南充市社区门诊患者签约家庭医生的意愿及影响因素研究

【摘要】目的：了解南充市社区门诊患者签约家庭医生的意愿及其影响因素，为
提高签约率提供理论参考。

方法2015年8～9月，采用自制调查问卷，随机抽取
南充市顺庆区、高坪区共5个社区卫生服务中心的门诊患者500名进行现场调查。

共发放问卷500份，有效回收问卷485份，有效回收率为97.0％。

结果485例
社区门诊患者中，有198例（40.8％）知晓家庭医生式服务，有126例（26.0％）签约了家庭医生式服务协议书。

不同年龄、文化程度、医疗费用支付方式、是否患有慢性病、就诊择医倾向、是否有转诊经历的社区门诊患者的家庭医生式服务签约率比较，差异均有统计学意义（P＜0.05）。

结论社区门诊患者对家庭医生式服务的了解程度不高，签约率偏低，建议加大宣传力度，明确家庭医生定位，完善社区家庭医生培养机制，提升家庭医生服务能力，完善配套措施和补偿机制，夯实信息化基础。

%Objective To understand the willingness and influencing factors
of the family doctor in the community out-patient department of Nanchong City to provide a theoretical reference for improving the signing rate. Methods By adopting the self-designed questionnaire,500 outpatients were randomly extracted from 5 community health service centers in Shunqing Dis-trict and Gaoping District of Nanchong City. The field survey was conducted. A total of 500 questionnaires were issued and 485 questionnaires were valid. The effective recovery rate was 97%. Results
Among 485 community outpatients,198 cases(40.8%) knew the mode of family doctor service,and 126 cases(26.0%) signed the family doctor service agreement. The family doctor ser-vice signing contract rate had statistical difference among the outpatients with different ages ,educational
level,payment models of medical fees,medical seeking tendency,whether having chronic diseases and whether having referral experience(P<0.05). Con-clusion The awareness degree of community outpatients on the family doctor service is not high with lower signing rate. The fac-tors influencing the signing rate include age,payment models of medical fees,medical seeking tendency,whether having chronic diseases and referral experience. It is recommended to enhance publicity ,perfect the training mechanism of community family doctor service,raise the family doctors′ability of serv ice,improve the supporting measures and compensation mechanism and lay a solid information foundation.
