【摘要】目的探讨冠心病患者幽门螺杆菌(HP)感染的发生及其与血脂代谢的关系.方法选取2013年5月至2016年5月在我院治疗的冠心病患者150例(冠心病组),同时选取健康者100名作为对照组.采用14C尿素呼气试验检测HP感染情况,并根据结果将冠心病患者分为未感染(n=76)、轻度感染(n=53)和重度感染(n=21);同时根据SYNTAX评分将患者分为低危、中危和高危患者.测定各研究对象血脂水平.结果冠心病组和对照组HP感染率分别为49.33%和41.00%,未见统计学差异(P>0.05).冠心病组TC、TG和LDL-C分别为(5.50±0.81)mmol/L、(2.31
±0.52)mmol/L和(3.70±0.83)mmol/L,明显高于对照组,而HDL-C为
(0.93±0.27)mmol/L,明显低于对照组,差异均有统计学意义(P均<0.05);重度感染患者TC和TG分别为(6.11±l.09)mmol/L和(2.53±0.31)mmol/L,明显高于未感染和轻度感染患者(P<0.05);重度感染和轻度感染患者HDL-C分别为
(0.82±0.36)mmol/L和(0.99±0.42)mmol/L,明显低于未感染患者(P<0.05),而LDL-C分别为(3.83± 1.10)mm ol/L和(3.31±1.03)mmol/L,明显高于未感染患者(P<0.05).冠心病不同HP感染患者中不同冠状动脉病变比例比较未见统计学差异(P>0.05).结论 HP感染与冠心病本身没有相关性,但HP感染可能影响机体脂质代谢而促进冠心病发展.%Objective To investigate the relationship between Helicobacter pylori (HP) infection and serum lipid metabolism in patients with coronary heart disease.Methods From May 2013 to May 2016,selected
150 patients with coronary heart disease (CHD group),while 100 cases of healthy persons as control group,HP infection was detected by 14C urea breath test,according to the results,patients with coronary heart disease were divided into non infection (n=76),mild infection (n=53) and severe infection (n=21),according to SYNTAX score,patients were divided into low risk,medium risk and high risk patients,measured each subjects serum lipid levels.Results The infection rates of HP in CHD group and control group were 49.33% and 41.00%,there was no significant difference(P>
0.05).TC,TG and LDL-C in CHD group were(5.50±0.81)mmol/L,(2.31± 0.52) mmol/L and (3.70±0.83)mmol/L,significantly higher than that in control group,while HDL-C was (0.93± 0.27) mmol/L,significantly lower than that in control group,the difference was statistically significant (P<0.05).Severe infection patients TC and TG were (6.11 ± 1.09) mmol/L and (2.53±0.31) mmol/L,significantly higher than that of non infection and mild infection patients (P<0.05).Severe infection and mild infection patients HDL-C were (0.82±0.36)mmol/L and (0.99±0.42)mmol/L,significantly lower than that of non infection patients (P<0.05),and LDL-C were (3.83±1.10)mmol/L and (3.31±1.03)mmol/L,significantly higher than those in non infection patients(P<0.05).Different proportions of coronary lesions in patients with different HP infection difference was no statistical significance (P>
0.05).Conclusion HP infection is not associated with coronary heart disease,but HP infection may promote the development of coronary heart disease by affecting the body lipid metabolism.
