技术在国内外的应用和准入情况
• 超声引导下血管穿刺,在临床上已经有十多年的使用经 验,根据发表的文章及指南,与基于体表标记的方法相 比较,在中心静脉穿刺期间使用超声引导,产生的并发 症更少,成功插入套管的尝试次数更少,过程持续时间 更短且操作的失败次数更少。因此,美国医疗保健研究 与质量局(AHRQ)和英国临床优化研究所(NIC E) 已发布了声明,提倡超声引导下进行静脉插管操 作。

技术原理
• 超声引导置管(Ultrasoundguided cannulation)被定义为 在针穿刺皮肤之前用超声扫描来 确定针的存在及其位置,然后进 行即时的超声引导的血管穿刺过 程。超声协助置管(Ultrasoundassisted cannulation)是指在 没有超声即时引导的情况下,用 针穿刺之前,用超声扫描来确定 目标血管的存在及其位置。超声 血管内定位(Ultrasound verification of intravascular placement)是用超声成像描述来 确定导引钢丝和导管在目标血管 内的正确位置。

Arterial cannulation may be very difficult and time-consuming in severe trauma patients with palpation method due to weak pulse. Complications were relate to multiple attempts to cannulate the artery. The purpose of this study was to establish a new artery cannulate method with ultrasound guided, avoiding traditional going through and draw backtechnique. compare ultrasound guided versus traditional palpation placement of arterial lines for time to placement, number of attempts, sites used.

超声造影-超声造影定义超声造影(ultrasonic contrast)又称声学造影(acoustic contrast),是利用造影剂使后散射回声增强,明显提高超声诊断的分辨力、敏感性和特异性的技术。

随着仪器性能的改进和新型声学造影剂的出现超声造影已能有效的增强心肌、肝、肾、脑等实质性器官的二维超声影像和血流多普勒信号,反映和观察正常组织和病变组织的血流灌注情况,已成为超声诊断的一个十分重要和很有前途的发展方向。

有人把它看作是继二维超声、多普勒和彩色血流成像之后的第三次革命。

超声造影-超声造影原理血细胞的散射回声强度比软组织低1000-10000倍,在二维图表现为“无回声“,对于心腔内内膜或大血管的边界通常容易识别。

但由于混响存在和分辨力的限制,有时心内膜显示模糊,无法显示小血管。

超声造影是通过造影剂来增强血液的背向散射,使血流清楚显示,从而达到对某些疾病进行鉴别诊断目的的一种技术。

由于在血液中的造影剂回声比心壁更均匀,而且造影剂是随血液流动的,不易产生伪像。

超声造影剂对于不同的应用,需要选用不同的造影剂。

目前最受关注的是用来观察组织灌注状态的微气泡造影剂。

通常把直径小于10微米的小气泡称为微气泡。

造影剂的分代是依据微泡内包裹气体的种类来划分的。

第一代造影剂微泡内含空气,第二代造影剂微泡内含惰性气体。

以德国先灵(Schering)利声显(Levovist)为代表的第一代微气泡声学造影剂,其包裹空气的壳厚、易破,谐振能力差,而且不够稳定。

当气泡不破裂时,谐波很弱,而气泡破裂时谐波很丰富。

所以通常采用爆破微泡的方式进行成像。

它利用爆破的瞬间产生强度较高的谐波。

心脏应用时,采用心电触发,腹部应用时,采用手动触发。

以意大利博莱科(Bracco)声诺维(Sonovue)为代表的第二代微气泡造影剂,其内含高密度的惰性气体六氟化锍,稳定性好,造影剂有薄而柔软的外膜,在低声压的作用下,微气泡也具有好的谐振特性,振而不破,能产生较强的谐波信号,可以获取较低噪声的实时谐波图像,这种低MI的声束能有效地保存脏器内的微泡,而不被击破,有利于有较长时间扫描各个切面。

US-FNAB技术及超声弹性成像技术在TI-RADS_4类甲状腺结节中的诊断价值

US-FNAB 技术及超声弹性成像技术在TI-RADS 4类甲状腺结节中的诊断价值史文洁1,谢伟2,张爱华11.句容市人民医院超声科,江苏句容 212400;2.句容市人民医院普外科,江苏句容 212400摘要 目的 探究超声引导细针穿刺活检技术(ultrasound-guided fine needle aspiration biopsy , US-FNAB )及超声弹性成像技术在TI-RADS 4类甲状腺结节中的诊断价值。

方法 采用随机抽样法选取句容市人民医院在2020年5月—2023年5月收治的86例甲状腺结节患者作为研究对象,均分别采取US-FNAB 及超声弹性成像技术(ultrasonic elastography , UE )进行检查,以病理学检查作为金标准,评估US-FNAB 技术及UE 技术对TI-RADS 4类甲状腺结节的诊断效能。

结果 经病理学检查结果可知,86例患者中,共存在102个结节,其中恶性结节74个,良性结节28个。

UE 检查结果显示,恶性结节72个,良性结节30个,相比病理学检查,恶性结节检出率为86.49%,US-FNAB 检查结果显示,恶性结节72个,良性结节30个,相比病理学检查,恶性结节检出率为94.59%。

US-FNAB 特异度92.86%、准确率94.12%、灵敏度94.59%,均高于UE 的71.43%、82.35%、86.49%,差异有统计学意义(P<0.05)。

结论 在疑似TI-RADS 4类甲状腺结节患者的诊断中,采用US-FNAB 技术的诊断效能明显高于超声弹性成像技术。

Methods 86 patients with thyroid nodules admitted to Jurong People's Hospital from May 2020 to May 2023 were selected as the study subjects using random sampling method. Both US-FNAB and ultrasound elastography (UE) were used for examination, and pathological examination was used as the gold standard to evaluate the diagnostic efficacy of US-FNAB and UE tech‐niques for TI-RADS type 4 thyroid nodules. Results According to pathological examination results, a total of 102 nodules were present in 86 patients, including 74 malignant nodules and 28 benign nodules. The UE examination results showed 72 malignant nodules and 30 benign nodules, with a detection rate of 86.49% compared to pathological examination. The US-FNAB examination results showed 72 malignant nod‐ules and 30 benign nodules, with a detection rate of 94.59% compared to pathological examination. The specificity, accuracy, and sensitivity of the US-FNAB were 92.86%, 94.12%, and 94.59%, respectively, which were higher than those of the UE 71.43%, 82.35%, 86.49%, the dif‐ference was statistically significant (P <0.05). 8.导管位置异常:置管后应常规行X线导管定位检查。发现导管 异位后,即应在透视下重新调整导管位置,如不能得到纠正, 则应将导管拔除,再在对侧重新穿刺置管。 9.心脏并发症:如导管插入过深,进入右心房或右心室内,可发 生心律失常,如导管质地较硬,还可造成心肌穿孔,引起心包 积液,甚至发生急性心脏压塞(心包填塞),因此,应避免导 管插入过深。 10.静脉血栓形成:可发生于长期肠外营养支持时,常继发于异 位导管所致的静脉血栓或血栓性静脉炎。一旦诊断明确,即应 拔除导管,并进行溶栓治疗。

swellinginfection Ultrasound is a useful way of examining many of the body's internal organs, including but not limited to the:heart and blood vessels, including the abdominal aorta and its major branchesliver gallbladderspleenpancreas kidneysbladderuterus, ovaries, and unborn child (fetus) in pregnant patientseyesthyroid and parathyroid glandsscrotum (testicles)Ultrasound is also used to:guide procedures such as needle biopsies, in which needles are used toextract sample cells from an abnormal area for laboratory testing.image the breasts and to guide biopsy of breast cancer (see theUltrasound-Guided Breast Biopsy page).diagnose a variety of heart conditions and to assess damage after a heartattack or diagnose for valvular heart disease.Doppler ultrasound images can help the physician to see and evaluate:blockages to blood flow (such as clots).narrowing of vessels (which may be caused by plaque).tumors and congenital malformation.With knowledge about the speed and volume of blood flow gained from a Doppler ultrasound image,the physician can often determine whether a patient is a good candidate for a procedure like angioplasty.How should I prepare?You should wear comfortable, loose-fitting clothing for your ultrasound exam. You may need to remove all clothing and jewelry in the area to be examined.You may be asked to wear a gown during the procedure.Other preparation depends on the type of examination you will have. For some scans your doctor may instruct you not to eat or drink for as many as 12 hours before your appointment. For others you may be asked to drink up to six glasses of water two hours prior to your exam and avoid urinating so that your Ultrasound: KidneyUltrasound: Liverthe transducer make secure contact with the body and eliminate air pocketsbetween the transducer and the skin. The sonographer (ultrasound technologist)or radiologist then presses the transducer firmly against the skin in variouslocations, sweeping over the area of interest or angling the sound beam from a farther location to better see an area of concern.Doppler sonography is performed using the same transducer.When the examination is complete, the patient may be asked to dress and wait while the ultrasound images are reviewed. However, the sonographer or radiologist is often able to review the ultrasound images in real-time as they are acquired and the patient can be released immediately.In some ultrasound studies, the transducer is attached to a probe and inserted into a natural opening in the body. These exams include:Transesophageal echocardiogram. The transducer is inserted into the esophagus to obtainimages of the heart.Transrectal ultrasound. The transducer is inserted into a man's rectum to view the prostate.Transvaginal ultrasound. The transducer is inserted into a woman's vagina to view the uterus and ovaries.Most ultrasound examinations are completed within 30 minutes to an hour.What will I experience during and after the procedure?Most ultrasound examinations are painless, fast and easy.After you are positioned on the examination table, the radiologist or sonographerwill apply some warm water-based gel on your skin and then place thetransducer firmly against your body, moving it back and forth over the area ofinterest until the desired images are captured. There is usually no discomfortfrom pressure as the transducer is pressed against the area being examined.If scanning is performed over an area of tenderness, you may feel pressure or minor pain from the transducer.Ultrasound exams in which the transducer is inserted into an opening of the body may produce minimal discomfort.If a Doppler ultrasound study is performed, you may actually hear pulse-like sounds that change in pitch as the blood flow is monitored and measured.Once the imaging is complete, the gel will be wiped off your skin.After an ultrasound exam, you should be able to resume your normal activities immediately.Who interprets the results and how do I get them?A radiologist, a physician specifically trained to supervise and interpretNote: Images may be shown for illustrative purposes.
