经现场检查有如下问题:模板:1)、顶板模板风化存在起皮现象,个别接缝处起鼓变形严重;2)、加固梁侧模板、柱模板使用的对拉丝松动。

3)、A-D 轴交1-21轴个别柱模板整体下沉15cm左右。

钢筋:1)、板筋、梁筋、柱筋、马凳筋生锈严重;2)、所有钢筋绑丝生锈腐蚀,一碰就断;3)、柱箍筋因扎丝腐蚀,全部滑落到柱根部。

针对以上问题根据相关施工规制定专项整改方案。

第三章、针对现场情况采用:对模板架体进行加固,柱模板更换、梁板柱钢筋人工钢丝刷除锈如下:1、钢筋工程首先组织钢筋工对梁、板钢筋重新绑扎牢固并将垃圾清理干净,然后采用人工钢丝刷方法进行除锈(因梁板模板不拆除故梁底部钢筋不用除锈及绑扎)。

除锈合格后再将钢筋垫块马凳按照施工规放置到位。

柱钢筋进行人工钢丝刷除锈后将所有箍筋按照设计要求绑扎到位。

2、模板加固2#保鲜库地上一层(除1-7轴交E-M轴)轴对支撑系统进行加固,局部模板修补,对所有柱模板进行更换。

支撑系统加固:1)、梁底所有小横杆采用钢管加顶丝进行回顶,间距不大于900mm;2)、顶板模板接缝变形较小部位采用钉子固定并粘贴海绵条;3)、对于起皮严重的顶板模板将油毡剪成条铺设到顶板上用钉子固定;4)、梁侧变形严重的模板拆除,重新安装同原规格的模板加固;5)、安排专人对所有对拉丝、钢管扣件及顶丝逐一检查,对发现松动的及时进行加固处理;6)、对于所有柱模板拆除钢筋除锈绑扎完成后重新加固,并检查其垂直度。

7)、外脚手架拆除后重新按要求搭设。

3、质量责任根据甲方下发的《钢筋除锈,加固专项会议纪要》第一条保鲜库区一层模板不拆除,但必须进行加固。

钢筋进行除锈,从新绑扎。

脚手架加固。

外架拆除后,从新搭设外架。

但在浇筑前必须进行加固处理。

我单位已针对此会议纪要对钢筋、模板进行了除锈加固整改。

针对以上事宜结合施工现场实际情况有以下事项不能保证钢筋混凝土的质量,对此产生的质量安全事故与我单位无关。

1>梁底部钢筋无法除锈且不能与箍筋绑扎到一起造成钢筋与混凝土的握裹力达不到设计要求。

浇筑混凝土后其承载力、抗拉抗剪力达不到设计要求。

2>由于钢筋锈蚀较严重除锈后钢筋的直径、屈服强度及抗拉(抗压)强度均不能满足设计要求。

3>由于采用人工钢丝刷除锈钢筋与混凝土的握裹力达不到设计要求。

4>现有顶板模板大部分已开裂,如果不拆除浇筑混凝土后严重影响楼板的设计厚度及观感效果,不能满足其承载荷载值。

5>经检查2#保鲜库地上一层(除1-7轴交E-M轴)模板标高普遍低于设计标高2-5cm。

第四章、施工准备1、技术准备(1)组织施工技术人员在施工前认真学习技术规、标准、工艺规程,熟悉图纸,了解设计意图,核对建筑和结构及土建与设备安装专业图纸之间的尺寸是否一致。

(2)对施工队进行技术交底。

(3)对施工人员进行安全和技术培训,加强班组的技术素质。

2、物资准备1)、材料准备确保材料质量合格,货源充足,按材料进场计划分期分批进场,并按规定地点存放,做好遮盖保护。

同时对各种进场材料进行抽检试验并附有新钢管应有产品质量合格证;2)、机具准备根据施工机具需用量计划,做好机械的租赁和购买计划,并做好进场使用前的检验、保养工作,确保运转正常。

3)、周转材料准备做好模板、碗扣、钢管、U托等周转料的备料工作,分批分期进场。

3、劳动力准备(1)根据开工日期和劳动力需要量计划,组织工人进场,并安排好工人生活。

水、电管线架设和安装已完成,能够满足工程施工及工程管理、施工人员生活的用水、用电需要。

(2)做好施工人员进场的安全、质量、防火、文明施工等教育工作,进行岗前培训,对关键技术工种必须持证上岗,按规定进行三级安全技术交底,交底容包括:施工进度计划;各项安全、技术、质量保证措施;质量标准和验收规要求;设计变更和技术核定等。

必要时进行现场示,同时健全各项规章制度,加强遵纪守法教育。

第五章、主要施工方法1、统一要求1)、模板组拼模板组装要严格按照模板配板图尺寸拼装成整体,模板在现场拼装时,要控制好相邻板面之间拼缝,两板接头处要加设海绵条,以防漏浆,拼装完成后用钢丝把模板和竖向钢管绑扎牢固,以保持模板的整体性。

拼装要求详见下表。

2)、模板定位(1)中心线和位置线的放线首先引测建筑的边柱或墙轴线并以该轴线为起点,引出每条轴线。

根据轴线位置弹出用墨线弹出模板的线、边线以及外侧控制线,施工前三线必须到位,以便于模板的安装和校正。

(2)标高测量引测楼层500mm 标高控制线,并根据该500mm 线将板底的控制线直接引测到墙柱子上。

(3)找平工作模板承垫底部应预先找平,以保证模板位置正确,防止模板底部漏浆。

可用1:3水泥砂浆抹找平层。

2、柱模板1)、柱模板设计及布置原则本工程框架柱截面尺寸包括:900×900、850×850、800×800、750×750。

正方形及矩形框架柱模面板采用12mm厚覆面木胶合板,次楞为30×70mm 矩形木楞,柱箍Φ48×3.0mm双钢管,柱外用M14高强对拉螺栓外拉结。

柱子模板布置原则:①次楞间距不得大于200mm;②主楞按450mm一道布置(不得大于500mm),第一道柱箍在距结构面200㎜处,最上侧一道柱箍距梁底不大于300mm;③柱对拉螺杆按照均分相应柱边布置(如下图)。

3、板模板1)、模板及支撑体系的选择及设计本工程地上板厚为。

(1)模板采用12mm 厚木胶合板,采用梁侧模包底模,楼板模压梁侧模法施工。

(2)板支撑体系大部分采用碗扣脚手架(A-D轴交1-21轴采用钢管扣件脚手架)支撑系统,满堂布置。

板底模下设30mm×70mm木枋作为次龙骨,其中心间距为250mm。

主龙骨为Φ48mm×3.0mm钢管,间距900mm。

(3)扫地杆距地面高度不得大于200mm,立柱自由端长度不得大于500mm 2)、满堂架加固本工程框架柱间距主要为8400mm,梁间间距为6000×4200mm以下图为一个平面图3、梁模板:对于变形严重的量侧模板进行更换1)梁区中现浇楼板的起拱,除按设计要求起拱外,还应将整块楼板的支模高度上提5mm,确保混凝土浇筑后楼板厚度和挠度满足规要求。

2)梁底支撑间距应能保证在混凝土重量和施工荷载作用下不产生变形。

3)在梁模与柱模连接处,应考虑模板吸水后膨胀的影响,其下料尺寸一般应略微缩短些,使混凝土浇筑后不致模板嵌入混凝土。

4)要注意梁模与柱模的接口处理、主梁楼板与次梁模板的接口处理,以及梁模板与楼板模板接口处的处理,谨防在这些部位发生漏浆或构件尺寸偏差等现象。

5)用钢管连接并夹紧梁侧模板,位置及间距同前面所述要求。

安装水平向钢管背楞之后安装拉杆(模板为竖拼时的做法)。

6)按楼板尺寸,铺设楼板模板,从一侧开始铺设多层板,尽可能选用整的,并且是经包边角处理的多层板,余下尺寸再需裁切,以利于多次周转使用。

7)楼板模板的接缝处理:模板与模板接缝处,一是要保证两块模板的高度差不能太大。

二是要保证接缝的严密,也就是保证砼不漏浆。

为了达到这两个目的,须在接缝处模板下垫木枋,通过木枋校正两块模板的高差,并在接缝处形成构造密封,可有效防止漏浆。

第六章、质量标准及技术控制措施第一节进场模板的质量标准(1)外观质量检查标准(通过观察检验)任意部位不得有腐朽、霉斑、鼓泡。

不得有板边缺损、起毛。

每平方米单板脱胶不大于0.001m2。

每平方米污染面积不大于0.005m2。

(2)规格尺寸标准检查厚度检测方法:用钢卷尺在距板边20mm 处,长短边分别测3点、1点,共取8点计算平均值;各测点与平均值差为偏差。

长、宽检测方法:用钢卷尺在距板边100mm 处分别测量每板长、宽各2点,取平均值。

对角线差检测方法:用钢卷尺测量两对角线之差。

翘曲度检测方法:用钢直尺量对角线长度,并用楔形塞尺(或钢卷尺)量钢直尺与板面间最大弦高,后者与前者的比值为翘曲度。

第二节模板分项工程质量要求必须符合《混凝土结构工程施工及验收规》(GB 50204-2002 )及相关规要求。

1一般规定(1)模板及其支架应根据工程结构形式荷载大小地基土类别施工设备和材料供应等条件进行设计模板及其支架应具有足够的承载能力刚度和稳定性能可靠地承受浇筑混凝土的重量侧压力以及施工荷载(2)在浇筑混凝土之前应对模板工程进行验收模板安装和浇筑混凝土时应对模板及其支架进行观察和维护发生异常情况时应按施工技术方案及时进行处理(3)模板及其支架拆除的顺序及安全措施应按施工技术方案执行2模板安装2.1主控项目(1)安装现浇结构的上层模板及其支架时,下层楼板应具有承受上层荷载的承载能力,或加设支架;上下层支架的立柱应对准,并铺设垫板。

检查数量:全数检查。

检验方法:对照模板设计文件和施工技术方案观察。

(2)在涂刷模板隔离剂时,不得沾污钢筋和混凝土接槎处。

检查数量:全数检查。

检验方法:观察。

2.2一般项目(1)模板安装应满足下列要求:模板的接缝不应漏浆;在浇筑混凝土前,木模板应浇水湿润,但模板不应有积水;模板与混凝土的接触面应清理干净并涂刷隔离剂,但不得采用影响结构性能或妨碍装饰工程施工的隔离剂;浇筑混凝土前,模板的杂物应清理干净;检查数量:全数检查。

检验方法:观察。

(2)对跨度不小于4m 的现浇钢筋混凝土梁、板,其模板应按要求起拱。

当设计无具体要求时,起拱高度宜为跨度的1/1000~3/1000检查数量:按规要求的检验批,在同一检验批,对梁,应抽查构件数量的10%,且不应少于3件;对板,应按有代表性的自然间抽查10%,且不得小于3间。

对大空间结构板可按纵横轴线划分检查面抽查10%且不少于3面;检验方法:水准仪或拉线、钢尺检查。

(4)固定在模板上的预埋件、预留孔洞均不得遗漏,且应安装牢固,其偏差应符合表8-3的规定。

检查数量:按规要求的检验批,在同一检验批对梁、柱,应抽查构件数量的10%,且不应少于3间;对大空间结构,墙可按相邻轴线间高度5m左右划分检查面,板可按纵横轴线划分检查面,抽查10%,且不得小于3面。
