1．企業的代理證明、營業執照、稅務登記證、組織機構代碼、生產許可證等影本；

（Copies of agency certificate, license, tax registration, organization code, bank accounts, production permit and etc of the enterprise）

(1) 供應商詳細位址、郵編、電話、傳真、聯繫人等；

(3) 設備條件（包括主要生產加工設備，檢驗儀器、儀錶、設備清單）；

（equipment condition (including main production and processing equipments, test apparatuses, meters and equipment bill)）

(5) 人力資源/RD資源/QA資源/CS資源；

（human resources/R&D resources/QA resources/CS resources）

(6) 主要原材料來源；

(main materials source)

(7) 是否建立品質保證體系,取得國際或國內品質認證及其認證證書的影本；

（whether having set up quality guaranty system, obtained international or domestic quality certification,

whether the copies of the certificates available）

(8) 是否實行全面品質管制；

（whether having implemented entire quality management）

(9) 是否有生產3C產品資格，有效期內的工廠審核報告影本；本公司沒有生產3C 產品

(whether having the qualification to produce 3C products and the copy of the check report of the factory

within validity period）

(10) 產品是否是自行設計、仿製、引進、加工；

（whether the product is self designed, imitated, introduced or processed）

(11) 供貨保證能力及初步意向價格；

（supply capacity and primary price intended）

3．主要產品目錄、規格型號及有關企業標準；

（main product catalogues, specifications, models and related enterprise standards）4．產品品質歷史情況、參加評比、獲獎情況；

（product quality history, participation in contest and awards）

5．產品報價情況；

7．與其他廠家合作情況；

（cooperation with other factories）

8．環境保護資料及儀器設備清單，承諾遵守國家和地方環境保護法律、法規的檔。對ISO14000《環境管理體系》的認識和意向以及是否準備認證；

（environment protection documents and apparatus and equipment bills, documents of compliance of the national and local environment protection laws and regulations, understanding and intention to ISO14000 Environment Management System and whether to participate in certification）
