【摘要】目的观察鸦胆子油乳剂联合博来霉素胸腔内注射治疗肺癌胸腔积液的疗效和不良反应.方法对64例肺癌胸腔积液患者采用胸腔放置引流管放净胸腔积液后,分为2组,单药组36例,联合组28例.单药组胸腔内注入博来霉素45～60 mg;联合组胸腔内每次注入鸦胆子油乳剂80～100 ml,博来霉素45～60 mg;1周后进行下一次治疗,连续2～3周,治疗1个月后观察疗效、生活质量和不良反应.结果单药组有效率为72.2%(26/36),生活质量有效率为58.3%(21/36),不良反应为发热(＜38.5℃)11例,胃肠道反应6例,胸痛3例.联合用药组治疗肺癌胸腔积液的有效率为92.9%(26/28),生活质量有效率为89.3%(25/28),不良反应为发热(＜38.5℃) 3例,胃肠道反应2例,胸痛0例.结论鸦胆子油乳剂联合博来霉素治疗肺癌胸腔积液比单用博来霉素的近期疗效、生活质量明显提高(P<0.05),不良反应减少,值得进一步研究及推广使用.%To observe the efficary and adverse reaction of brucea javanice oil emulsion ( BJOE ) combined with bleomycin ( BLM ) in the treatment of pleural effusion of lung cancer by thoracic injection . Methods 64 cases with lung cancer with pleural effusion were randomly divided into the monotherapy group of 36 cases and the combined group of 28 cases. The pleural effusion in all cases was drained by tube in thorax . The thoracic cavity of the monotherapy group was injected with 45 to 60 mg bleomycin, and 80 to 100 ml brucea oil emulsion and 45 to 60 mg of
bleomycin were injected into the thoracic cavity of the combined group. One week after the next treatment ,continuous 2 to 3 weeks. Then the treatment efficacy,quality of life and adverse reactions were observed after one month . Results The effective rate of monotherapy group was 72. 2% (26/36). The quality of life for the effective rate was 58. 3% (21/36) . Adverse reactions to fever ( <38.5℃. ) 11 cases,6 cases of gastrointestinal reactions , chest pain 3 cases. The effective rate of combined group was 92. 9% ( 26/28 ) , and the quality of life for the effec tive rate was 89. 3% (25/28). Adverse reactions to fever (38. 5℃. ) 3 cases,2 cases of gastrointestinal reactions , chest pain 0 case. Conclusion Combined treatment improves the shoft-term effect and quality of life and decreases the toxicities in the treat ment of lung cancer with pleural effusion .
