超声监视下温盐水灌肠复位治疗小儿肠套叠效果观察

【摘要】目的:探讨超声监视下温生理盐水灌肠复位对小儿肠套叠的治疗效果及安全性.方法:选取2015-12～2017-08我院急诊入院的48例肠套叠患儿为观察组,行超声监视下温生理盐水灌肠复位;回顾性分析2013-01～2015-11在我院住院的42例肠套叠患儿为对照组,行X线监控下空气灌肠整复治疗.比较两组患儿的复位成功率、中转手术率、不良反应发生率及复发率.结果:两组患儿治疗效果均满意.观察组复位成功率为97.92％,高于对照组的85.71％,差异有统计学意义(P ＜0.05);观察组中转手术率、不良反应发生率及复发率分别为2.08％、4.16％、2.08％,明显低于对照组的14.28％、16.67％、19.05％,差异有统计学意义(P＜0.05).结论:超声监视下温生理盐水灌肠复位治疗小儿肠套叠安全无创,效果显著,不良反应发生率与复发率低,为临床治疗小儿肠套叠的首选方案.%Objective: To explore the effect and safety of warm saline enema reduction on children intussusception under ultrasonic surveillance. Methods: Forty - eight cases of intussusception patients treated in emergency department in our hospital from Dec. 2015 to Aug. 2017 were selected as the observation group. The warm saline enema reduction on children's intussusception was carried out under ultrasonic monitoring. Forty-two cases of intussusception patients in our hospital from Jan. 2013 to Nov. 2015 were analyzed retrospectively as the control group, who was treated by air
enema under X-ray monitoring. The rate of success, the rate of turned to operation, the incidence rate of adverse reaction and the rate of recurrence were compared between the two groups. Results: The treatment effect of the two groups was satisfactory, the successful rate of reduction in the observation group was 97.92％, which was higher than that in control group(85.71％), the difference was statistically significant(P ＜0.05). The rate of turned to operation, the incidence of adverse reactions and the recurrence rate in the observation group were 2.08％, 4.16％,
2.08％, which were significantly lower than those in the control group (14. 28％, 16.67％,19. 05％), the difference was statistically significant(P＜0.05). Conclusion: Ultrasound monitoring of warm saline enema for reduction of intussusception in children is safe, non-invasive, effective, with low incidence of adverse reactions and recurrence rate, so it is the first choice for clinical treatment.
