The first patient who died on my watch was an older man with a faulty heart. We tried to slow it down with treatment, but it suddenly stopped beating completely. Later, whenever I would have a case like that one, I found myself second-guessing my clinical management. However, it turns out that thinking twice may actually cause more harm than good.
In a working paper, Emory University researchers found that when doctors delivering a baby have a bad result, they are more likely to switch to a different delivery method with the next patient, often unnecessarily and sometimes with worse results.
Because doctors make so many decisions that have serious consequences, thefalloutfrom second-guessing appears especially large for us. A 2006 study found that if a patient had a bleed after being prescribed (开药) warfarin, the physician was about 20% less likely to prescribe later patients the blood thinner that prevents strokes (中风). However, if a patient was not on warfarin and had a stroke physicians were still no more likely to prescribe warfarin to their other patients.
These findings highlight interesting behavioral patterns in doctors. In the blood-thinner study, doctors were more affected by the act of doing harm (prescribing a blood thinner that ended up hurting doctors were more affected by the act of doing harm(prescribing a blood thinner that ended up hurting a patient) and less affected by letting harm happen (not prescribing a blood thinner and the patient having a stroke). Yet a stroke is often more permanent and damaging than a bleed.
But this phenomenon is not unique to medicine. ''Overreaction to Fearsome Risks'' holds true for broader society.
For instance, sensational headlines about shark attacks on humans in Florida in 2001 caused a panic and led the state to prohibit shark-feeding expeditions. Yet shark attacks had actually fallen that year and, according to the study, such a change was probably unnecessary given the extremely small risk of such an attack happening.
Humans are likely to be influenced by emotional and often irrational (不理性的) thinking when processing information, bad events and mistakes. As much as we don't want to cause an unfortunate event to happen again, we need to be aware that a worst situation that can be imagined doesn't necessarily mean we did anything wrong. When we overthink, we fail to rely on thinking based on what we know or have experienced. Instead, we may involuntarily overanalyze and come to the wrong conclusion.
I have treated dozens of patients who presented with the same illnesses as my first patient, who died more than a year ago. Instead of second-guessing myself, I trusted my clinical instinct (本能) and stayed the course. Every one of those patients survived. You should trust your instinct in your life, too.

Getting rid of dirt, in the opinion of most people, is a good thing However, there is nothing fixed about attitudes to dirt.
In the early 16th century, people thought that dirt on the skin was a means to block outdisease, as medical opinion had it that washing off dirt with hot water could open up the skin and let ills in. A particular danger was
thought to lie in public baths. By 1538, the French king had closed the bath houses in his kingdom. So did the king ofEnglandin 1546. Thus began a long time when the rich and the poor inEuropelived with dirt in a friendly way. Henry IV, king ofFrance, was famously dirty. Upon learning that a nobleman had taken a bath, the king ordered that, to avoid the attack of disease, the nobleman should not go out.
Though the belief in the merit(优点) of dirt was long-lived, dirt has no longer been regarded as a nice neighbor ever since the 18th century. Scientifically speaking, cleaning away dirt is good to health. Clean water supply and hand washing are practical means of preventing disease. Yet, it seems that standards of cleanliness have moved beyond science since World War II. Advertisements repeatedly sell the idea: clothes need to be whiter than white, cloths ever softer, surfaces to shine. Has the hate for dirt, however, gone too far?
Attitudes to dirt still differ hugely nowadays. Many first-time parents nervously try to warn their children off touching dirt, which might be responsible for the spread of disease. On the contrary, Mary Ruebush, an American immunologist(免疫学家), encourages children to play in the dirt to build up a strong immune system. And the latter position is gaining some ground.

For as long as I can remember, my body and I have shared a relationship of discontent. Growing up, I was
skinnier than the other kids and at five I was told I wasn't attractive and that I must put on some weight. But no matter how much I ate, I stayed thin for years until adolescence. Then I started putting on weight almost immediately. I remember feeling happy as I began to fill out. However, my joy didn't last long.
I was thirteen when I was first called fat. Friends and neighbours would make jokes on my big size. As I entered my late teens, I had completely lost confidence in my body and, subsequently, in myself. Having failed to live up to conventional beauty standards, I was convinced that if I wanted to be loved, I needed to offer more, doing anything to please everyone around.
I entered adulthood thinking I wasn't “enough”—an idea that was seeded not only by the fact that “skinny” is celebrated, but also by the language associated with accounts of losing weight—self­improvement, discipline—all virtues. Being fat quickly categorizes you as lazy and undisciplined. Consumed by thoughts of the way my body looked, I didn't notice the other ways my body needed attention. I failed to realize, for example, that my period was much heavier and more painful than ever before. Actually I developed a rare disease and later I had two surgeries.
I was always made to feel that my weight was the root of a lot of problems in my life; I have learnt this is not true. After a lot of self­reflection and some professional help, I realized I never learnt to like myself. While two decades of self­hatred cannot be undone overnight, I have taken first steps to acceptance.
I am now much lighter than before, in body and mind. There are days I find voices on social media saying I am too fat to be loved or to be worthy, but I am learning not to focus on that thought for long. As long as I like myself, just the way I am, opinions at the end of the day are just water off a duck's back.
