一、申请实用新型专利，应当提交实用新型专利请求书、权利要求书、说明书、说明书附图、说明书摘要、摘要附图。
申请文件应当一式一份。

二、本表应当使用国家公布的中文简化汉字填写，表中文字应当打字或者印刷，字迹为黑色。
外国人姓名、名称、地名无统一译文时，应当同时在请求书英文信息表中注明。
三、本表中方格供填表人选择使用，若有方格后所述内容的，应当在方格内作标记。
四、本表中所有详细地址栏，本国的地址应当包括省（自治区）、市（自治州）、区、街道门牌号码，或者省（自治区）、县（自治县）、镇（乡）、街道门牌号码，或者直辖市、区、街道门牌号码。
有邮政信箱的，可以按规定使用邮政信箱。
外国的地址应当注明国别、市（县、州），并附具外文详细地址。
其中申请人、专利代理机构、联系人的详细地址应当符合邮件能够迅速、准确投递的要求。
五、填表说明
.本表第①、②、③、④、⑤、⑥、栏由国家知识产权局填写。
.本表第⑦栏实用新型名称应当简短、准确，一般不得超过个字。
.本表第⑧栏发明人应当是个人。
发明人有两个以上的应当自左向右顺序填写。
发明人姓名之间应当用分号隔开。
发明人可以请求国家知识产权局不公布其姓名。
若请求不公布姓名，应当在此栏所填写的相应发明人后面注明“（不公布姓名）”。
.本表第⑨栏应当填写第一发明人国籍，第一发明人为中国内地居民的，应当同时填写居民身份证件号码。
. 本表第⑩栏申请人是中国单位或者个人的，应当填写其名称或者姓名、地址、邮政编码、组织机构代码或者居民身份证件号码；申请人是外国人、外国企业或者外国其他组织的，应当填写其姓名或者名称、国籍或者注册的国家或者地区。
申请人是个人的，应当填写本人真实姓名，不得使用笔名或者其他非正式的姓名；申请人是单位的，应当填写单位正式全称，并与所使用的公章上的单位名称一致。
. 本表第栏，申请人是单位且未委托专利代理机构的，应当填写联系人，并同时填写联系人的通信地址、邮政编码、电子邮箱和电话号码，联系人只能填写一人，且应当是本单位的工作人员。
申请人为个人且需由他人代收国家知识产权局所发信函的，也可以填写联系人。
. 本表第栏，申请人指定非第一署名申请人为代表人时，应当在此栏指明被确定的代表人。
. 本表第栏，申请人委托专利代理机构的，应当填写此栏。
. 本表第栏，申请是分案申请的，应当填写此栏。
申请是再次分案申请的，还应当填写所针对的分案申请的申请号。
.本表第栏，申请人要求外国或者本国优先权的，应当填写此栏。
.本表第栏，申请人要求不丧失新颖性宽限期的，应当填写此栏，并自申请日起两个月内提交证明文件。
．本表第栏，申请人要求保密处理的，应当填写此栏。
．本表第栏，申请人同日对同样的发明创造既申请实用新型专利又申请发明专利的，应当填写此栏。
未作声明的，依照专利法第九条第一款关于同样的发明创造只能授予一项专利权的规定处理。
（注：申请人应当在同日提交发明专利申请文件。

．本表第、栏，申请人应当按实际提交的文件名称、份数、页数及权利要求项数正确填写。
. 本表第栏，委托专利代理机构的，应当由专利代理机构加盖公章。
未委托专利代理机构的，申请人为个人的应当由本人签字或者盖章，申请人为单位的应当加盖单位公章；有多个申请人的由全体申请人签字或者盖章。
.本表第⑧、⑩、栏，发明人、申请人、要求优先权声明的内容填写不下时，应当使用规定格式的附页续写。
. 申请人应当在缴纳申请费通知书（或费用减缓审批通知书）中规定的缴费日前缴纳申请费和申请附加费。
申请人要求优先权的，应当在缴纳申请费的同时缴纳优先权要求费。
.一件专利申请的权利要求（包括独立权利要求和从属权利要求）数量超过项的，从第项权利要求起，每项权利要求增收附加费元；一件专利申请的说明书页数（包括附图页数）超过页的，从第页起，每页增收附加费元，超过页的，从页起，每页增收附加费元。
.申请人请求减缓费用的，应当在提交申请文件的同时提交费用减缓请求书及相关证明文件。
.各种专利费用可以直接到国家知识产权局缴纳，也可以通过邮局或者银行汇付。
.通过邮局汇付的，收款人姓名：国家知识产权局专利局收费处商户客户号：；并应当在汇款单附言栏中写明申请号、费用名称（或简称）及分项金额。
.通过银行汇付的，户名：中华人民共和国国家知识产权局专利局，开户银行：中信银行北京知春路支行，账号：；并应当在银行汇款单中写明申请号、费用名称（或简称）及分项金额。
.对于只能采用电子联行汇付的，应当向银行付电报费，正确填写并要求银行至少将申请号及费用名称两项列入汇款单附言栏中同时发至国家知识产权局专利局。
.应当正确填写申请号位阿拉伯数字（注：最后一位校验位可能是字母），小数点不需填写。
.费用名称可以使用下列简称：
印花费——印实用新型专利申请费——申

.费用通过邮局或者银行汇付遗漏必要缴费信息的，可以在汇款当日通过传真或电子邮件的方式补充。
(传真电话：；电子邮箱：.)补充完整缴费信息的，以汇款日为缴费日。
当日补充不完整而再次补充的，以国家知识产权局收到完整缴费信息之日为缴费日。
补充缴费信息的，应当提供邮局或者银行的汇款单复印件、所缴费用的申请号（或专利号）及各项费用的名称和金额。
同时，应当提供接收收据的地址、邮政编码、接收人姓名或名称等信息。
补充缴费信息如不能提供邮局或者银行的汇款单复印件的，还应当提供汇款日期、汇款人姓名或名称、汇款金额、汇款单据号码等信息。
.未按上述规定办理缴费手续的，所产生的法律后果由汇款人承担。
