【摘要】Objective To investigate the value of heart-type fatty acid binding protein,troponin T, high-sensitivity C-reactive protein combined detection in acute myocardial infarction. Methods 70 cases with cute myocardial infarction in our hospital were selected, the heart-type fatty acid binding protein, troponin T,high-sensitivity C-reactive protein were collected and detected.Observed the test results,compared the diagnosis levels of acute myocardial infarction with single and three joint detection. Results After admission 3h,H-FABP positive rate of 97.1%,significantly higher than the positive rate of hs-CRP and cTnT(P<0.05);cTnT,hs-CRP were 24-48h after the onset peaked at 72h after the onset of hs-CRP,H-FABP,cTnT positive rate was significantly reduced.Single test,the positive predictive value of H-FABP,the highest specificity (96.5%,96.0%),specificity of joint detection,positive predictive values were reached 100.0%, while the sensitivity and negative predictive value of 97.8%. Conclusion Heart-type fatty acid binding protein,troponin T,high-sensitivity C-reactive protein combined detection for the diagnosis of acute myocardial infarction has a high positive predictive value and specificity,can effectively reduce the
misdiagnosis rate,should be widely applied.%目的：探讨心型脂肪酸结合蛋白、肌钙蛋白T、超敏C反应蛋白检测在急性心肌梗死中的诊断价值。

方法选取我院收治的70例急性心肌梗死患者作为研究对象，采集患者入院后不同时刻的静脉血进行心型脂肪酸结合蛋白、肌钙蛋白T、超敏C反应蛋白检测，观察检测结果，比较单项检测和三项联合检测诊断急性心肌梗死的水平。

结果在入院后3h，H-FABP
阳性率为97.1%，显著高于hs-CRP和cTnT的阳性率（P＜0.05）；cTnT、hs-CRP均在发病后24～48h达到峰值，在发病后72h，hs-CRP、H-FABP、cTnT
阳性率均明显降低。

单项检测中，H-FABP的阳性预测值、特异度最高（96.5%、96.0%），联合检测的特异度、阳性预测值均达到了100.0%，同时敏感度、阴性预测值达97.8%。

结论心型脂肪酸结合蛋白、肌钙蛋白T、超敏C反应蛋白联合
检测，对于诊断急性心肌梗死具有较高的阳性预测值和特异度，能有效降低误诊率，值得推广应用。
