XIAP和XAF1在宫颈上皮内瘤变及鳞癌组织中表达的临床病

理学研究

摘要目的:通过观察XIAP和XAF1在正常宫颈上皮、宫颈上皮内瘤变、宫颈癌

中表达,探讨XIAP和XAF1在宫颈发生和演进中的临床病理学意义。方法:收集

宫颈组织,经病理学诊断为正常宫颈上皮49例,宫颈上皮内瘤变Ⅰ级53例,宫

颈上皮内瘤变Ⅱ级41例,宫颈上皮内瘤变Ⅲ级44例,宫颈鳞癌51例,应用免

疫组织化学技术检测XIAP和XAF1在以上组织中的表达;对宫颈鳞癌进行临床分期,分析XIAP和XAF1表达与临床分期的相关性。结果:免疫组化结果显示,XIAP阳性信号主要定位在细胞核,其阳性表达率在正常宫颈鳞状上皮、CINⅠ级、CINⅡ级、CINⅢ级、鳞癌中分别为26.56%(13/49),28.3%(15/53),

31.7%(13/41),68.18%(30/44),76.47%(39/51),采用卡方检验分析五个组间阳性率表达的差异有统计学意义。组间比较显示CINⅢ级组与宫颈鳞癌组中XIAP

表达率较宫颈正常组,CINⅠ级组,CINⅡ级组显著增高。免疫组化结果显示,

XAF1的表达主要定位在细胞核,其阳性表达率在正常宫颈鳞状上皮、CINⅠ级、CINⅡ级、CINⅢ级、鳞癌中分别为77.55%(38/49),69.81%(37/53),

75.61%(31/41), 22.22%(8/44),27.5%(11/51),采用卡方检验分析五个组间

阳性率表达的差异有统计学意义。组间比较显示宫颈正常组,CINⅠ级组,CINⅡ

级组较CINⅢ级组与宫颈鳞癌组中XAF1表达率显著增高。进一步分析XIAP和

XAF1表达相关性,显示两个指标之间表达有关联。XIAP和XAF1的表达与临床分

期无显著相关性。结论:⑴XIAP在正常宫颈上皮、宫颈上皮内瘤变、宫颈鳞癌中

的表达存在差异,在CINⅢ级与宫颈鳞癌的表达较其它组显著增高,提示该因子

过表达参与宫颈鳞癌的发生。⑵XAF1在正常宫颈上皮、宫颈上皮内瘤变、宫颈鳞癌中的表达存在差异,在CINⅢ级与宫颈鳞癌的表达较其它组显著降低,提示该

因子表达降低与宫颈鳞癌癌变相关。⑶ XIAP和XAF1表达间存在相关性,这与以

往研究报道它们功能上存在相关性是一致的。⑷本组数据不支持XIAP和XAF1表

达与宫颈癌临床分期相关。

Clinical Pathological Study of XIAP and XAF1 expression in cervical intraepithelial neoplasia and squamous-celled carcinoma

neoplasiaⅠ(CINⅠ),41 CINⅡ,44 CINⅢ, 51 cervical carcinoma, XIAP and XAF1 expression were assayed by immunohistochemical method, and relation of XIAP and XAF1 expression to clinical stage was analysized. Results: Immunohistochemical results showed that XIAP positive signal located in cell Nucleus, its expression positive rate were 26.56%(13/49),28.3%(15/53),

31.7%(13/41),68.18%(30/44),76.47%(39/51) in normal cervical epithelium, cervical intraepithelial CINⅠ, CINⅡ, CINⅢ, cervical carcinoma. XIAP expressed significantly higher in CINⅢand cervical carcinoma than others. Expression positive rate of XAF1 were 77.55%(38/49),

69.81%(37/53), 75.61%(31/41), 22.22%(8/44),27.5%(11/51)in normal cervical

epithelium, cervical intraepithelial CINⅠ, CINⅡ, CINⅢ, cervical carcinoma. XAF1 expressed significantly lowerer in CINⅢ and cervical carcinoma than others. Further analysis showed there is significant relation between XIAP and XAF1 expression. There is no significant relation of XIAP to XAF1 in clinical stage. Conclusion: ⑴ There is a difference in XIAP expression among normal

cervical epithelium、cervical intraepithelial neoplasias、cervical carcinoma, XIAP expressed higher in CINⅢ and cervical carcinoma than others which suggest XIAP overexpression involved in development of cervical carcinoma;⑵There is a difference in XAF1 expression among normal

cervical epithelium、cervical intraepithelial neoplasias、cervical carcinoma, XAF1 expressed lower

in CINⅢ and cervical carcinoma than others which suggest XIAP hypo-expression was associated with development of cervical carcinoma;⑶ There is a relation between XIAP and XAF1 expression; ⑷ In the study, there is no relation of XIAP and XAF1 expression to clinical stage.

宫颈癌是最常见妇科恶性肿瘤,研究显示其发生是多步骤、多基因参与的结果,与人乳

头状瘤样病毒感染,癌基因的激活及抑癌基因的失活,免疫因素,营养因素,端粒酶高表达

或活性增高,细胞因子如TGF的产生有关。在宫颈癌发生的研究中,大量研究显示宫颈上皮

内瘤变(cervical intraepithelial neoplasias,CIN)是一组与宫颈浸润癌密切相关的癌前期病变反

映了宫颈癌发生中连续发展的过程,即由宫颈鳞状上皮CINⅠ, CINⅡ, CINⅢ→原位鳞癌→早期

浸润鳞癌→浸润性鳞癌的一系列病理变化,并且涉及一些基因在这些病变中的差异表达。肿

瘤的发生和发展机制最重要的机制之一为细胞周期的紊乱而导致细胞凋亡和细胞增殖细胞凋

亡的失衡,即细胞凋亡被抑制以及细胞过度增殖而导致肿瘤发生。宫颈癌的发生也与细胞凋

亡调控异常密切有关。本研究中我们关注的是X连锁凋亡抑制蛋白(X-linked Inhibitor of apoptosis protein, XIAP)及其负性调节因子XAF1在宫颈鳞状上皮癌前病变和鳞癌中的表达差异。IAP是哺乳动物体内的唯一的调节细胞凋亡核心因子,其中XIAP 是IAP家族中调节作用

最强的凋亡抑制蛋白。XAF1(XIAP associated factor1)是XIAP负性调节因子之一。在本研究中,我们将从临床病理学角度,观察XIAP和XAF1在宫颈上皮内瘤变和宫颈癌组织中的表达,探讨这两种因子在宫颈癌发生中的意义。

1 材料与方法

1.1实验材料: 宫颈组织标本来自湖南省肿瘤医院,经病理学诊断为正常宫颈上皮49例,宫颈上皮内瘤变Ⅰ级53例,宫颈上皮内瘤变Ⅱ级41例,宫颈上皮内瘤变Ⅲ级44例,宫颈

鳞癌51例,病理诊断为中分化鳞癌。其中根据宫颈癌分期标准综合FIGO、UICC、AJCC的名称。按照临床分期将51例宫颈癌分为两组Ⅰ-Ⅱ组,Ⅲ-Ⅳ组,其中Ⅰ-Ⅱ组有20例,Ⅲ-Ⅳ组

有31例。51例宫颈癌中年龄29~70岁,中位年龄46岁。

1.2 主要试剂: XIAP兔多克隆抗体(sc-11426 Santa Cruz公司产品),XAF1兔多克隆抗体(Abnova)。柠檬酸盐(枸橼酸)抗原修复缓冲液(ZLI-9065)。显色剂 DAB(DAB-1030,)和

SP试剂盒(Kit-9730)均为福建迈新生物技术公司产品。

1.3实验方法

1.3.1 免疫组织化学技术SP法,即链霉菌抗生物素蛋白-过氧化物酶连结法。切片烤片,68℃,4hr;常规二甲苯脱蜡,梯度酒精脱水, 3%H2O2孵育15min,PBS冲洗3x5min,置

0.01M枸橼酸缓冲液(PH 6.0)中用微波修复20min,自然冷却, PBS冲洗3X5min。正常羊

血清工作液封闭,室温15 min,倾去勿洗;分别滴加XIAP和XAF1一抗4℃冰箱孵育过夜,PBS冲洗3x5min(用PBS缓冲液代替一抗作阴性对照)。滴加生物素标记二抗, 37℃孵育

30min, PBS冲洗3x5min;滴加辣根过氧化物酶标记的链霉素卵白素工作液,37℃孵育30min, PBS冲洗3x5min;DAB/ H2O2反应染色,自来水充分冲洗后;苏木素复染,常规脱水,透明,干燥,封片。免疫组化结果判断: ① XIAP,XAF1阳性信号均定位于细胞核,细胞核中可见棕

黄色或棕褐色颗粒。② 具体判断标准:根据阳性细胞在全部癌细胞中所占比例以及阳性细胞

染色强度判定实验结果:A:按显色细胞数记分,阳性细胞数<1/3为1分,阳性细胞数1/3~
