【摘要】目的分析右美托咪定对老年患者手术麻醉药物用量和应激反应的影响.方法选取永康市第一人民医院2015年2月~2016年8月收治的100例进行髋关节置换术老年患者为研究对象,随机分为对照组和实验组,每组50例.实验组患者进行麻醉诱导前应用0.5ug/kg右美托咪定,持续时间为15 min,速度为0.5μg/(kg?h)持续到手术结束前0.5 h.对照组给予相同容量的生理盐水.比较分析实验组与对照组应激反应及麻醉药物用量情况.结果实验组患者丙泊酚与舒芬太尼用量分别为(520.72±112.47)mg与(44.20±13.62)ug显著少于对照组用量,差异有统计学意义(P<0.05).麻醉诱导前2组患者皮质醇水平与IL-6水平差异无统计学意义.手术开始0.5 h后,实验组皮质醇水平与IL-6水平显著低于对照组,差异有统计学意义
(P<0.05).结论右美托咪定能够显著减少老年患者手术麻醉药物丙泊酚及舒芬太尼用量,在很大程度上降低应激反应.%Objective To investigate the effects of dexmedetomidine on the amount of narcotic drugs and stress response in elderly patients. Methods 100 cases of elderly patients undergoing hip replacement from February 2015 to August 2016 were randomly divided into the control group and the experimental group, 50 cases in each group. The patients in the experimental group received 0.5ug/kg dexmedetomidine before anesthesia induction, with a duration of 15min and a speed of 0.5ug/(kg?h), which lasted for 0.5h before the end of the
operation. The control group was given the same volume of normal saline. The stress response and dosage of narcotic drugs in the experimental group and the control group were compared and analyzed. Results The dosage of propofol and sufentanil in the experimental group were (520.72±112.47) mg and (44.20±13.62)ug significantly less than that of the control group,the difference was statistically significant (P<0.05). Before anesthesia induction, there was no significant difference between the two groups in cortisol levels and IL-6 levels. After the operation, the cortisol level and IL-6 level of the experimental group were significantly lower than that of the control group (0.5 h), and the difference was statistically significant (P<0.05). Conclusion Dexmedetomidine can significantly reduce the dosage of anesthesia drugs propofol and sufentanil in elderly patients, and reduce the stress response to a great extent.
