川崎病患儿外周血LncRNA SNHG5 miR-132的差异表达及临床意义沈韬 张海涛 顾旭华 金律[摘 要] 目的 研究川崎病(KD )患儿外周血长链非编码RNA (LncRNA )SNHG5、微小RNA (miR )-132的差异表达及临床意义。

方法 选择2016年1月至2020年12月在苏州市中西医结合医院儿科确诊的62例KD 患儿作为KD 组,另选取同期进行健康体检的80例健康儿童作为对照组。

采用荧光定量聚合酶链式反应检测两组受试者外周血LncRNA LINC00999、miR-6780、LncRNA PSORS1C3、miR-216a 、LncRNA SNHG5、miR-132、miR-92的表达水平,采用超声心动图评估KD 患儿的冠状动脉病变(CAL )情况并分为CAL 组和无CAL (NCAL )组;采用Pearson 检验分析LncRNA SNHG5与miR-132的相关性;采用受试者工作特征(ROC )曲线分析ln⁃cRNA SNHG5及miR-132对KD 及KD 合并CAL 的诊断价值。

结果 KD 组外周血中LncRNA SNHG5的表达水平高于对照组,miR-132的表达水平低于对照组,差异有统计学意义(P <0.05)。

LncRNA LINC00999、miR-6780、LncRNA PSORS1C3、miR-216a 、miR-92表达水平与对照组比较,差异无统计学意义(P >0.05);KD 组外周血中LncRNA SNHG5与miR-132的表达水平呈负相关(r =-0.527,P <0.001);KD 组中CAL 组患儿外周血中LncRNA SNHG5的表达水平高于NCAL 组,miR-132的表达水平低于NCAL 组,差异有统计学意义(P <0.05)。

经ROC 曲线分析,LncRNA SNHG5及miR-132的表达水平对KD (截断值分别为1.205、0.871,灵敏度分别为86.25%、75.00%,特异度分别为88.71%、85.48%)及KD 合并CAL (截断值分别为1.138、0.643,灵敏度分别为63.46%、67.31%,特异度分别为80.00%、80.00%)均具有诊断价值。

2020版:肠道EB病毒感染组织检测和病理诊断共识(全文)中国人群EB病毒(Epstein-Barr virus)血清学阳性率高达95%以上[1,2,3],绝大部分属于潜伏感染。

当全身或局部免疫力低下时,可引起EB 病毒的再激活,导致EB病毒机会性感染。

IBD患者肠道局部炎症刺激和免疫抑制剂的使用,易导致EB病毒重激活并加重病情[4,5]。

肠道EB病毒感染的诊断及其对IBD病情发展的影响日益受到关注。

结肠黏膜活组织检查(以下简称活检)标本中EB病毒感染检测是确定肠道EB病毒感染的主要方法。

为统一肠道EB病毒感染的组织标本检测及判读方法,并进一步规范肠道EB病毒感染的临床病理诊断,中华医学会消化病学分会消化病理协作组组织专家进行了肠道病理组织EB病毒感染检测质量控制和EB病毒感染意义共识讨论会,对EB病毒感染的检测方法、判读、计数和临床病理意义等开展广泛讨论并达成共识。

一、肠道黏膜活检取材肠道EB病毒感染时,病毒在黏膜组织中分布不均,溃疡处病毒负荷显著高于非溃疡黏膜[5,6],为确定有无EB病毒感染,内镜活检取材部位应重点位于溃疡处。

但是初诊患者或诊断不明的患者,为明确诊断,内镜活检应为系统性活检,上消化道活检部位应包括食管、胃体、胃窦和十二指肠,下消化道活检部位应包括回肠末端、盲肠、升结肠、横结肠、降结肠、乙状结肠和直肠,取材应取溃疡旁黏膜组织,避免只在溃疡底部取材。

二、EB病毒感染检测方法EB病毒感染检测方法包括原位杂交、PCR和免疫组织化学技术。

原位杂交检测EB病毒编码的小RNA(Epstein-Barr virus-encoded RNA, EBER)在EB病毒潜伏期和病毒复制期均大量存在于感染细胞的细胞核内,EBER原位杂交可在组织切片确定EB病毒感染细胞及其在组织中的定位[7,8,9]。

PCR可定量检测EB病毒DNA,但无法确定感染细胞及其在组织中的定位,常用于检测外周血EB病毒负荷[10]。
