INSTRUCTIONS

一、收养人情况

Basic information of adoptive parents

1、由收养人据实填写，姓名、出生日期应该与护照或其他有效身份证件一致。

This information should be filled out truthfully by the adoptive parents. Name and DOB should be consistent with that shown in passports or other ID certificates.

2、“身份证件号”，填写申请收养人护照号或其他有效身份证件号。

For "ID No.", please fill in the passport number or number of other ID certificates.

3、“健康状况”，填写“健康”或“患病”或“残障”。

For "Health Status", please fill in "healthy", "diseased" or "disabled".

4、“婚姻状况”，填写“已婚”或“未婚”或“离异”或“丧偶”。

For "Marital Status", please fill in "married","single","divorced", or "widowed".

5、“经济状况”，填写申请收养人全年收入总和。

For "Annual income", please fill in the total annual income of the applicants.

6、“子女情况”，填写子女数量，包括：继子女、养子女、现已不在一起共同生活的子女。

For "Offspring info.", please fill in the number of children, including step child, adoptive child, and child who does not live within the family.

二、送养人情况

Basic information of the person placing out child for adoption

1、由送养人据实填写。

This information should be filled out truthfully by the person placing out child for adoption.

2、送养人是福利机构的，由法人代表在“送养人或福利机构意见”栏内签署意见。

In the case where welfare institution acts as the person placing out child for adoption, the legal person's opinion should be filled in the blank "Opinion of the person placing out child for adoption or welfare institution".

三、被收养人情况

Basic information of the adopted child

1、由送养人或福利机构据实填写。

This information should be filled out truthfully by the person placing out child for adoption or welfare institution.

2、“身体状况”，填写“正常”或“特需”。

For "Physical Status", please fill in "Healthy" or "Special Needs".

四、收养登记审批情况

Approval status of adoption registration application

1、由收养登记受理单位据实填写。

This information should be filled out truthfully by the entity processing the application for adoption registration.

2、被收养人情况栏中的“身体状况”，填写“正常”或“特需”。

For "Physical status" of the adopted child, please fill in "Healthy" or "Special Needs".
