4. RED SURFACES / RED LEAD
6. DUST SCREENING
“Bleeding” may occur when testing surface is painted red. However the color that rubs off a surface is often visibly different from the pink to red color that develops when a 3M™ LeadCheck™ Swab detects lead. The easiest way to test for bleeding is to CRUSH VIAL “B” ONLY, and bring a drop of the clear colorless fluid to the tip of the swab. Rub the tip of the swab on the surface. Any color that appears on the tip has “bled” from the test surface and may make reading the test results difficult.

Under controlled laboratory conditions, 3M™ LeadCheck™ Swabs will indicate the presence of lead as low as 1-2 micrograms. 方法:选取在本院行乳牙根管治疗的88例(185颗)患儿为研究对象,按随机数字表法分为观察组44例(92颗)及对照组44例(90颗);对照组患儿采用复合树脂对踽坏牙齿进行修复,观察组患儿行乳牙预成冠修复;对比两组总有效率、患儿患牙修复情况及不良情况发生率。

结果:观察组总有效率为97.33%,高于对照组8&89%,差异有统计学意义(P<0.05);观察组边缘密合率为95.65%、邻接较好率为94.57%、修复体完整率为100.00%、牙体完整率为100.00%,均高于对照组的62.22%、5&89%、64.44%及91.11%,差异有统计学意义(P<0.05);观察组不良情况发生率低于对照组,差异有统计学意义(P<0.05)。

3M ESPE 有两种不同的不锈钢预制临时牙冠边缘,以便适合于牙科医生的特殊需求。

就位后,用探针检查牙冠与预备体之间是否有缝,如有必要可以修剪牙冠边缘。

4. 检查咬合检查咬合。

摘下牙冠,再次检查上下牙咬合关系。

如果牙冠妨碍了牙列的正常咬合关系,则需要重新预备患牙,以便试戴后有正常的咬合关系。

选定Unitek 牙冠后,牙冠边缘需要修整以使牙冠顺利就位。

5. 隔离患牙隔离患牙。

冲洗患牙区,用棉卷隔湿,彻底吹干。

6. 调水门汀调水门汀,,粘固牙冠粘固牙冠。

根据生产商的提示调拌水门汀,要防止空气进入。

7. 把牙冠戴到预备体上把牙冠戴到预备体上。

和第3步相同,牙冠就位后多余的水门汀会被挤压出去。

注意注意:最后就位时,不要用大拇指加太大压力。

8. 迅速检查牙冠与邻牙接触区及咬合。

病人咬合应该在正常的自然的咬合力下,如果用棉卷等其他方法增加 咬合力分布,要切记尽可能放置在缺牙间隙近远中径。

七、冠的修剪和成形边缘长的表现:就位后边缘发白,出血,咬合高,咬合痛。

冠的自带边缘就是按照牙龈的走形,所以修剪边缘注意按照冠的边缘平行修剪,要整个一圈都剪掉,不能哪里长减哪里。

一般最多修剪0.5-1mm。

(如果使用临面缩紧钳导致的边缘过长,可以只修剪一部分)逆时针,连续修剪,注意支点,因为冠只有0.2mm,很容易修剪,但修剪后容易变形,需要使用边缘缩紧钳重新成形。

八、抛光、粘接抛光:要求不高,主要是把修剪边缘的毛刺平滑一下,用手指感受不要有毛刺。

清洁:酒精清洁。

粘接:粘接剂要够多,满。

可以使用玻璃离子,也可以为了操作时间短可以使用双固化。

注意隔湿。

九、常见问题:就位不好的可以使用邻面调整钳缩短近远中径。

如果牙冠妨碍了牙列的正常咬合关系,则需要重新预备患牙,以便试戴后有正常的咬合关系。

选定Unitek 牙冠后,牙冠边缘需要修整以使牙冠顺利就位。

5. 隔离患牙隔离患牙。

冲洗患牙区,用棉卷隔湿,彻底吹干。

6. 调水门汀调水门汀,,粘固牙冠粘固牙冠。

根据生产商的提示调拌水门汀,要防止空气进入。

7. 把牙冠戴到预备体上把牙冠戴到预备体上。

和第3步相同,牙冠就位后多余的水门汀会被挤压出去。

注意注意:最后就位时,不要用大拇指加太大压力。

8. 迅速检查牙冠与邻牙接触区及咬合。

医院儿童牙病护理常规由于儿童的解剖、生理、病理和就诊心理及行为等特点,使儿童牙病的护理有别于成人,只有充分了解儿童的特殊性,才能更好、更主动地做好儿童牙病的护理工作。

一、诊疗行为诱导到儿童牙科来就诊的患儿,大多有不同程度的恐惧感,尤其以幼儿期和学龄前期儿童多见。

表现为害怕、紧张、哭闹;不愿与医护人员交谈,不愿接受甚至拒绝治疗,消除这些不利的诊疗行为,达到完成诊疗目标的方法就是诊疗行为诱导。

这项诱导工作通常由护理人员完成,所以这也是儿童牙科特别的一项护理工作。

(一)一般患儿的诊疗行为诱导1.环境感化法:营造适合儿童特点的诊疗环境,可以减轻患儿的恐惧心理。

2. 言语交流法:要避免成人化和专业化,采用简单易懂、具体形象的形体语言和童语,而不是生硬的、命令式的语言。

例如把水枪比作淋浴器,告诉患儿这是用来给牙齿洗澡的;把气枪比作电风扇是给牙齿吹风的;把拍片检查说成是给牙齿照张相片等。

3.鼓励为主法:因患儿情感发育的特点,在口腔治疗中,出现不配合的行为是很自然的。

因此应以鼓励为主,批评为辅,交替使用来诱导患儿,对患儿的每一点进步都要及时给予表扬和鼓励。

4.适应法:许多患儿在接受口腔治疗时,由于初次接触医院的特殊环境,听到或感受到牙钻的转动声,金属器械的碰撞声,容易产生恐惧心理,可先让患儿用手摸一摸口镜、探针等诊疗器械,消除其恐惧心理后才开始检查患牙。

牙钻的试验体会:先开机,让患儿熟悉声音和喷水状况,示意患儿不会产生疼痛,最后在口内患牙上制洞,应稳、准、轻、快地完成治疗工作。

5.短时操作法:儿童生性好动,固定于某一姿势的持续时间短,操作时间宜短不宜长,尽量在短时间内完成治疗工作。

6.参观学习法:对胆小或有恐惧症的患儿,治疗前可让其参观正在接受治疗的其他小朋友的治疗情况,再让合作儿童讲述自己的诊疗感受和体会,这样可增强自己治牙的勇气,达到消除恐惧,接受治疗的目的。

(二)不合作患儿的诊疗行为诱导不合作患儿的诊疗行为诱导方法主要有3种,即固定法、镇静法和全身麻醉法。

乳磨牙预成冠的临床进展摘要】目的:本文详细地分析了小儿乳磨牙大面积缺损后,利于预成冠进行修复的临床效果分析,并对其疗效进行系统评价。

方法:选自我院于2016年1月-2015年6月期间口腔科收治的42例小儿乳牙龋坏患者,随机分为观察组和对照组,观察组采用树脂充填后预成冠修复,对照组则采用树脂充填治疗。

分别对两组乳磨牙治疗后的6个月,一年,2年的疗效进行系统的评估。

结果:观察组21例患者在6月修复体的脱落率为0,一年的脱落率为4.76%,2年的脱落率为9.52 %,对照组21例患者在6月修复体的脱落率为4.76%,一年的脱落率为19%,2年的脱落率为33.3%,对照组治疗效果明显优于对照组,观察组治疗效果明显优于对照组,结论:小儿乳磨牙大面积龋坏时,预成冠的修复有利于患者恢复正常的解剖外形和咬合关系,促进颌骨的正常发育。

值得在临床上广泛地推广和应用。

恢复患患儿的正常咬合关系等优点。

我国是小儿龋病的高发时期,5岁的患龋率为高达66.55%,其因素多为乳牙的解剖形态特点和饮食习惯有很大的关系,另外小儿的自洁效果相对较差,在充填治疗中患儿的远期的充填效果较差,脱离率极高。

针对以上情况我院门诊儿牙收治了42例小儿牙体缺损的患者作为作为研究对象,利于预成冠修复取得了满意的效果,具体案列分析如下。

1.资料与方法1.1 一般资料本文案例选取了我院儿牙科42例大面积龋坏的患者,男20例,女22例,年龄在4~7岁之间,平均为5.9岁,其中,第一乳磨牙为22例,第二乳磨牙为20例。

42例患儿均为邻面龋,MO为12例,MOD为18例,DO为12例。

遵照水门汀生产厂家的提示,等待合适的时间,然后用刮除器、探针等去除多余的水门汀,用牙线来回地把邻间隙清理干净,要确保龈缘处没有多余的水门汀。

11.粘固好的牙冠应该尽可能接近原来的牙齿,包括在牙弓中占据的间隙,与邻牙的接触关系以及与对合牙的咬合关系。

X 线片能更好地揭示这一点。

1. 从套装中挑选合适尺寸的牙冠,要与原来牙的近远中径相同。

2. 和预备前的测量比较,然后确认。

3. 试戴

5. 修剪牙冠

用Unitek 冠剪在划线下1毫米修剪,要注意接头,避免对龈缘的刺激,如有必要用不产热的磨石打磨。

6. 修整牙冠边缘
修整牙冠边缘。

用Unitek 边缘修整剪修整边缘多余的部分。

7. 试戴试戴。

8. 检查接触区
检查接触区。

用牙线检查接触点是否合适,用探针检查牙冠与预备体之间是否密贴。

9. 检查咬合检查咬合。

10. 完成修整
完成修整。

如果接触点要重新建立,则用Unitek 牙冠修整钳修整邻面。

11. 修整牙冠。

用Unitek 牙冠修整钳完成修整,使牙冠与预备体贴合。

12. 打磨抛光边缘。

为防止修剪过的部分刺激牙龈组织,牙冠边缘要用无热的人造石打薄打
光滑,然后用橡皮轮等合适的工具抛光,使牙冠便于病人清洁。

在粘固之前要再试戴一次。

13. 剩下的粘固步骤同上5-11。

注意注意:不锈纲牙冠成分包括镍铬,很少一部分人对这两种金属过敏,如果一旦出现过敏,请立刻咨询医生。
