软组织扩张器在修复皮肤缺损中的临床应用效果

【摘要】Objective To explore the clinical application and effectiveness of soft tissue expander in repairing of cutaneous deficiency .Methods 15 patients with cutaneous deficiency were treated with the soft tissue expand-er .In the first stage the tissue expanders were inserted into the subcutaneous pocket beside the lesion area .In the second stage the expander was taken out after the expansion area reached
requirement ,then the lesions excised ,skin flaps were made and transferred to repair skin defects .Results the flaps in the 15 cases were survived after operation and all wound surfaces was primary healing ,the duration of expanding is 6-10 weeks .No complica-tions had happen such as hematoma ,infection ,wound dehiscence ,expander exposure and leakage .All of the le-sion area were covered and healed well after the expanded skin flap transplantation .15 patients were followed up from 5 months to 2 year with an average of 14 months after operation .The long-term results showed that the flaps were smooth and glossy and color and texture of the flaps matched the recipient sites well without swelling and pig-mentation .The appearance of the repaired sites looked satisfactory .Conclusion The tissue expansion is a better option for repairs
of some skin and soft tissue defects after the lesion excised in certain areas .%目的：探讨软组织扩张器在修复皮肤缺损中的临床应用及疗效。

方法对15例皮肤缺损患者，一期将软组织扩张器置入病变区附近的皮下进行扩张，达到所需的皮肤面积时，二期取出扩张器，切除病损，设计皮瓣，修复皮肤缺损创面。

结果本组15例术后扩张皮瓣均成活，创面Ⅰ期愈合。

扩张时间为6～10周，无血肿、感染、切口裂开、扩张器外露及渗漏等并发症发生。

随访5月～2年，平均14月，修复的皮瓣光滑平整，质地柔软，无臃肿，肤色与周围皮肤相近，无色素沉着。

结论软组织扩张器是修复皮肤缺损的一种可行有效方法，可获得功能和美学双重修复的满意结果。
