病理生理
ALD的病理特点是大体可见脑白质病变区呈固状或橡胶状, 肾上腺皮质萎缩,髓质不受影响;镜下可见脑组织、肾上 腺、周围神经、睾丸内有特征性的包涵体,包涵体内富含 VLCFA 酯化的胆固醇。脑部最显著的特征是沿大脑前后轴 由枕向额顶方向发展的脱髓鞘改变,故在任何情况下,总 是枕区白质受累最重,而U型纤维保留;另一特征是血管周 围的炎性细胞浸润。

肾上腺脑白质病(肾上腺脑白质营养不 良, adrenoleukodystrophy,ALD):又称X-连锁隐性遗传的Schilder病,亦
称嗜苏丹性脑白质营养不良伴肾上腺皮质萎缩,本病是性连隐性遗传 病伴外显不全、过氧化物体病(peroxisomal disease)(ICD-10-E)。
发病机制
约占所近有A年LD病已人的查4-明7%。其致病基因为编码ALD蛋白的ABCD1基因,位 约占所于有A染LD病色人的体4-X7%q。28。

ALD是一种临床异质性很强的遗传性代谢性疾病。表现 洛伐他汀(Lovastatin) ,加强VLCFA的-氧化作用,可使ALD患者皮肤成纤维细胞中的VLCFA水平降低。
