近 10年来口腔种植学在中国得到了迅速发展 , 每年有数以千计的患者接受口腔种植修复 ,但种植 修复临床长期效果的研究尚未见报告 。我们回顾北 京大学口腔医学院 ·口腔医院口腔种植中心 10 年 来种植修复的临床结果 ,分析 10 年存留率 ,现报告 与讨论如下 。

5. 种植体周围软组织功能性重建与美学处理手 术 : 2 629 例 患 者 中 接 受 此 类 手 术 者 713 例 (27112% ) ,男性 407 例 ,女性 306 例 。在 5 590 枚 种植体中有 987枚用于此类手术 ,约占 17166% ,其 种植体系统分布 : Cam log 361 枚 , B r…nemark 36 枚 ,百度文库Ankylos 44 枚 , Frialit22 402 枚 , IM Z 137 枚 , Komet 6枚。

资料和方法
11 病例来源 : 1994 年 8 月至 2004 年 8 月北京 大学口腔医学院 ·口腔医院口腔种植中心接受种植 治疗的 患者 2 629 例 , 共植 入 5 590 枚 牙种 植 体 (表 1) 。

6. 修复方式 :种植固定修复 2 314例 (用种植体 4 014枚 ) ,占总种植患者的 88102% ;种植活动修复 315 例 (用 种 植 体 1 576枚 ) , 占 总 种 植 患 者 的
11198 % 。 71定期复查时间 :所有接受种植治疗的患者于
术后 1周 、3个月 、6个月 、上部结构修复后即刻 、修 复后 1年均需复查 。
10. 所有患者的追踪随访结果由一位中年资医 师记录 。
结果
自 1994年 8月至 2004年 8月在北京大学口腔 医学院 ·口腔医院口腔种植中心共有 2 629例患者 接受了种植修复 ,共植入 5 590 枚种植体 。所有植 入的种植体的系统 、型号 、部位 、时间 、手术方法 、手 术类型 、修复时间及方式均从口腔种植中心手术及 修复记录中总结整理 ;并按手术前患者记录的地址 , 集中通知患者在 2005年 1—8月复诊 。截止至 2005 年 8月共 1 025例患者接受了复诊追踪 ,占 3910%。 610例 患 者 回 信 答 复 了 追 踪 并 寄 回 X 线 片 , 占 2312 % 。

口腔种植修复临床效果十年回顾研究

【摘要 】 目的 评估骨结合种植体应用 10 年的临床效果 。方法 1994年 8 月至 2004年 8月 在北京大学口腔医学院 ·口腔医院口腔种植中心植入的骨结合种植体共 5 590枚 ,其中 B r…nemark种 植体 161枚 , Frialit22种植体 1 436 枚 , IM Z种植体 1 012 枚 , Ankylos种植体 767 枚 , Cam log种植体 2 189枚 , Komet种植体 25枚 。全部种植体均在 2004 年 8 月前完成上部结构修复 ,其中固定修复 2 314例 ,活动修复 315例 ,共计 2 629例患者 。平均追踪 7712个月 (7～121个月 ) 。观察方法为 X线 检查和临床检查 ,采用 W heeler存留标准评估 ,纳入 Kap lan2M eier存留曲线统计 。结果 种植修复体 完成时 2 624例患者对修复效果满意 , 5例患者对修复体不满意 ,重作后 4例表示满意 , 1例表示可以 接受 。随访中有 105例患者共 178枚种植体确诊为种植体周围炎 。有明确记录的共 68枚种植体脱 落 。10年随访累计修复体松动 21件 ,修复体折断 4件 ,烤瓷冠崩瓷 51件 。据 W heeler存留标准统计 10年存留率为 9617%。结论 种植修复患者的满意度高 ,本组种植体的 10年存留率达到 9617%。
4. 植入方法 : 除经典的 B r…nemark 外科种植方 法外 ,还运用多种骨量扩增技术 ,如上颌窦底提升植 骨技术 、上置法植骨技术 、牙槽突垂直骨牵引技术 等 [ 125 ] (表 4) 。

8. 定期复查项目 :修复体状况 ———断裂 、崩瓷 、 松动 ;有无种植体周围炎及其他并发症 ; X线片对比 观察种植体边缘骨吸收状态 。
9. 信访追踪 :对未能定期复查和未能在 2005年 6月以前前来复查的患者信访追踪 ,内容同定期复 查项目 ,并要求患者在其所在地拍曲面断层 X线片 寄回复查 。截止日期为 2005年 8月 31日 。
