度普利尤单抗治疗1例重度嗜酸性粒细胞型哮喘患者的个体化处理

度普利尤单抗治疗1例重度嗜酸性粒细胞型哮喘患者的个体化处理明丹丹1, 2金美玲3 李晓宇1叶晓芬1［1. 复旦大学附属中山医院药剂科上海 200032；2. 中山大学附属第五医院药学部广东珠海519000；3. 复旦大学附属中山医院变态（过敏）反应科上海 200032］摘要1例重度嗜酸性粒细胞型哮喘患者，血清免疫球蛋白E水平正常，外周血嗜酸性粒细胞计数增高，需长期口服糖皮质激素以控制症状。

患者曾接受奥马珠单抗治疗5次，效果不佳，后换用度普利尤单抗，哮喘症状明显改善，但出现外周血嗜酸性粒细胞计数进一步增高的不良反应。

停用度普利尤单抗，外周血嗜酸性粒细胞计数下降，但哮喘症状复又加重。

综合评估患者病情并权衡疾病加重和药物治疗风险后，决定继续使用度普利尤单抗治疗，但采用个体化的治疗方案，密切监测患者病情和外周血嗜酸性粒细胞计数，及时调整度普利尤单抗治疗的间隔时间，效果颇佳。

Department of Allergy,Zhongshan Hospital, Fudan University, Shanghai 200032, China)ABSTRACT A patient with severe eosinophilic asthma had normal serum immunoglobulin E level and elevated peripheral blood eosinophil count, to whom long-term oral glucocorticoid therapy was required so as to control symptoms. The patient received omalizumab for 5 times with poor results, and then asthma symptoms were significantly improved after switched to dupilumab, but there were adverse reactions of further increase in peripheral blood eosinophil count. Peripheral eosinophil count decreases when dupilumab was discontinued, but asthma symptoms relapsed. It was decided to continue to use dupilumab treatment after comprehensively assessing the condition and weighing the risk of exacerbation and medication. However, the individualized treatment plan was adopted, the disease condition and peripheral blood eosinophils were closely monitored and the interval of d upilumab treatment was adjusted in time and the effect was quite good.KEY WORDS severe eosinophilic asthma; dupilumab; eosinophilia本文通过1例度普利尤单抗治疗的重度嗜酸性粒细胞型哮喘病例，探讨如何解决因药物不良反应导致的药物治疗矛盾，以期为此类患者临床治疗提供参考。

58中国处方药第19卷第1期·医院药学·药物相关问题（drug-related problems，DRPs）是指药物治疗过程中出现的实际或潜在干扰预期治疗结果的事件或情况，包括药物不良反应、药物不良事件等[1]。

因疾病及患者本身的多样性，药物治疗方案也相对复杂，导致了很多DRPs的出现，大幅增加医疗支出。

美国的一项回顾性研究显示，DRPs的发生率高达61.0%～63.7%[2-3]。

1995年，美国学者 Johnson和Bootman[4]用自己设计的疾病成本模型评估得出，美国每年与此有关的花费高达766亿美元。

Ernst和Grizzle[5]在5年后，用该模型再次计算，发现这些费用已经翻了一番，达到1 774亿美元，某院药物相关问题分类系统的建立及初步验证黎晓亮，宋柳全＊，伍黎芳（梧州市工人医院药学部，广西梧州 543001）【摘要】目的对现有药物相关问题分类系统进行改进，在此基础上建立某院药物相关问题分类系统，为更好地监控药物相关问题，促进患者用药安全及节省医疗费用提供帮助。

方法在对公开发表的DRPs分类系统进行分析汇总的基础上，选择一相对较完善的系统，结合某院的实际情况，建立适合某院的药物相关问题分类系统，并进行初步验证。

结果建立的新系统包含5个部分，75个子类别。

结论新的分类系统尽管还有许多需要优化的地方，但参与评估的药师都给予该系统较高的评价，认为新的分类系统更切合某院的实际情况。

有利于发挥药师在患者治疗中的作用，为患者提供更优良的药学服务。

方法:随机抽查2006年1月～12月1年间的急诊处方5000张,就不合理用药进行分析。

结果:筛选出不合理处方367张,占总数的7134%,分别表现在重复用药、选药不当、联合用药品种过多、配伍不合理、用法、用量不当和用药禁忌等方面。

结论:药师需深入临床,指导临床合理用药。

方法计算机检索中国生物医学文献数据库、中国学术期刊网络出版总库、中文科技期刊数据库、中国重要会议论文全文数据库和PubMed 、EMbase ，并以Google 辅助检索。

纳入循证药学相关文献，提取循证药学定义、作者信息、发表年限及研究主题等信息。

描述性分析现有循证药学定义异同，评价循证药学学科发展现状。

结果 ① 循证药学定义：共获得4个定义，各自就实践主体、实践方法和实践领域做出不同描述。

② 循证药学相关研究：共获得循证药学相关文献214篇（英文12篇，中文202篇），中文文献数量逐年递增，近3年以每年10篇的速率增长；作者主要来自医疗机构（以药剂科最多）；作者研究方向以临床药学和医院药学为主；研究主题以临床药学实践与发展、临床合理用药和医院药学发展为主。

结论 ① 现有循证药学定义差异较大，尚缺乏公认定义。

② 循证药学在国际上知晓度和关注度较低，在国内正日益受到关注。

③ 循证药学学科缺乏完整理论体系，应尽快完善循证药学知识体系，探索和构建循证药学实践模式。

Methods We searched CBM, VIP, CNKI, PubMed, EMbase, and Google for supplement. We extracted defi nitions of evidence-based pharmacy, types of studies, published year, topics of studies, and the author information from the included studies. We compared the defi nitions on diff erent domains, and assessed the current development of evidence-based pharmacy literature. Results a) Th e defi nition of evi-dence-based pharmacy: We found 4 defi nitions, and there were great diff erences among them on the people who practiced evidence-based pharmacy as well as the methods and areas of the practice. b) Th e evidence-based pharmacy literature: 12 English and 202 Chinese articles were included in the review. Th e number of Chinese evidence-based pharmacy articles was increasing annually, especially in recent 3 years; Most of their authors were from hospitals, especially from pharmacy department; Th eir study orientations mostly involved clinical pharmacy and hospital pharmacy; Th e topics mainly refered to the practice and development of clinical pharmacy, rational drug use, and development of hospital pharmacy. 方法：抽取2020年1月—2021年6月某院使用烟酰胺的出院病历资料，采用回顾性分析方法对患者一般情况、科室用药排名、不良反应及不合理用药等进行分析。

结果：烟酰胺的用药人群以中老年人居多，不良反应发生率低且轻微，用药科室以心血管内科、肿瘤内科等为主。

不合理用药以无适应证用药为主，其他包括用药剂量偏小、溶媒选择不适宜、用药疗程偏长等。

结论：某院烟酰胺在临床应用中存在诸多不合理现象，医院应加强辅助用药管理，持续提升合理用药水平。

Results: The population of nicotinamide use was mostly middle-aged and elderly, and the incidence of adverse reactions was low and mild, mainly in cardiovascular medicine and medical oncology. 临床药师参与1例莫西沙星联用华法林致INR升高的药学监护陈泳伍1*叶晓芬2**蔡映云2吕迁洲2（1.安徽省立医院药剂科合肥 230001；2.复旦大学附属中山医院药剂科上海 200032）摘要目的：分析莫西沙星与华法林之间的相互作用，促进临床合理用药。

方法：通过对1例房颤合并肺部感染患者在抗感染治疗过程中INR升高的原因分析及药学监护，探讨莫西沙星与华法林发生相互作用机制。

结果：莫西沙星可能通过竞争结合华法林蛋白结合位点引起游离的华法林短暂升高，从而增强华法林的抗凝作用。

结论：对于合并用药种类较多的华法林治疗患者，应密切监测凝血功能，临床药师参与临床药物治疗有助于发现药物相互作用，优化药物治疗方案。

Methods: The possible mechanism of moxifloxacin-warfarin interaction was investigated by analysis of the causes of elevated INR during anti-infection treatment in a case of atrial fibrillation complicated with pulmonary infection and pharmaceutical care. Results: Moxifloxacin may cause a transient increase in free warfarin by competing with warfarin protein-binding sites so as to increase the warfarin’s anticoagulant effect. Conclusion: Blood coagulation should be closely monitored in patients treated with warfarin in combination with other types of drugs. ▲通讯作者老年人药物代谢动力学、药效学和药动学都会发生变化,可因药物蓄积而引发不良反应;同时老年人往往有多重用药,药物相互作用会进一步增加不良反应的发生率,老年人的用药安全和临床合理用药问题引起了广泛关注。

潜在不适当用药(potentially inap⁃propriate medication,PIM)最早由Beers 等[1]提出,是指药物有效性尚未确立和/或药物发生不良反应事件风险超过预期临床获益,同时缺少较安全的可替代药物[2]。

国外使用比较广泛的是Beers 标准、老年人处方筛查工具(screening tool of older persons’prescription,STOPP)/老年人处方遗漏筛查工具(screening tool to alert to right treatment,START)标准,这两个标准都经过了多次更新[3-8];《中国老年人潜在不适当用药判断标准》(以下简称“《中国标准》”)由我国于2017年发布[9-11]。

本文拟对Beers 标准(2019版)、STOPP/STAR 标准(2014版)及《中国标准》进行比较,为临床用药提供参考。

1三个标准的概况Beers 标准已经历5次更新,是目前应用最广泛的老年人PIM 评价工具[2-6],2019版Beers 标准提出了更清晰全面的老年人用药方案,包括30种老年人避免使用的药物,40种在某些疾病或综合征下避免使用或慎用的药物,并重新梳理了16项标准。

STOPP/START 标准是用于指导老年患者用药的老年人处方筛查和处方遗漏筛查工具[7-8]。

医院药学部党建工作与业务工作深度融合的探索与实践

2017年10月起新疆维吾尔自治区公立医院统一执行取消15%药品加成的政策,这项重大改革使得公立医院药学学科面临前所未有的挑战,医院药学部门从医院的利润部门转变为成本部门,药师队伍仍存在人才专业分工不细、专业素养急需提高的问题,医院对药学部门的投入可能会减少,而相关如药事服务费、药师立法等政策支持力度仍不足。

ICU脓毒症患者应用哌拉西林

ICU 脓毒症患者应用哌拉西林/他唑巴坦药物浓度的影响因素分析洪曦菲林雪朱忠华郑绍鹏［摘要］　目的　探究影响重症医学科（ICU ）内脓毒症患者应用哌拉西林/他唑巴坦治疗后血浆哌拉西林浓度的因素。

方法　回顾性收集2020年8月至2022年3月黄山市人民医院ICU 患有脓毒症，并接受哌拉西林/他唑巴坦常规剂量4.5 g （哌拉西林4 g/他唑巴坦0.5 g ）静脉滴注，每8小时输注1次，同时监测哌拉西林浓度的患者82例，根据治疗药物监测（TDM ）情况分为达标组（n =34）与未达标组（n =48），采用多因素logisitic 回归分析筛选哌拉西林浓度不足的影响因素，通过受试者工作特征（ROC ）曲线分析危险因素对血药浓度不足的预测价值。

结果　单因素分析显示，年龄、身体质量指数（BMI ）、外伤、手术、急性生理学与慢性健康状况评分Ⅱ（APACHE Ⅱ）、序贯器官衰竭（SOFA ）评分、肌酐清除率（CrCl ）与哌拉西林药物浓度不足差异均有统计学意义（P ＜0.05）。

多因素lo⁃gistic 回归显示：BMI （OR =1.611，95%CI ：1.069～2.427）、CrCl （OR =1.249，95%CI ：1.059～1.474）是哌拉西林浓度不足的独立危险因素（均P ＜0.05）。

BMI 预测哌拉西林浓度不足的曲线下面积（AUC ）为0.784，最佳截断值为20.78 kg/m 2，灵敏度为83.3%，特异度为73.5%；CrCl 的AUC 为0.975，最佳截断值为41.62 mL/min ，灵敏度为97.9%，特异度为88.2%。

结论　对于ICU 脓毒症患者，BMI 和CrCl 是哌拉西林药物浓度不足的独立危险因素，二者与药物浓度呈负相关，都有很好的预测价值，而CrCl 对哌拉西林药物浓度不足的预测价值更高。

疫苗处方审核、登记接种信息：疫苗在药品范畴内［13］,药师有审核疫苗处方的责任。疫苗接种后，需要药师即刻登记接种人群信息(个人信息、既往接种情况、本次接种情况等)，方便患者下次接种查询。最早提出药师参与疫苗管理的阿根廷卫生部门要求药师接种疫苗后必须记录接种信息［1］;自2015年起，瑞士沃德州要求药师利用电子系统记录疫苗接种信息,同时药房需保存该信息至少8年［14］；2016年，《澳大利亚药学指南》(《The Pharmacy Guild of Australia》)规定药师对患者接种疫苗后需保留接种记录数年并登记电子信息［15］。
