三种剂量舒芬太尼在患儿腹腔镜手术中的麻醉效果研究

【摘要】目的：研究三种剂量舒芬太尼在小儿腹腔镜手术中的麻醉效果。

方法选取我院2013年3月至2014年3月收治的腹腔镜手术患儿60例为研究对象，采用随机数字表法将其分为A、 B、 C三组，每组20例。

对所有患儿使用罗库溴铵和丙泊酚进行诱导，并按照不同的剂量给予A组0.3μg/kg舒芬太尼，给予B组0.4μg/kg舒芬太尼，给予C组0.5μg/kg舒芬太尼，可根据镇痛效果追加
0.1μg/kg舒芬太尼。

对比三组患者术中的麻醉效果。

结果 C组舒芬太尼使用量明显高于A组和B组(P＜0.05)， A组舒芬太尼的追加例数明显高于B组和C组(P ＜0.05)， B组的麻醉效果明显优于A组和C组(P＜0.05)。

三组PACU期间舒芬太尼的追加量、麻醉恢复时间、舒芬太尼的追加例数以及CHIPPS评分比较差异均不显著(P>0.05)。

结论对腹腔镜手术患儿使用0.4μg/kg舒芬太尼，麻醉效果最好，能够有效避免患儿在麻醉诱导期的气管插管的心血管反应。

%Objective To investigate the anesthetic effect of three different doses of sufentanil in pediatric laparoscopic operation. Methods 60 cases of children with laparoscopic operation treated in our hospital from March 2013 to March 2014 were selected and randomly divided into group A, group B and group C, with 20 cases in each group. All patients were given rocuronium and propofol for induction, and group A was given 0.3μg/kg sufentanil for anesthesia, group B was given 0.4μg/kg sufentanil, group C was given
0.5μg/kg sufentanil. 0.1μg/kg sufentanil could added to patients according to the analgesic effect. The anesthetic effect was compared between two groups. Results The usage of sufentanil of group C was significantly higher than that of group A and group B (P<0.05), the additional cases of sufentanil of group A were significantly higher than those of group B and group C (P <0.05), the anesthetic effect of group B was significantly better than that of group A and group C (P<0.05). The additional quantity of sufentanil, recovery time of anesthesia, additional cases of sufentanil and CHIPPS score of three groups in PACU period had no statistical difference (P >0.05). Conclusions 0.4 μg/kg s ufentanil has best anesthetic effect on children with laparoscopic operation, which can effectively avoid the cardiovascular responses of tracheal intubation in anesthesia induction period.
