摘要：目的探讨对胆脂瘤中耳炎患者施以改良乳突根治术+鼓室成形术治疗后获

得的临床效果。方法方便选择该院2014年12月—2016年11月收治的100例胆

脂瘤中耳炎患者作为实验对象；临床选择数字奇偶法对胆脂瘤中耳炎患者分組；

对照组（50例）：临床施以传统乳突根治术治疗；观察组（50例）：临床施以

改良乳突根治术+鼓室成形术治疗；对胆脂瘤中耳炎疾病疗效以及外耳道变化回

顾性分析。结果对两组胆脂瘤中耳炎患者的气骨导差情况对比发现，观察组胆脂

瘤中耳炎患者总改善率（100.00%）同对照组胆脂瘤中耳炎患者总改善率

（58.00%）加以比较发现，提高幅度非常明显（P<0.05）；观察组胆脂瘤中耳炎

患者外耳道变化同对照组胆脂瘤中耳炎患者比较，观察组正常患者45例

（90.00%），小腔5例（10.00%），中腔0例（0.00%）；对照组正常患者19例（38.00%），小腔患者19例（38.00%），中腔9例（18.00%），差异有统计学

意义（P<0.05）。结论对胆脂瘤中耳炎患者，临床施以改良乳突根治术+鼓室成

形术治疗，对于患者的外耳道形状可以加以有效维持，获得显著胆脂瘤中耳炎疗效，从而优化胆脂瘤中耳炎患者的生活质量。

[abstract] Objective To investigate the clinical effect of modified mastoidectomy plus tympanoplasty on cholesteatoma otitis media. Methods 100 patients with cholesteatoma otitis media admitted to our hospital from December 2014 to November 2016 were selected as the experimental subjects conveniently；the patients with cholesteatoma otitis media were divided into two groups by digital parity method；the control group（50 cases）was treated with traditional mastoidectomy；the observation group（50 cases）was treated with modified mastoidectomy + tympanoplasty；and the cholesteatoma otitis media disease was treated with modified mastoidectomy and tympanoplasty. A retrospective analysis of the therapeutic effect and the changes of external auditory canal was made. Results Comparing the air-bone conductivity of two groups of patients with cholesteatoma otitis media，it was found that the total improvement rate of patients with cholesteatoma otitis media in the observation group（100.00%）was significantly higher than that in the control group（58.00%）（P < 0.05）；the change of external auditory canal in patients with cholesteatoma otitis media in the observation group

was significantly higher than that in patients with cholesteatoma otitis media in the control group. There were 45 normal patients（90.00%），5 small lumen patients （10.00%）and 0 middle lumen patients（0.00%）in the observation group，19 normal patients（38.00%）in the control group，19 small lumen patients（38.00%）and 9 middle lumen patients（18.00%）in the observation group，with significant difference（P < 0.05）. Conclusion Modified radical mastoidectomy plus tympanoplasty can effectively maintain the shape of the external auditory canal in patients with cholesteatoma otitis media，and achieve significant curative effect of cholesteatoma otitis media，thereby optimizing the quality of life of patients with cholesteatoma otitis media.
