿瘤治疗反应的重要预测指标是肿瘤生物学研究的主要目的之一。

本文简要综述HER2基因过度表达与乳腺癌治疗反应的预测和效果。

二、HER2过表达与对治疗反应的预测和效果1.HER2过表达与对化疗的反应[3]对CMF治疗方案的反应:大多数的研究报告结果证明用这一方案治疗的乳腺癌患者中,HER2阴性的肿瘤患者比HER2阳性的患者更有效;而最近的一个研究则显示HER2阳性的病人也能从这个方案中获得很大效益。

对紫杉醇的反应:有文献评价了用不同方案治疗乳腺癌,包括紫杉醇,结果表明HER2阳性的患者反应率65%,HER2阴性的患者35%,作者认为这是因为HER2的信号传导通路上调了p21wafl,反过来抑制了p34cdc2激酶,这样紫杉醇可发挥它的凋亡活性。

2.HER2过表达与内分泌的治疗反应预测三苯氧胺是乳腺癌治疗最有效的药物之一。

在过去的25年里,医学家主要通过ER的水平来指导三苯氧胺的应用,通过评定原位癌组织中雌激素受体(ER)和孕激素受体(PR)来选择可能对其有反应的患者。

很多试验证实这个分子标记物对预测三苯氧胺反应的作用是有效的[4]。

在最近的几年里,对其他的一些三苯氧胺标记物也进行了研究,包括pS2(另一种雌激素调节蛋白)、热休克蛋白27和70、bcl-2蛋白、c-erbB-2(HER-2/neu)癌蛋白以及突变的p53肿瘤抑制蛋白。

1.Her-2结构:Her2位于染色体17q21,编码分子量为185KD的跨膜糖蛋白,由胞内酪氨酸激酶结构域、跨膜区和胞外结构域(extraeellular domain,ECD)三部分组成,是具有蛋白酪氨酸激酶(protein tyrosine kinace,PTK)活性的跨膜蛋白,属于表皮生长因子受体家族的成员。

2.Her-2功能:目前还没有发现能与Her2直接结合的配体,但HER-2与其他受体结合成异二聚体后能发挥信号转导功能,与不含Her2的二聚体相比其信号更强;另一方面,Her2的异二聚体使EGFR(表皮生长因子受体)在细胞膜过表达,加速了细胞的增殖,导致肿瘤形成和生长加快。

较多的研究发现Her2原癌基因通过其介导的主要信号通路如Ras—Raf—Mek—MAPK(丝裂原活化蛋白激酶通路、P13K—Akt(磷脂酰肌醇一3一激酶)通路等使细胞增殖周期缩短,恶性表现增强以及抗凋亡。

HER2肠癌的判读标准与HER2阳性乳腺癌类似。

主要有以下几种
方法:
1. 免疫组织化学染色(IHC):通过IHC染色检测HER2蛋白在
肠癌组织中的表达水平。

HER2阳性结果分为0、1+、2+和3+四个等级。

当IHC分级为3+时,属于HER2阳性肠癌。

2. 原位杂交(ISH):ISH是一种用来检测HER2基因拷贝数的方法。

通过观察HER2基因是否存在拷贝数增加的现象,判断HER2阳性
肿瘤。

HER2 ISH阳性结果为HER2基因拷贝数≥6。

3. FISH:FISH是一种高级ISH技术,可以确定HER2基因扩增。

HER2基因扩增定义为HER2基因拷贝数增加超过CEP17拷贝数的1.8倍。

FISH阳性结果为HER2基因扩增。

总的来说,当IHC分级为3+,或ISH或FISH检测结果为阳性,
则可判定为HER2阳性肠癌。

ER、PR、Ki-67表达的判定值建议采用报告阳性 细胞的百分比。Ki-67高低表达的判定值在不同 病理实验中心可能不同,可采用20%~30%作为判 断Ki-67高低的界值;同时,以20%作为PR表达 高低的判定界值*,可进一步区分Luminal A样 和Luminal B样(HER-2阴性)
上述不满足Luminal A样条件的Luminal样肿瘤均 可作为Luminal B样亚型
