管状胃对食管癌术后胃食管返流的影响

【摘要】Objective To discuss the clinical influence of tube gastroesophagostomy on gastroesophageal re-flux in patients with esophageal carcinoma. Methods Eighty-six patients with esophageal carcinoma were divided in-to the observation group (n=43) and the control group (n=43), which all received radical resection. After operation, the observation group was given tube gastroesophagostomy, while the control group was given whole gastroesopha-gostomy. Then the gastroesophageal reflux status was compared between the two groups. Results Episodes of gastric acid reflux in 24 h, Episodes of gastric acid reflux more than 5 min were less in the observation group than the control group, and the difference was statistically significant (P<0.01). The longest acid reflux time, total time of pH<4 were shorter in the observation group than the control group, and the difference was statistically signifi-cant (P<0.01). RDQ scores 1, 3 months after operation were significantly lower in the observation group than the control group (P<0.01), and the gastroesophageal reflux rate was also significantly lower in the observation group (P<0.01). Conclusion Tube gastroesophagostomy can effectively reduce postoperative gastroesophageal reflux de-gree, reduce
gastroesophageal reflux rate in patients with esophageal cancer. It has important clinical significance to improve the quality of life in postoperative patients.%目的：探讨管状胃对食管癌术后患者胃食管返流的影响。

方法86例食管癌患者随机分为观察组和对照组各43例，均接受食管癌手术治疗。

观察组给予管状胃代食管吻合术，对照组给予全胃代食管吻合术，比较两组患者术后的胃食管返流情况。

结果观察组患者24 h酸返流次数、≥5 min酸返流次数均
少于对照组，差异均有显著统计学意义(P<0.01)；最长酸返流时间、pH＜4总时
间均短于对照组，差异均有显著统计学意义(P<0.01)；观察组患者术后1个月、3个月时的RDQ评分均低于对照组，且胃食管返流发生率也低于对照组，差异均有显著统计学意义(P<0.01)。

结论管状胃用于食管癌患者术中食管重建能够有效减
轻患者术后的胃食管返流程度，降低胃食管返流发生率，对于患者术后生活质量的提高具有重要临床意义。
