主体结构分部工程质量评估报告
(金源科技大厦1#楼)
一工程概况
1、项目名称:金源科技大厦相关配套房工程
2、工程地点: 滨湖区建筑路/明园路交界处
3、建设规模:建筑面积约64805.25㎡,地下室建筑面积12023㎡,地下2层,地上24层。

框架木刻核心筒结构商务楼。

金源科技大厦工程项目组成;由三幢主楼结构大楼组合成,分为#1楼和#2楼(双子楼),本评估为#1楼为24层单位工程主体结构,1至5层为裙房(层高为5500、4500),6至24层为标准层共19层(层高为3750),结构特征为核心筒框架结构,其层高、总高、平面形状详见附图-1、2(注明)。

该楼设有共同的地下车库,地下室为二层,主要用来停放车辆,有局部地下结构设有地下人防工程,人防区建筑面积4400平方米(另外评估)。

全部为钢筋砼框架结构。

二评估依据
1、设计图纸,设计变更通知单,图纸会审记录。

2、施工合同、监理合同。

3、《建筑工程施工质量验收统一标准》GB50300—2001。

4、《砼结构工程施工质量验收规范》GB50204—2002。

5、《建设工程监理规范》GB50319-2000。

6、人防验收规范。

7、无锡市、区建设主管部门有关规定;

本工程符合国家法律、法规、主体工程施工阶段严格执行工程建设强制性标准。

四设计单位配合情况
对于工程上的一些技术问题,设计单位能及时答复,设计修改通知单也能及时到达各参建单位,避免了施工后再返工现象,分部工程验收设计单位能积极参
加,并提出合理化建议,主体工程满足设计功能和设计指标。

五监理合同履约情况
1 、监理工程师严格按监理合同开展工作,“独立公正、遵约守法、严格热情、创新提高”。

2 、工程质量监理情况
2.1 、建筑工程原材料质量控制措施
(1)对材料进场严格控制,杜绝一切不合格材料进场投入使用,保证工程所用材料符合图纸设计要求、施工规范规定及合同约定要求。

(2)对进场的钢筋、商品砼、砖砌体及水电、消防材料等原材料均按规范要求见证取样送检,同时查验其产品合格证书、出厂检测报告及质量保证书,保证所有进场材料经检验合格后方可投入使用。

2.2 、构配件及设备进场检验情况
(1)对进场的钢管和扣件进行取样复试检查,对未刷防锈漆的要求刷防锈漆进行保护,对锈蚀严重和存在质量缺陷的钢管和扣件不得投入使用,并及时督促施工单位清退出场。

(2)钢筋螺纹机、切割机现场临时配电箱等设备能提供产品合格证书。

(3)工程上使用的计量和测量仪器提供产品合格证书和校验报告。

2 .3、模板安装工程质量控制措施
(1)要求模板及其支撑体系的承载力、刚度和稳定性必须满足设计要求及安全规范规定,模板拼缝必须严密,避免漏浆,同时要求模板接头处必须平整,保证砼成型后外观质量满足设计要求。

(2)复核柱及剪力墙模板垂直度是否符合规范要求,对每一施工段的梁、板、柱及剪力墙的截面尺寸、标高、轴线进行复核,确保砼成型尺寸符合设计要求。

2.4、钢筋制作安装工程质量控制措施
(1)督促施工单位加大自检力度,坚持跟班检查钢筋制作、安装质量,保证钢筋的锚固长度、搭接长度、钢筋接头位置等构造要求符合设计及施工规范规定。

(2)监理工程师不定期巡查,对钢筋的制作、安装进行严格控制,检查过程中发现问题要求施工单位及时整改,经复查符合要求后方同意进行下道工序施工。

(3)对每一施工段柱钢筋电渣压力焊的焊接质量进行全数检查,不合格的和有质量缺陷的一律要求割除重新焊接,并按照规范规定的取样频率由监理工程师见
证取样送检。

2.5、砼工程的质量控制措施
(1)本工程结构砼采用“久安搅拌站”预拌混凝土,工程开工前监理部对砼配比进行审查,并二次和甲方人员对搅拌站性能进行考察,能满足质控要求。

(2)每次混凝土浇筑前,监理人员都要查看混凝土设定配比与配比单吻合,监督砼配比并做坍落度试验,确保混凝土半成品质量符合设计要求。

(3)砼浇筑过程中,监理人员进行全过程旁站监理,严格控制砼供料、振捣、浇筑质量,同时要求施工单位安排专业钢筋工、木工及水电工跟班看护相应工种的施工质量,特别是梁柱节点的钢筋与砼的隐蔽前状况,发现问题及时整改,并按照规范规定要求施工单位留置相应数量的同条件及标准养护砼试块,旁站监理人员作好旁站监理记录,如实反映砼浇筑情况。

(4)砼浇筑完毕后的2d内,要求施工单位必须安排专人定时对砼湿润养护,保持砼表面处于湿润状态。

2.6、水电预留、预埋的质量控制措施
(1)检查预埋管件壁厚、内外径、焊缝刀口毛刺等原材质量及防锈漆的涂刷。

(2)对现场预留预埋构件全数检查,确保预留预埋准确无误、无一遗漏。

(3)检查柱筋电渣压力焊的防雷接地跨焊焊接质量及可靠性。

(4)水、电监理工程师签认隐蔽验收记录后方可砼浇筑。

六质量评定情况
1.土方开挖基坑成型
土方开挖单位和基础施工单位做到认真协调,基础定位放线经监理复核后,可进行基坑土方开挖。

当挖至设计桩顶标高时,通知设计单位、甲方、勘察院等相关单位进行基坑、工程质量验收。

2 、模板分项工程
2.1 模板支设符合要求,具有足够的强度、刚度和稳定性。

2.2 模板拼缝比较严密,模板接头处平整度一般,板面较清洁。

2.3 主体结构分部工程中模板分项工程共123个检验批,经查验该分项工程所有检验批的主控项目及一般项目均满足施工质量验收规范要求,该分项工程评定合格。

3、钢筋分项工程
3.1 质量保证资料齐全,见证取样送检的试件检测结果均合格。

3.2 本分项工程共有372个检验批,经查验该分项工程所有检验批的主控项目及一般项目均符合施工质量验收规范要求,该分项工程评定合格。

4 、砼分项工程
4.1 质量保证资料齐全,主体结构所有砼试块全部达到龄期,试块试压评定结果全部合格。

4.2本分项工程共有186个检验批,经查验该分项工程所有检验批的主控项目及一般项目均符合施工质量验收规范要求,该分项工程评定合格。

4.3现浇结构尺寸检查42个检验批,经查验该分项工程所有检验批的主控项目及一般项目均符合施工质量验收规范要求,该分项工程评定合格。

5、砖砌体分项工程
5.1质量保证资料齐全,主体结构所有砂浆试块全部达到龄期,试块试压结果全部合格。

5.2该分项工程共有12个检验批,经查验该分项工程所有检验批的主控项目及一般项目均符合施工质量验收规范要求,该分项工程评定合格。

6、防水工程
6.1防水工程检验批19个,防水材料进场后进行取样复试,合格后准予使用,严格按要求施工。

6.2对后浇带、施工缝、预埋、预留等部位,严格采用止水钢板、止水翼环等措施。

6.3混凝土浇灌时,严格添加剂的配比,确保防水混凝土性能。

6.4防水工程共19个检验批。

七工程质量控制资料
工程质量保证资料基本齐全,主体分部工程技术档案已归档、整理,到工程竣工以后统一装订。

八、工程结构(钢筋保护层、砼实体强度、楼板厚度)委托无锡市检测中心进行现场检测,楼板厚度尺寸偏差合格率为64%,均超过设计厚度,梁钢筋保护层厚度合格率为92.4%,板钢筋保护层厚度为92.64%,均判定为合格;地下室柱砼现龄期芯样强度值为73.9MPa、44.3MPa、78.1MPa,主楼一层柱砼现龄期芯样抗压强度值为67.8MPa、二层为73.8MPa、三层为60.7MPa,八层为49.0MPa、九层为47.4MPa、
十层为60.3MPa、十七层为48.9MPa、十八层为56.4MPa、十九层为46.0MPa、二十一层为35.9MPa、二十二层为45.8MPa、二十三层为51.0MPa、地下室第一次为44.3MPa不合格,第二次复测能符合fcu﹒min≥0.9fcu﹒k为45.0MPa,现实际为
47.0MPa。

其检测结果为满足设计及规定标准的要求,判定为“合格”
九、工程质量事故处理情况
本工程无质量事故记录。

十、工程观感质量验收情况
主体结构砼观感质量评为“一般”。

十一、评估结论
主体结构分部工程中共有个762个检验批,其中土方开挖共8个检验批,模板分项工程,模板安装123个。

钢筋分项工程,钢筋加工、钢筋安装372个。

砼分项工程,原材料配合比、砼施工检验批186个、现浇结构尺寸42个。

砖砌体分项工程12个,防水19个。

经检查,检验批全部合格,分项工程全部合格,主体结构〖分部工程质量〗经评定为“合格”等级。
