我院超说明书用法处方分析
[摘要] 目的调查分析我院2014年一季度存在的超说明书用药情况，并分析其原因，为促进我院合理用药提供数据。

方法抽取我院2014年一季度处方35152张，根据药品说明书内容，判断是否超说明书用药；并分析超说明书用药的类型、超说明书用药产生的严重程度等。

结果共抽取35152份处方，其中超说明书用药处方10630张，占30.24%；超说明书用药类型中居前三位的是：给药频次（49.19%）、给药剂量（40.45%）及给药途径（9.03%）。

超说明书用药产生的严重程度以四级以下警示（轻微）为主。

结论我院处方超说明书用药的情况较普遍，医院应构建相应的管理制度，规范超说明书用法用药程序。

f-label uses of prescription drugs and its severity were analyzed. results in 35152 prescriptions， there were 10630 （30.24%） prescriptions that existing problems of off-label uses， the problems such as frequency （49.19%）， dose （40.45%）， and route of medication （9.03%） were among the top three types. the severity of the off-label uses of prescription drugs produced as below the level 4 （minor）. conclusion the situation of off-label uses of prescription drugs was frequently seen in our hospital， the hospital shouid build
corresponding managewent system and normalize the off-label uses program.

超说明书使用（unlabeled use， off-label use）是相对于按说
明书使用（labeled use）而言，即药品说明书之外的用法，是指药
品使用的剂量、患者群体、适应证和给药途径不在sfda批准的说明
书之内的用法。

按说明书用药时药品的安全性、有效性能得到保证，也被医疗行为相关各方和法律承认认可，但按说明书用药的缺点，
如知识更新滞后、特殊人群信息缺乏、审批不够严谨等也客观存在。

因此，体现最新用药进展、满足特定患者的医疗需求的超说明书使
用在临床用药实践中也是广泛存在的，也为我国卫生部门默认。

另
一方面超说明书使用由于sfda不支持、法律不承认，在临床使用中
也会引发一系列的用药问题：增加患者的用药风险、侵犯患者的权益、易引起医患矛盾和医疗纠纷[1]。

为促进我院药品的规范使用，
提高我院药品合理使用水平，现对我院2014年一季度在检查中出现
的超说明书用药进行分析。

1 资料与方法
1.1 一般资料
以我院合理用药系统中提示的2014年一季度中的不合理处方与
平时处方点评中出现的不合理处方为样本，提取其中的年龄、性别、就诊科室、临床诊断、用药情况为素材进行统计分析。

1.2 超说明书用药判断标准
根据国家食品药品监督管理局批准的最新药品说明书来判断患
者超说明书用药情况。

超说明书用药包括下列情况：适应证、适用
对象、剂量、给药频率、给药途径、禁忌证等。

一张处方出现2种
以上超说明书用药的在统计时分别统计。
