血余蛋黄油对表柔比星致慢性皮肤溃疡疮面愈合及氧化应激反应调节作用的实验研究

c 雄性小鼠 72只 , 表柔比星 2 g /L 皮内注射制备慢性皮肤溃疡动物模型 , 随机分为 4组 : 模型组、 重组 人表皮细胞生长因子组 (生长因子组 )、 维生素 E 衍生物 T ro lo x 组 ( T rolox 组 )、 血余蛋黄油组, 每组 18 只, 隔日换药 1次直至溃疡愈合, 造模后第 13 天、 第 27 天每组各取材 6 只, 另设正常对照组 10 只。采 用 I m age P ro plus图像软件分析疮面面积, 分光光度计法检测疮面组织超氧化物歧化酶 ( SOD)、 脂质 过氧化物 (MDA )水平, HE、 M asson 、 天狼星红染色观察疮面组织形态学及胶原变化 。结果 血余蛋黄 油能够促进疮面结痂, 缩短溃疡愈合时间, 平均愈合时间由对照组的 29 . 75 d 缩短至 26 . 16 d , 与生长 因子组 26 . 29 d接近 ; 并能够促进溃疡疮面的表皮细胞及胶原修复; 造模后 13 d疮面组织氧化应激损 伤明显, SOD 活力为 ( 9 . 67 2 . 80) U /mg, MDA 为 ( 15 . 48 3 . 72) m o l/ g , 血余蛋黄油治疗后 SOD 活 力提高为 ( 16 . 64 5 . 20) U /mg, MDA 水平降低为 ( 10 . 86 1 . 30 ) m ol /g, 与对照组比较差异显著, P< 0 . 05 。造模后 27 d疮面组织基本愈合, 各组 SOD 活力及 MDA 水平接近正常水平。结论 血余蛋 黄油能够促进皮肤溃疡小鼠疮面结痂, 缩短愈合时间; 其作用机制与其能够促进疮面表皮细胞及胶原 修复 , 降低疮面组织脂质过氧化损伤, 提高抗氧化能力有关 。 关键词 : 血余蛋黄油; 氧化应激; 皮肤溃疡 ; 小鼠 中图分类号 : R285 . 5

R esu lts Xueyudanhuang O in t m ent i m proved th e healing o f der m al u lceration . T he average healing t i m e w as shortened from 29 . 75 days in th e contro l group to 26 . 16 days in th e Xueyudanhuang O in tm ent group , wh ich w as a l m ost as the sa m e as that in the rhEGF group ( 26 . 29 days). Xueyudanhuang O in tm ent also pro m oted the repairin g o f epider m a l cells and co llagen o f derm al u lceration . T he in ju ry o f ox id ative stress w as serious 13 days after the m odel estab lishm en,t show ed as SOD activity w as ( 9 . 67 2 . 80) U /mg, andMDA leve lw as ( 15 . 48 3 . 72) m ol / g . A fter the treatm ent w ith Xueyudanhuang O in t m ent SOD activity in creased to ( 16 . 64 5 . 20) U /m g and MDA leve l decreased to ( 10 . 86 1 . 30) mo l/ g. T here w as a sig nif icant difference com pared w ith those in the control group ( P < 0 . 05). The w lceration was healed in itially 27 days after the establishm ent of the m odel , and SOD activ ities and MDA leve ls w ere al m ost no r m al in a ll group . Conclusion Xueyudanhuang O in tm ent can i m prove th e scabb ing of der m al u lceration , shorten the hea ling ti me . The m echanism m ay be related to that it can prom ote the repairing of ep ider m a l cells and co llagen o f der m al ulceration , relieve th e in ju ry of ox idative stress and in crease the antiox id ant act iv ity in m ic e . K ey w ord s : Xueyudanhuang O in tm en; t ox ida tiv e stress; der m a l u lceration; m ice 慢性溃疡缠绵难愈的主要原因之一是疮面细胞 的凋亡和增殖平衡失调 , 改善疮面氧化应激状态是 扭转细胞凋亡过度、 增殖不充分 , 促进伤口愈合的重 要途径之一。本实验选择表柔比星氧化应激损伤致 小鼠表皮溃疡模型, 观察北京中医医院临床行之有 效外用药血余蛋黄油的作用 , 揭示其可能的作用机 制 , 为油类中药制剂的临床应用提供理论指导。 1 材料与方法 1 . 1 动物 Ba lb / c 雄性小鼠 82 只, 体重 ( 19 1 . 2 试剂、 药物和仪器 注射用盐酸表柔比星 ( Ep irub icin , EP I), 10 mg / 支, 浙江海正药业 股份有限公司 , 生 理盐水配制 成 2 g /L溶 液, 现 用 现 配; 血 余 蛋 黄 油 ( 京 药 制 字 Z20053416), 30 mL /瓶 , 由血余炭、 蛋黄组成 , 由北京 中医医院制剂室 提供; 重组人表皮 细胞生长因 子, 2 000 I U /mL, 深圳市华生元基因工程发展有限公司; 维生素 E 衍生物 T ro lo x , 美国 Sigm a 公司, 生理盐水配 制成 3 g /L; M asson 染色试剂盒, 福州迈新生物技术 开发公司; 天狼星红 , 美国 S igm a 公司; 超氧化物歧化 酶 ( SOD )、 脂质过氧化物 (MDA) 检测试剂盒, 南京建 成生物技术公司。电磁搅拌器, 美国 W il m ing ton 公 司; 全自动细胞分析仪, 美国 T her m o 公司。 1 . 3 造模方法 72 只 Ba lb / c 雄性小鼠戊巴比妥钠腹腔注射麻 1) g, 由中国 医学科学院实验动物中心提供 , 合格证号 0093638 。 醉 ( 40 m g / kg) , 背部备毛 2 cm ! 2 cm, 2 g /L 表柔比 星皮内注射 0 . 15 mL /只 , 造模后 3~ 5 d 出现溃疡, 11~ 13 d溃疡面积最大。 1 . 4 分组及给药 72 只 Balb / c 雄性小鼠造模后根据表柔比星渗 漏致红色疮面大小将动物随机分为模型组、 重组人 表皮细胞生长因子组 (简称生长因子组 ) 、 维生素 E 衍生物 T rolox 组 (简称 T ro lo x 组 ) 、 血余蛋黄油组 , 每 组 18 只, 另设正常对照组 10 只。模型组给予生理 盐水 , 生长因子组给予 2 000 I U /mL 重组人表皮细 胞生长因子, T rolox 组给予 3 g /L 维生素 E 衍生物 T ro lox, 血余蛋黄油组给予血余蛋黄油, 正常对照组 不予处理。将 0 . 5 cm ! 0 . 5 c m 纱布浸泡各组药物, 外敷于疮面, 医用胶带包扎, 隔日换药 1 次。 1 . 5 检测指标与方法 1 . 5 . 1 溃疡疮面愈合情况检测 每次换药 时溃疡疮面附 标尺, 数 码相机拍 照, I m age P ro plus自动图像分析疮面面积, 计算疮面面 积大小及愈合速度。 1 . 5 . 2 溃疡疮面组织形态学及胶原变化情况检测 造模后 13 、 27 d 每组选择溃疡疮面大小均匀的 6 只动物, 处死后剪取溃疡疮面及部分创周正常组 织, 1 /2 疮面组 织于 10 % 福 尔马林中 固定, 常 规脱 水, 石蜡包埋, 4 m 厚 切片。脱蜡脱水后进行 HE 染色、 M asson 染色及 1 % 苦味酸天狼星红染色 , 观察 疮面组织形态学改变及 I 、III型胶原。 M asson 染色

前期临床及实验研究表明针对放射治疗所导致的放射性皮炎血余蛋黄油能够促进溃疡愈合减轻疮面疼痛烧灼症状及其他不适辅助完成放射治疗疗程对于生长因子无效或效果不明显的患者血余蛋黄油仍然有效伤口愈合是一个连续的复杂过程愈合速度及程度既受伤口局部因素的控制也受全身因素的影疮面组织细胞氧化应激状态及其引起的细胞过度凋亡和增殖不足是皮肤溃疡经久不愈的关键原因之一
