目的 ：应 用 胎 儿 心 脏 超 声 短 轴 平 面评 价 孕 18-23 周 6 天胎 儿肺 、主 动 脉 内径 ，建 立 不 同 孕 周 胎 儿 肺 动 脉 (PA) 、主 动 脉 (A O) 内径 及 两 者 比值 (PA/AO) 正 常 参 考 范 围 ，并 初 步 探 讨 胎 儿 心脏 流 出道 畸 形和 非 流 出道 畸 形 状 态 下胎 儿 肺 、主 动 脉 内径 两 者 比值 的改 变。 方 法 ：选 取 2008 年 9 月 至 20 10 年 10 月在 复 旦 大 学 附 属 妇 产 科 医 院 常 规 产 前 检 查 ， 并 随 访 其 至 入 院分 娩 后 三 天 内接 受 新 生 儿 心 脏 超 声 检 查 的 孕 妇 。在 胎 儿 静 息 状 态 下 扫 查胎 儿心 脏 超 声短 轴平 面 获 得 肺 、主 动 脉 内径 ，计 算得 出两 者 之 比值 。如 果 由于产 前 超 声 检 查发现胎儿 畸形而选择 中止妊 娠 ，记录其胎 儿 尸解病 理报 告 。应用 相 关分析评 价 正 常 胎 儿 肺 动 脉 (PA ) 、主 动 脉 (AO ) 内径 及 两 者 比值 (PA/A0) 与 孕 周 的相 关 关 系 。 以 孕 周 和 肺 、主 动 脉 内径 作 为 自变 量 和 因变 量 ，应 用 回 归 分 析 拟 合 胎 儿 肺 、主 动 脉 内径 随孕周增 长 的回归模型 ，建立 回归方 程 并进 行拟 合优度 检验 。应用 单变量 方差分 析评 价 胎 儿 心 脏 流 出道 畸 形 和 非 流 出道 畸 形 状 态 下 胎 儿肺 、 主 动 脉 内径 及 两 者 比值 的 改 变。 结果 ：成 功 收集 正 常胎 儿 66 99 例 ，孕 周介 于 18-23 周 6 天 。并收 集胎 儿 心脏 畸形 64 例 ，其 中胎 儿 心脏 流 出道 畸 形 35 例 ， 非流 出道 畸 形 29 例 。胎 儿 肺 、主动 脉 内径 测 量 重 复 性 检 验 表 明观 察者 间 的重 复 测 量 结 果 均 无 统 计 学 差 异 。正 常 胎 儿 肺 、主 动 脉 内径 与 孕 周 高 度 相 关 （主 动 脉 内径 ：相 关 系 数 r=0.373 ，P〈0 .00 0 1; 肺 动 脉 内径 ：相 关 系 数 r = 0 .3 5 7 ，P 〈0 .0 0 0 1 )。以 孕 周 和 肺 、主 动 脉 内径 分 别 作 为 自变 量 和 因 变 量 ，得 到 线 性 回 归 方 程 : Y (主 动 脉 内径 (cm )) = 一 1.8 65+ 0.238X ( 孕周 ）；Y (肺 动 脉 内径 (cm )) = 一 1.9 16+0.2 56X ( 孕 周 ）。 回 归 方 程 的残 差 满 足 正 态 分 布 及 方 差 齐 性 ，根 据 正 态 分 布 理 论 ，建 立 18— 23 周 6 天 胎 儿 肺 、 主 动 脉 内径 及 两 者 比值 的正 常 范 围参 考 值 表 。 单 变 量 方 差 分析 显 示 ，经 孕周 校 正 后 ，胎 儿 心 脏 流 出道 畸形 时 ，其 肺 动 脉 内径 及 肺 、 主 动 脉 内径 比值 不 同于 同 孕周 正 常胎 儿 ；而胎 儿 心脏 非流 出道 畸形 时 ，肺 动 脉 (PA) 、 主 动 脉 (AO) 内径 及 两 者 比值 (PA /A0 )其 差 异 未 出现 统 计 学 意 义 改 变 。 结 论 ：本 研 究 建 立 了适 合 本 地 区 的处 于 孕 18— 23 周 6 天 胎 儿 肺 、 主 动 脉 内径及 两 者 比值 的正 常 范 围参 考 值 表 。胎 儿 心脏 流 出道 畸 形 时 ，其肺 、主 动 脉 内径 比值 会 发 生 改 变 ；胎 儿 心 脏 非流 出道 畸 形 时 ，肺 、主 动 脉 内径 比值 无 改变 。 关键 词 ：胎 儿 ，心脏 超 声 ，短轴 平 面 ，肺 动 脉 内径/ 主动 脉 内径 比值
