【摘要】Objective To establish a method for detection of platelet-associate autoantibodies against platelet-specific receptors using flow cytometric bead array , and evaluate the effect of idiopathic thrombocytopenic purpura( ITP )treating with glucocorticoid. Methods The platelets from blood samples of 67 patients and 30 healthy controls were isolated and lysed,which were hatched with coated antigen microspheres and added PE labeled polyclonal antibody,The effect of therapy with glucocorticoid in ITP group were evaluated. Results The fluorescence of antibody of platelet GPⅡb/Ⅲa and GPⅠb/Ⅸ in ITP group were 4. 58( 0. 22 - 30. 20 ),4. 78( 0. 56 - 22. 00 ),while in controls were 0. 92( 0. 23 -2. 28 ),0. 87( 0. 22 -2. 13 ),respectively. The intensity ratios of fluorescence was set as positive, that was CD41a > 0. 025% and CD42 >2. 12% ,two platelet antibodyies were detected by flow cytometric bead array simultaneously. The sensitivity was 95. 5% , and the specificity was 90. 9% ,positive predictive value was 96. 7% . A negative correlation was found between antibody detection results and the effect of glucocorticoid-therapy( r = -0. 318 ). Conclusion The way of flow cytometric bead to detect platelet-associate autoantibodies against platelet-specific has high
sensitivity and specificity in ITP's laboratory diagnosis and has a good curative effect.%目的建立流式微球技术检测血小板膜表面糖蛋白特异性自身抗体(GPⅡb/Ⅲa、GPⅠb/IX),探讨其在特发性血小板减少性紫癜(ITP)患者体内表达情况及其与激素治疗效果之间的关系.方法取67例ITP患者及30例健康对照组的外周血,分离、裂解血小板与单抗包被微球共同孵育后加入PE标记多克隆抗体,流式细胞仪分析,评价ITP患者激素治疗效果.结果 ITP患者血小板GPⅡb/Ⅲa、GPⅠb/IX自身抗体荧光强度为4.58(0.22~30.20)、4.78(0.56~22.00),对照组为0.92(0.23~2.28)、0.87(0.22~2.13).以微球荧光强度比率(测定/对照)CD41a>0.025%、CD42>2.12%定为阳性,用流式微球技术同时检测两种抗血小板抗体,诊断ITP敏感性为95.5%(64/67)、特异性为90.9%,阳性预测值为96.7%.ITP患者激素治疗效果与抗体检测结果呈负相关(r=-0.318).结论流式微球技术检测血小板膜糖蛋白自身抗体对ITP诊断的敏感性、特异性较高,对治疗及预后也有一定临床指导意义.
