音乐疗法在牙科焦虑患者阻生智齿拔除术中的应用

【摘要】目的:观察音乐疗法在高度牙科焦虑症患者阻生智齿拔除术中的临床应用效果.方法:收集2016-06—2016-11期间,就诊于我科,需行双侧对称性下颌第三磨
牙拔除术的高度牙科焦虑症患者 25 例.采用自身对照分组方式,第1次拔牙或第2
次拔牙随机完成音乐疗法研究组或对照组.研究组采用局部麻醉下给予音乐疗法配
合高速涡轮机辅助拔除阻生牙,对照组采用常规局部麻醉加高速涡轮机拔牙.分别记
录2组患者术前30 min、术前5 min、麻醉注射时、切开时、拔牙时、拔牙结束时、拔牙结束30 min 时,患者的血压、心率,并进行术后满意度,术前与术后改良牙科焦虑量表评分.结果:实验组满意度高于对照组,差异有统计学意义( P＜0.05).实验组中,术前5 min较术前30 min麻醉注射时、拔牙开始时、拔牙时,收缩压和心率
降低且差异有统计学意义(P＜0.05).对照组中,术前5 min与术前30 min、术后30 min血压与心率差异无统计学意义(P＞0.05).实验组与对照组各个时间段的收缩压、舒张压、心率、牙科焦虑量表评分,均差异无统计学意义( P＞0.05).结论:音乐疗法
配合高速涡轮机辅助拔除阻生牙具有一定的平缓心率和降血压的作用,但是降低的
程度很小,与对照组降低的程度相比,差异无统计学意义.说明音乐疗法并不具有明显平缓血压、心率的作用,不能够有效降低下颌阻生智齿拔除患者的焦虑程度,但能提
高患者满意度.%Objective: To observe the clinical effect of music therapy in odontectomy of impacted wisdom tooth. Meth-ods: 25 patients who underwent bilateral mandibular third molar odontectomy were randomly
divided into experimental and control group during June, 2016 to November 2016. Patients in the experimental group were applied music therapy combined with high speed turbine minimally invasive exodontia and local anesthesia, while the control group used conven-tional local anesthesia and high speed turbine minimally invasive exodontia without music therapy. The vital signs such as systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) were respectively measured at 30 minutes before surgery, starting of anesthesia injection, beginning of incision, tooth extraction, end of surgery, and 30 minutes after surgery. The degree of satisfaction, and the modified dental anxiety scale scores before and after surgery were recorded. Results: Clinical data of SBP, DBP, HR in experimental group were the same as those in control group. The score of mod-ified dental anxiety scale in the experimental group was the same as in control group after operation. But the degree of sat-isfaction in experimental group was higher than that of the control group. Conclusion: Music therapy combined with high speed turbine minimally invasive exodontias had no obvious effects to reduce the dental anxiety. However, it increases the degree of satisfaction.
