ICU呼吸机相关性肺炎发生原因及预防措施

【摘要】目的：探讨ICU机械通气患者发生呼吸机性相关肺炎(VAP)的影响因素，以及预防护理措施。

方法抽选我院2012年6月~2015年6月收治的ICU机械通气患者200例，回顾分析患者的一般资料和治疗方法，获取呼吸道分泌物进行细
菌培养，观察病原菌分布，总结感染影响因素。

结果200例患者中发生VAP 41
例(20.5%)，病原菌分布以铜绿假单胞菌(27.3%)、金黄色葡萄球菌(18.2%)为主。

年龄≥60岁、血清白蛋白<30 g/L、呼吸机应用时间≥7 d、应用抗酸剂、抗生素、胃内容物反流患者VAP发生率高，差异有统计学意义(<0.05)。

结论 ICU机械通
气患者VAP发生率高，且相关影响因素较多。

提示医护人员加强护理管理，制定
并实施有效的预防措施，降低VAP发生率。

%Objective To discuss the ICU mechanical ventilation in patients with occurrence of ventilator associated pneumonia (VAP) factors, as well as the preventive nursing measures. Methods Selected from June 2012 to June 2015 were analyzed in 200 cases of patients with ICU mechanical ventilation, general information and treatment in patients with retrospective analysis method, bacterial culture for respiratory secretions, observe the pathogenic bacteria distribution and summarizes the influencing factors of infection. Results The occurring in 200 cases of patients with VAP in 41 cases (20.5%), distribution of pathogens in pseudomonas aeruginosa (27.3%), staphylococcus aureus
(18.2%). Age 60 years or more, serum albumin <30 g/L, breathing machine application time 7 d or higher, antacids, antibiotics, stomach contents reflux patients with VAP incidence rate is high, the difference was statistically significant ( < 0.05). Conclusion ICU mechanical ventilation in patients with VAP incidence rate is high, and the related influencing factors. Prompt medical staff to strengthen nursing management, formulation and implementation of effective preventive measures and reduce the incidence of VAP.
