【摘要】Objective To investigate effects of radioactive 125I particle stent implantation combined with Cisplatin + 5 Fluorouracil (CF) chemotherapy regimen on serum cytokeratin 19 fragment (Cyfra21-1),carcinoembryonic antigen (CEA) and survival time of patients with locally advanced esophageal carcinoma.Methods A total of 64 patients with locally advanced esophageal carcinoma admitted during March 2013 and January 2015 were divided into control group (n =32) and study group (n =32) according to different treatment methods.Control group was treated with CF chemotherapy only,while study group was treated with radioactive 125I particle stent implantation combined with CF chemotherapy,and one cycle was 3 weeks,and the treatment was continued for 4 cycles.In two groups,clinical efficacy and survival time after treatment,serum Cyfra21-1 and CEA levels before and after treatment,Karnofsky function status (KPS) scores and changes of dysphagia index scores were observed,and incidence rate of complications in study group was also observed.Results The total effective rate in study group was significantly higher than that in control group (P ＜ 0.05).After treatment,values of serum Cyfra21-1 and
CEA levels and dysphagia index scores were significantly lower than those before treatment in two groups,and the values in study group were significantly lower than those in control group (P ＜ 0.05).KPS scores were significantly higher than those before treatment in two groups,and the score in study group was significantly higher than that in control group (P ＜ 0.05).Survival time in study group was significantly longer than that in control group (P ＜ 0.05).All patients in observation group were successfully placed with radioactive 125I particle stent,and no stent related complications were observed during the follow-up period.Conclusion Radioactive 125I particle stent implantation combined with CF chemotherapy in treatment of locally advanced esophageal cancer may achieve good effect,and it may effectively reduce serum CEA and Cyfra21-1 levels,improve dysphagia symptom and body function status and prolong survival time.%目的探讨放射性125I粒子支架置入联合顺铂+5-氟尿嘧啶(CF)化疗方案治疗局部晚期食管癌的临床效果及对血清细胞角蛋白19片段(Cyfra21-1)、癌胚抗原(CEA)水平和生存期的影响.方法选取2013年3月-2015年1月陕西省核工业二一五医院治疗的64例局部晚期食管癌患者,根据治疗方法不同分为对照组(n=32)与研究组(n=32).对照组仅采用CF化疗方案治疗,研究组采用放射性125I粒子支架置入联合CF化疗方案治疗,3周为1个周期,持续治疗4个周期.观察治疗结束后两组临床疗效和生存期,治疗前后血清Cyfra21-1、CEA水平,卡氏功能状态评分(KPS)及吞咽困难指数评分变化情况,研究组并发症发生情况.结果研究组总有效率高于对照组(P＜0.05).治疗后,两组血清Cyfra21-1、CEA水平及吞咽困难指数评分低于治疗前,且研究组低于对照组(P＜0.05);两组KPS评分高于治疗前,且研究组高于对照组(P＜0.05).研究组生存期长于对照组(P＜0.05).研究组均
成功置入放射性125I粒子支架,随访期间均未发生支架相关并发症.结论放射性125I粒子支架置入联合CF化疗治疗局部晚期食管癌效果显著,可有效降低血清CEA、Cyfra21-1水平,改善患者吞咽困难症状及机体功能状态,延长生存期.
