抗体免疫缺陷病(原发性免疫缺陷病)【病因】(一)发病原因体液免疫缺陷病包括,Bruton免疫球蛋白缺乏症,可变异型免疫球蛋白缺乏症,选择性IgA缺乏,选择性IgM缺乏。

1.Bruton免疫球蛋白缺乏症又称先天性免疫球蛋白缺乏症(congenital agammaglobulinemia)。

先天性免疫球蛋白缺乏。

2.常见的可变异型免疫球蛋白缺乏症(common variable agammaglobulinemia) 又称特发性晚发免疫球蛋白缺乏症(idiopathic late onset immunoglobulin deficiency),可变异型免疫球蛋白缺乏。

3.选择性IgA缺乏这是一组乳糜性腹泻的病因。

大多数选择性IgA缺乏者包括儿童,存有自身抗体,自身免疫性疾病则是这些人常见的。

4.选择性IgM缺乏也是产生多种感染疾患的原因。

(二)发病机制原发性免疫缺陷症是属于细胞免疫缺陷病,主要是胸腺发育不全,T细胞功能不足或是由于T细胞数少而导致B细胞功能缺陷。

如Di George综合征其病因是胚胎期中Ⅲ、Ⅳ咽囊发育障碍导致胸腺发育不良、甲状旁腺功能减退和人血管畸形等。

在临床免疫学检验上,表现为TH细胞数低下,抗体形成能力受到限制。

Nezelof综合征及核苷磷酸化酶缺陷为常染色体隐性遗传性疾病,致使胸腺发育不全和细胞免疫功能缺陷。

体液免疫缺陷病的直接表现是免疫球蛋白总量减少,或免疫球蛋白种类不全、或IgG的亚类不全。

也可以在总量上不明显减少,但某一种类免疫球蛋白突出减少。

Bruton免疫球蛋白缺乏症属于伴性型隐性遗传导致的免疫球蛋白减少。

其他类型免疫球蛋白减少,有的也可查到家族病史。

在免疫球蛋白缺乏的患者中,有的是B细胞功能不足,有的则是由于T细胞功能影响,或TH和TS细胞比例倒置。

【症状】1.X性连锁无丙种球蛋白血症(X-linked agammaglobulinemia) 临床上表现为:病人均为男性。

而重型先天性中性粒细胞减少症(severe congenital neutropenia, SCN),此类疾病目前我们的认识还比较少,它是原发性免疫缺陷病中的一类。

最早由瑞典儿科医师Kostmann在1956年报道。

SCN是以外周血成熟中性粒细胞缺乏为主要症状的一类异质性疾病,并且已被证实SCN与多种基因突变有关[1]。

近年来越来越多的报道基因诊断在临床诊疗过程中发挥着重要作用[2]。

本文先后对2例粒细胞减少患儿进行基因测序,发现分别存在CXCR4基因突变及G6PC3基因突变,本文意在揭示其基因类型与临床表现的关系,为本病的临床研究及基因诊断提供依据。

The room temperature of the interior heated area must be 50° F (10° C) or higher. The freeze resistant package may be surface or re-cessed mounted. If recess mounted the surface of the cover must be flush with the interior wall surface. The package is furnished with screws for mounting the cover to the box. If the box is recess mounted, do not fasten the top and bottom of the cover to the box. Use the holes on the front only.1.Wall should already be framed for the fountain using the rough-in dimensions shown in Fig's. 1, 2, 3, 4, 5, or6. Shown dimensions pertain to installation location (framing must support up to 150 Lbs. weight for singlefountain and 300 Lbs. for dual fountains). These dimensions are required for compliance with ANSI Standard A117.1 and ADA (Framing shown for reference).2. Attach wall plate assembly to wall as shown in Fig's. 3, 4, or 6 using 5/16" x 2" long bolts and flat washers(not provided). Tighten securely, (Fastener must match wall type, i.e. lag screws for wood studs, bolts and anchors for masonry construction).3. Install back panel. Place the upper edge of the panel above hanger on the wall. Slide the panel down until itengages the hanger. Be sure back panel is firmly engaged before releasing it.4. Remove bottom access panel from fountain basin and save screws. Install the fountain to the back paneland wall using (4) 5/16" x 3/4" long bolts and washers (provided) thru holes in back panel. A washer is used on the bottom two (2) holes only. Tighten securely.5. Assemble the operating cable to the fountain bracket. (Fountain should be mounted to exterior wall)Create a loop in the cable and thread the free end of the cable through the wall into the freeze resistant box.The adjustment nuts should be in the middle of threaded area on the operating cable. See Figure 7.6.Connect free end of operating cable to the valve-operating bracket. The end of the cables must be recessedinto the indents on the pivot brackets. Remove cable free play by adjusting the jam nuts on the ends of the operating cable. See Figure 7.7.Connect water line from fountain bubbler into freeze resistant box. The connection to the box uses a quickconnect water fitting. Position the water line, in the fountain, to drain back into interior mounted box. Any water left standing, in the exterior line, can freeze . See Fig. 13.To insert tubing, push tube straight into fitting until it reaches a positive stop. To remove tubing from the fittings, relieve water pressure, push in on dark gray collar while pulling out on the tubing. See Figure 12. 8.Connect drain and water supply lines to the freeze resistant fountain. Refer to Figure 1, 2, 3, 4, 5 or 6 forcomponent positions. Inline strainer must be used on the inlet water line. See Fig. 11.Start-up1.Turn on building water supply and check all connections for leaks. Repair as required.2.Stream height is factory set at 45-50 PSI. If stream height (See Fig. 14) needs to be changed adjust theregulator in the freeze resistant package. Clockwise adjustment raises stream height, counter clockwise adjustment will lower stream.3.Adjust operating cable as required. Cable system should have a minimal amount of free play to allow forproper valve operation. If the system is too tight the valve will stay in the on position creating constant water flow. Too much free play will result in non-operation of the valve with the push-buttons.4.Note: Water from the drain back tube in the freeze resistant package, will continue to run while the valve isactuated.5.After cable system is adjusted properly stuff flexible insulation into any openings between the outside walland the interior box.6.Recheck all connections. bovine gamma-globulin 分子量法国生物学家Brachet和英国生物学家Hammersen于1932年发现,当新鲜的晒干黄疸猪血清在高盐条件下析出时,可得到一种Globulin。

它被称为γ-S-Globulin,后来又被称为γ-Globulin。

这种蛋白质是由220个氨基酸组成的多肽链,其中含有15个半胱氨酸(cysteine)并且没有碳酸酐基。

它的分子量约为146 kDa(千道尔顿),具体数值会受到具体实验方法和测量条件的影响。

牛源γ-球蛋白是一种来源于牛血清的γ-球蛋白,牛血清是一种天然的高分子混合物,包含多种不同的蛋白质。

γ-球蛋白是其中最丰富的成分之一,它在血浆中占总蛋白质的15-25%。

γ-球蛋白的分子量可以通过多种方法进行测定。

一种常用的方法是凝胶过滤法(gel filtration)或者凝胶电泳法(gel electrophoresis)。

这种方法通常需要先将蛋白质进行裂解和鉴定,然后使用质谱分析仪器进行测量。

除了实验方法外,还可以通过基因组学和蛋白质组学的方法进行分子量的预测。

通过测量物种基因组中相应的基因或者利用已知的蛋白质序列,可以利用计算机算法预测蛋白质的分子量。

γ-球蛋白在免疫系统中起着重要的作用。

它主要由免疫球蛋白IgG组成,具有抗体的功能。

γ-球蛋白能够识别并结合特定的抗原,从而触发免疫反应。

在牛源γ-球蛋白中,IgG是主要的成分,它可以通过免疫电泳等方法进行分离和纯化。

γ-球蛋白有很多重要的应用,包括医学和生物技术领域。

These infants have recurrent pyogenic infections of the lungs, sinuses, and bones with such organisms as pneumococcus, haemophilus, and streptococcus.
Sometimes there is an associated increased frequency of infection. The disorder results from a delay in the onset of Ig synthesis despite normal numbers of B cells. T helper cells may be reduced. Premature infants are especially at risk because of lower levels of transplacental IgG at birth. The disorder is not familial.
Treatment
Bone marrow transplantation has been successful. Some success has been achieved with fetal thymus transplants. The severity of the heart disease often determines the eventual prognosis. Partial deficiency is compatible with prolonged survival.
Abnormalities of chromosome 22q (eg, deletion or monosomy) can be identified in 90% of cases. There seems to be an interruption of normal development of pharyngeal pouch structures near the 8th wk of gestation.

2.常见变异型免疫缺陷病(common variable immunodeficiency,CVID) 临床上表现为:男女均可发病,通常6岁以后开始有症状,20~30岁明显加重。

主要表现是病人反复感染。

常有慢性腹泻、乳糖耐受不良、吸收不良和蛋白质丢失性肠病。

贾第鞭毛虫感染是腹泻的常见原因。

部分病人胃酸减少,其中半数缺乏内因子,此外尚有脾大以及贫血和白细胞减少、血小板减少。

本病自身免疫性疾病发病率较高,如溶血性贫血、血小板减少性紫癜、甲状腺炎和类风湿性关节炎。

3.选择性IgA缺乏症(selective IgA deficiency) 临床上表现为:多数病人无症状,偶尔在检查时发现。

有的则有明显症状,多在10岁内出现,均因IgA缺乏,呼吸道、胃肠道、泌尿道等部位局部免疫功能降低,容易发生感染,也可发生过敏反应。

此外,类风湿性关节炎、系统性红斑狼疮、甲状腺炎与恶性贫血等自身免疫疾病发生率增高。

4.婴儿暂时性低丙球蛋白血症(infantile transient hypogammaglobulinemia) 临床上表现为:男女均可发病,一般病程持续6~18个月。

该病有细胞和体液免疫双重缺陷,患者常表现为反复感染。

该文报道1例62岁AB型胸腺瘤女性患者,反复细菌、真菌和病毒感染,血清Ig低下,B淋巴细胞缺如、CD4+ T淋巴细胞下降、CD4+/CD8+ T淋巴细胞比值下降,诊断为Good综合征。

该患者同时有粒细胞缺乏、慢性病性贫血,进一步骨髓宏基因组测序发现KIT与ARID1A基因突变,予补充Ig及抗感染治疗,患者症状好转出院。

随访患者仍有Ig下降,予定时复查血常规、Ig,并适时补充Ig。

Good综合征相对罕见,临床表现复杂,容易延误诊断,临床医师需引起重视。

Due to dual deficiency of cellular and humoral immune functions, patients constantly present with recurrent infection. This article reported one 62-year-old female patient with type AB thymoma presenting with recurrent bacterial, fungal and viral infection, low serum immunoglobulin, B lymphocyte deficiency, decreased CD4+ T cells and CD4+/CD8+ T ratio, which were consistent with the diagnosis of Good’s syndrome. Meanwhile, the patient was complicated with agranulocytosis and anemia of chronic disease. Furthermore, bone marrow genetic detection revealed mutations in KIT and ARID1A genes. The patient was discharged after immunoglobulin supplement and anti-infection therapy. During follow-up, serum immunoglobulin level was declined. Routine blood test was performed on a regular basis. Immunoglobulin supplement was given when necessary. Good’s syndrome is relatively rare in clinical practice.
