脑梗死患者血清脂联素水平变化及临床意义

【摘要】目的:动态检测脑梗死患者血清脂联素水平,观察脑梗死患者血清脂联素水平变化特点,探讨其临床意义.方法:采用病例对照研究,急性脑梗死120例为脑梗死组,同期未发生脑卒中的体检者120例为对照组.采用酶联免疫吸附法(ELISA),动态检测两组24 h、48 h、72 h、7 d不同时间点血清脂联素水平.比较不同时间点、脑梗死体积及神经功能缺损程度组脂联素水平的差异.结果:(1)脑梗死组的不同时间点血清脂联素水平均明显低于对照组,均具有明显的统计学差异(P<0.05).脑梗死组随时间的延长,血清脂联素水平呈递减趋势,不同时间点组间两两比较均有统计学差异(P<0.05);(2)血清脂联素水平在不同脑梗死体积组间差异无统计学意义
(P>0.05);(4)神经功能缺损程度越重,血清脂联素水平越低,不同神经功能缺损组血清脂联素水平差异均有统计学意义(P<0.05).神经功能缺损程度与血清脂联素水平成显著负相关(rs=-0.887,P=0.000).结论:脑梗死患者血清脂联素水平表达降低,其水平能反应病情的严重程度.%Objective To study the serum adiponectin level in patients with cerebral infarction and its clinical significance. Methods Detected the serum adiponectin level at different time point (24 h, 48 h, 72 h, 7 d) using enzyme-linked immunosorbent assay and compared the serum adiponectin level, the lipid peroxidation in cerebral infarct volume and neurologic defect between 120 patients with cerebral infarction and 120 healthy people. Results (1) The serum adiponectin levels decreased in
patients with cerebral infarction were significantly lower than those of the control group(P 〈 0.05). (2) The serum adiponectin levels decreased after cerebral infarction in a time-dependent manner. There was significant difference of serum adiponectin level between different time point (P 〈0.05). (3) There was no significant difference of serum adiponectin level between the volume of cerebral infarct (P〉 0.05). (4) There was negative correlation between serum adiponectin level and neurologic defect(rs = -0.887, P= 0.000). Conclusion The serum adiponectin levels decrease in patients with cerebral infarction, which correlates the severity of the cerebral infarction.
