【摘要】目的肺癌是全球癌症死亡的首要原因,其中非小细胞肺癌(non-small cell lung cancer,NSCLC)约占肺癌总数的85%.目前化疗是晚期NSCLC常规治疗方案.文中分析几种常见化疗药物敏感性相关基因mRNA在中国人群中表达情况及其与临床病理特征的相关性以便进行个体化治疗.方法应用实时荧光定量聚合酶链反应(real-time quantitative reverse transcription polymerase chain reaction,RT-PCR)检测49例NSCLC石蜡组织标本中编码3型β微管蛋白的TUBB3基因、核苷酸切除修复交叉互补组1基因(excision repair cross complementation group1,ERCC1)、胸苷酸合成酶基因(thymidylate synthetase,TYMS)、核糖核苷酸还原酶亚单位M1基因(ribonucleotide reductase M1,RRM1)mRNA表达水平.结果 TUBB3 mRNA在53.33% NSCLC组织中高表达;ERCC1 mRNA在
40.00%NSCLC组织中高表达,其中肺腺癌高表达率为53.33%、肺鳞状细胞癌为8.33%,两者之间的表达差异有统计学意义,P=0.019;TYMS mRNA在
76.19%NSCLC组织中低表达;RRM1 mRNA在78.57%NSCLC组织中低表
达.TUBB3、ERCC1、TYMS、RRM1基因mRNA表达与NSCLC患者的性别、年龄、肿瘤分化程度无显著相关性.结论多数中国人NSCLC肿瘤组织中TYMS、RRM1 mRNA低表达;肺腺癌ERCC1 mRNA表达明显高于肺鳞状细胞癌.研究结果提示,多数NSCLC患者对化疗药物吉西他滨、培美曲塞敏感,肺鳞癌患者较腺癌患者应用铂类抗癌药物更有效.%Objective Lung cancer is the leading cause of
death worldwide , and about 85% are identified as non-small cell lung cancers ( NSCLC ). A wide variety of chemotherapies are used for advanced NSCLC at present . This article was to investigate the mRNA expressions of the genes related to chemotherapeutic sensitivity in NSCLC in Chinese population and analyze their correlation with the clinical pathological features of the disease . Methods We determined the mRNA expression levels of TUBB3, ERCC1, TYMS and RRM1 in 49 paraffin-imbedded tissue samples of NSCLC using quantitative RT -PCR. Results High expressions of TUBB3 (53.33% ) and ERCC1 (40.00% ) were found in the NSCLC tissue samples. The expression of ERCC1 was significantly higher in adenocarcinoma (53. 33% ) than in squamous cell carcinoma (8. 33%) (P = 0.019). TYMS and RRM1 were lowly expressed in 76.19% and 78. 57% of the NSCLC samples, respectively. The expression levels of TUBB3, ERCC1, TYMS and RRM1 showed no statistically significant correlation with sex , age and tumor differentiation. Conclusion TYMS and RRM1 are lowly expressed in most of the Chinese patients with NSCLC. The expression level of ERCC1 is higher in adenocarcinoma than in squamous cell carcinoma of the lung. Our results suggest that NSCLC is mostly sensitive to gemcit-abine and pemetrexed. Platinum-based chemotherapy is more effective on squamous cell cancer than adenocarcinoma .
