去氨加压素联合奥昔布宁治疗儿童单症状性夜间遗尿症的疗效

【摘要】目的:探讨去氨加压素联合奥昔布宁治疗儿童单症状性夜间遗尿症的疗效.方法:将2013年1月—2014年12月在开封市儿童医院治疗的206例的夜间遗尿症患儿,按随机数字表法分为2组,100 μg组患儿101例给予100 μg去氨加压素进行治疗;200 μg患儿105例给予200 μg去氨加压素进行治疗,疗程持续2周.将单独使用去氨加压素治疗失败的患儿120例按随机数字表法分为观察组和对照组,观察组患儿61例给予去氨加压素+5 mg奥昔布宁进行治疗,对照组患儿59例给予去氨加压素+安慰剂进行治疗,疗程持续4周,检测患儿膀胱容量和壁厚指数.结果:100μg组与200μg组患儿的完全响应率为34.65%(35/101)、
36.19%(38/105),差异无统计学意义(P>0.05).观察组患儿的完全响应率、部分响应率为21.31%(13/61)、24.59%(15/61),均明显高于对照组的5.08%(3/59)、13.56(8/59),差异均有统计学意义(OR=0.24,95% CI=0.10-0.56,P<0.01),且观察组患儿完全响应者表现出显著较低的膀胱容量-后壁指数和平均膀胱容量,夜间尿量显著高于部分响应者和无响应者,差异均有统计学意义(P<0.01).结论:去氨加压素联合奥昔布宁治疗膀胱容量受限和膀胱壁增厚的遗尿儿童,疗效显著,值得临床推广.%OBJECTIVE:To investigate the efficacy of of desmopressin combined with oxybutynin in treatment of children with monosymptomatic nocturnal enuresis.METHODS:206 children with monosymptomatic nocturnal
enuresis admitted into Children's Hospital of Kaifeng City from Jan.2013 to Dec.2014 were divided into two groups via the random number table.100 μg group with 101 cases were given 100 μg desmopressin treatment,200
μg group with 105 cases were treated with 200 μg desmopressin treatment,the treatment course was 2 weeks.The children only received desmopressin with a failure were divided into observation group and control group via the random number table,61 cases in observation group were given desmopressin combined with 5 mg oxybutynin,while the control group received desmopressin combined with placebo,the treatment course last for 4 weeks,the bladder capacity and wall thickness index of children were detected.RESULTS: The complete response rate of children in 100 μg group and 200 μg group were respectively 34.65%
( 35/101 ) and 36.19%( 38/105 ),with statistically significant difference (P>0.05).The complete response rate,partial response rate of observation group were significantly higher than that of control group [ 21.31%( 13/61 ) VS 5.08%( 3/59 ),24.59%( 15/61 ) VS 13.56 ( 8/59 ) ],with significant difference ( OR=0.24,95%CI=0.10-0.56,P<0.01).And the significant low level of BVWI and average bladder capacity were observed in children from observation group with complete response,the nocturnal urine volume of complete response was significantly higher than that of partial response and no response,the difference was statistically significant ( P
<0.01 ).CONCLUSIONS: The efficacy of desmopressin combined with oxybutynin in treatment of children with monosymptomatic nocturnal enuresis is significant,which is worthy of clinical promotion.
