骨髓细胞形态学检查对白血病诊断的临床价值

摘要：目的 探究骨髓细胞形态学检查应用至白血病临床诊断中的实际价值。方法 整理 2018 年 3 月至 2019 年 4 月医院检验科收集 的 110 例白血病患者的临床资料进行研究，指导患者进行骨髓细胞形态学检查、免疫学检查及染色体检查，并计算其骨髓细胞形态学 检查的确诊率。结果 110 例患者中经骨髓细胞形态学检查检出 98 例白血病患者，其确诊率为 89.09%（98/110），有 5 例不能分辨白 血病亚型，3 例经 WHO 分型标准发现其原始细胞为 21%~29%，2 例为粒、红、巨三系细胞和粒系、巨核系两系细胞均有病态造血现象， 2 例的骨髓有核细胞明显增多并有骨髓细胞发生病态造血的现象。结论 对白血病患者的临床诊断中进行骨髓细胞形态学检查，确诊 率高，对早期确诊白血病、后续治疗白血病均有着积极意义。 关键词：骨髓细胞形态学检查；白血病；免疫学检查；染色体检查 中图分类号：R733.71 文献标识码：A DOI: 10.19613/ki.1671-3141.2019.65.010 本文引用格式 ：满川 . 骨髓细胞形态学检查对白血病诊断的临床价值 [J]. 世界最新医学信息文摘 ,2019,19(65):19-20.

0 引言
白血病属于一种造血干细胞恶性克隆的恶性疾病，临床 表现以贫血、出血、发热、肝肿大、脾肿大、失明、心包积液、抽 搐、感 染、贫 血 等，若 未 及 时 治 疗，会 对 患 者 的 日 常 生 活 及 生 命健康有着极为严重的负面影响 [1]。靶向治疗、化疗、放疗、 肝细胞移植等多种方法均为治疗白血病的常见手段，需结合 白血病分型、预后分层等多方面的信息制定治疗方案 [2]。因 此，对白血病采取有效的诊断方法对其临床治疗有着重要意 义。本研究对 110 例白血病患者进行研究，旨在探究骨髓细 胞形态学检查的临床价值。
ABSTRACT: Objective To explore the practical value of bone marrow cell morphology examination in clinical diagnosis of leukemia. Methods The clinical data of 110 patients with leukemia collected from March 2018 to April 2019 in the laboratory department of our hospital were collected and studied. The patients were instructed to undergo bone marrow cell morphological examination, immunological examination and chromosome examination, and the diagnostic rate of bone marrow cell morphological examination was calculated. Results Ninety-eight cases of leukemia were detected by morphological examination of bone marrow cells in 110 patients. The diagnostic rate was 89.09%(98/110). Five cases could not distinguish the subtypes of leukemia. Three cases were found to have 21%~29% primordial cells by WHO typing criteria. Two cases had abnormal hematopoiesis in granulocyte, erythrocyte, megakaryocyte and granulocyte. Two cases had marked increase of nucleated cells in bone marrow. Morbid hematopoiesis occurs in bone marrow cells. Conclusion Morphological examination of bone marrow cells in clinical diagnosis of leukemia has a high diagnostic rate, which has a positive significance for early diagnosis of leukemia and subsequent treatment of leukemia.
