血清标志物与糖尿病周围神经病变的相关性

【摘要】目的探讨血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)、超敏C反应蛋白(hs-CRP)与糖尿病周围神经病变的相关性.方法分别选取50例和55例单纯糖尿病和糖尿病周围神经病变患者作为糖尿病组和周围神经病变组,另选取60例健康志愿者作为健康对照组.检测并比较各组研究对象血清Hcy、Cys C、hs-CRP水平,并对患者行神经电生理检查,采用Pearson线性相关模型分析周围神经病变组患者血清Hcy、Cys C、hs-CRP水平和上述检查结果的相关性.结果周围神经病变组和糖尿病组血清Hcy和hs-CRP水平均远高于健康对照组(P<0.05),且糖尿病组血清Hcy 和hs-CRP水平均高于健康对照组(P<0.05);糖尿病组和健康对照组血清Cys C水平比较,差异无统计学意义(P>0.05),均显著低于周围神经病变组(P<0.05);糖尿病组和周围神经病变组正中神经(MCV)、腓肠神经(SCV)、运动转导波幅、正中神经F 波潜伏期水平均低于健康对照组(P<0.05),且周围神经病变组各项指标检查结果均低于糖尿病组(P<0.05);周围神经病变组血清Hcy和hs-CRP水平均和神经电生理检查各项指标存在明显负向相关性(P<0.05),而血清Cys C无明显相关性(P>0.05).结论糖尿病周围神经病变患者血清Hcy和hs-CRP水平与MCV、SCV、运动转导波幅、正中神经F波潜伏期均存在负向相关性.%Objective To analyze the relationship between the serun levels of homocysteine(Hcy),cystatin C(Cys C),high-sensitivity C reactive protein(hs-CRP) and diabetic peripheral neuropathy.Methods Fifty cases of patients with single diabetes and fifty-
five cases of patients with diabetic peripheral neuropathy were respectively selected as the diabetic group and peripheral neuropathy group,and another sixty cases of healthy volunteers were noted as the normal control group.The serum levels of Hcy,Cys C,hs-CRP were detected and compared among the groups,then the nerve electrophysiological examination was conducted.The relationships between the levels of serum Hcy,Cys C,hs-CRP in patients with diabetic peripheral neuropathy and the examination results were analyzed by Pearson linear correlation model.Results The levels of serum Hcy and hs-CRP in the peripheral neuropathy group and the diabetic group were far higher than those in the normal control group(P<0.05),which in the diabetic group were far higher than those in the normal control group(P<0.05).The difference in the levels of serum Cys C was not statistically significant(P>0.05),which were significantly lower than that in the peripheral neuropathy group(P<0.05).The levels of median nerve MCV,the gastrocnemius nerve SCV,motor conduction amplitude,median nerve F wave latency in the diabetic group and peripheral neuropathy group were far lower than those in the normal control group(P<0.05),which in the peripheral neuropathy group were far lower than those in the diabetic group(P<0.05).There were obviously negative relationships between the levels of serum Hcy and hs-CRP and the indexes of nerve electrophysiological examination in patients with diabetic peripheral neuropathy(r<0,P<0.05),while there was no obvious relationship between the level of serum Cys C and the
indexes(P>0.05).Conclusion There are negative relationships beyween the
levels of serum Hcy and hs-CRP and the indexes of median nerve MCV,the gastrocnemius nerve SCV,motor conduction amplitude,median nerve F wave latency in patients with diabetic peripheral neuropathy.
