水疗联合巴氯芬改善脑损伤患者肌痉挛的疗效观察

Observation on curative effects of hydrotherapy combined with baclofen in improving muscle spasm of patients with Brain Injury
LAN Haiyan CHEN Peng HU Fosheng Guangdong Work Injury Rehabilitation Hospital,Guangzhou 510970,China [Abstract] Objective To analyze curative effects of hydrotherapy combined with baclofen in improving muscle spasm of patients with brain injury so as to provide a reference for further improving the prognosis and quality of life of brain injury patients with muscle spasm. Methods Clinical data of 20 patients with brain injury who were admitted to our hospital from February 2015 to February 2017 were retrospectively analyzed.All the patients had different degrees of muscle spasm symptoms,and they were all given hydrotherapy combined with oral administration of baclofen for three months.The improvement of muscle spasm was observed. Results The total clinical effective rate of patients was 95.00%.Of whom,13 cases (65.00%) were significantly effective,4 cases (20.00%) were effective,2 cases (10.00%) improved and 1 case (5.00%) was ineffective.The average MBI of patients before treatment was (29.87±7.58) and that after treatment was (58.24±8.43).The difference of MBI in patients before and after treatment was statistically significant (P ＜ 0.05).No significant adverse reactions occurred in all patients. Conclusion The combination application of hydrotherapy and baclofen in the treatment of patients with brain injury can effectively control the spasm symptoms of patients and improve the self-care ability,which is worth promoting. 1.1 一般资料 本研究回顾分析 2015 年 2 月～ 2017 年 2 月
肌痉挛为脑损伤患者常见的并发症，但并不是 所有的肌痉挛都有害且需要特别治疗，适度的肌痉 挛可有效维持机体肌肉容积及骨骼矿化，增加髋及 躯干的稳定性，对于患者的安全移动有利，同时还 可减少深静脉血栓及肺栓塞的发生；但若肌痉挛无 法维持在一个有益的水平，将造成患者出现疼痛， 不利于肢体的随意运动，可导致关节挛缩，影响患 者正常的工作与生活 [1-2]。本院将水疗联合巴氯芬 应用于脑损伤患者的治疗中，在改善其肌痉挛方面 取得满意效果，现报道如下。 1水疗联合巴氯芬改善脑损伤患者肌痉挛的疗效观察

[ 摘要 ] 目的 对在脑损伤肌痉挛的治疗中联合运用水疗与巴氯芬的临床改善效果进行分析，以期为进一步 提高脑损伤肌痉挛患者的预后、改善生活质量提供参考。方法 对我院于 2015 年 2 月～ 2017 年 2 月接收 的 20 例脑损伤患者的临床资料进行回顾性分析，所有患者均存在不同程度肌痉挛症状，均给予为期 3 个月 的水疗并联合巴氯芬口服治疗，观察患者肌痉挛改善效果。结果 患者临床治疗总有效率为 95.00%，其中 显效 13 例（65.00%），有效 4 例（20.00%），好转 2 例（10.00%），无效 1 例（5.00%）；治疗前患者 MBI 平均为 （29.87±7.58），治疗后为（58.24±8.43），治疗前后患者 MBI 比较，差异有统计学意义（P ＜ 0.05）；所有患者 均未发生明显不良反应。 结论 将水疗与巴氯芬联合应用于脑损伤患者的治疗中可有效控制患者的肌痉挛 症状，提高患者生活自理能力，值得推广。 [ 关键词 ] 脑损伤；肌痉挛；水疗；巴氯芬 [ 中图分类号 ] R277.7 [ 文献标识码 ] A [ 文章编号 ] 2095-0616（2019）09-251-04
