最近两个学期内有课程考试不及格的学生不具备该奖学金申请资格。

Those who have failed to pass any courses in the latest two semesters are not qualified for the scholarship.

□ 自我评价，含本学年总结、下一学年研究计划及导师签字/ Self-statement including summary of study in this academic year, and study plan for next academic year with signature of the supervisor

□获奖证书或作品复印件/Certificates of Awards and Publications

□ XX大学在学期间发表的论著/Publications during the study at SAU

6. 申请人保证/I Hereby Affirm that:

(1) 我确认上述信息真实无误。本人确知，如果提供了任何不实材料都会导致失去申请和获得奖学金资格。

I declare that all the information given on this form is true and correct. I understand that any misrepresentation will disqualify my application for the scholarship.

(2) 我保证在华期间，遵守中国法律、XX大学校纪校规和奖学金有关章程。

I shall abide by the laws and decrees of the Chinese Government, the rules and regulations of XX University, and the regulations of XX University Scholarships.

(3) 若获得奖学金资助，我将按照规定按时缴纳所有费用，并确知该奖学金需逐年申请。Once Approved, I shall pay all the fees on time, and apply for the Scholarship year by year.
