「活出耆彩」小组计划书引言根据世界卫生组织的定义,全人健康的定义包括三方面:生理、社交及心理。
要满足这三方面的需要,的确不易。
工作员在中心实习了数个星期,观察到有约五成长者在这三方面的健康情况都未能达标:生理方面,部份长者整天坐在中心的沙发上看电视,除了上厕所及午膳外,基本上不会离开座位,显然,他们没有足够的运动量;社交方面,有些长者的社交圈子陕窄,他们活于自己的世界中,不大愿意结识新的朋友;最后,心理方面,部份长者存有较负面的情绪,例如他们会认为自己年纪大了,一无是处。
为了促进长者的全人健康,并响应老人中心的活动主题,故工作员尝试以社会工作的小组模式去介入,设计了这一个小组活动。
小组活动理念小组由来根据二零零一年香港统计署的数据,在一项对老人生活满足感的调查指出,在1172名的受访者中,有9.9%是独居者,约有62.4%的受访者并无任何亲戚可倾诉心事,而约有67.5%的被访者并无任何朋友可倾诉心事;有三个或以上之亲戚或朋友可与之倾诉心事者分别只占16.4%和18.3%。
每个老人平均有1.1位亲戚和1.2位朋友可倾诉心事。
另外,51.3%的受访者回答受伤和生病时,会有一个人可以帮忙,6.9%的受访者认为没有人能帮助,3.9%的受访者会有四个以上的人来帮忙他们,平均值是1.6个。
这些数据反映出长者的生活圈子狭窄。
在同一项调查中,有高达85.9%的长者有抑郁情绪,不论是轻微的或是明显的。
这都反映出长者抑郁的问题确实不容忽视。
其实,近年来愈来愈多社会人士关注长者健康。
正如引言提及过,健康的定义包括三方面:生理、社交及心理,而以上数据正正反映出长者的健康的不足。
作为一个社会工作的实习学生,我们有必要作出介入以促进长者的健康。
另一方面,工作员实习的机构社工正跟进几位抑郁症康复者,他们极需要社交生活去帮助他们进行精神复康。
工作员成立是次小组,社工便可以把这几位长者传介过来,让他们体验小组生活,于他们有益。
由于上述原因,工作员成立是次以追求全人健康为目标的舒缓小组。
另外,这同时亦切合实习机构今年的活动主题「长者全人健康」,实在是一举数得。
组员具体情况年龄小组的组员均来自机构属下的两间中心,分别是福禧老人中心及和禧天地。
福禧老人中心的会员是六十岁或以上,而和禧天地的会员则是介符五十至五十九岁之间。
在组员当中,年龄最少的是五十多岁,最大的有八十多岁。
绝大部份的组员是来自福禧老人中心,只有一位是来自和禧天地。
背景与性格组员当中有为数字是福禧老人中心非洲鼓队的成员,他们的性格较为开朗,亦勇于发表意见。
此外,他们相识多年,默契已然建立。
初步认为,这些组员在小组中会成为其中一个次小组。
另一方面,组员中有一位是来自和禧天地,她的年龄最小,并且有抑郁症的背景。
初部了解,这位组员的性格较为沉默,而且口齿有点不清,说话速度较慢。
基于以上原因,工作员需留意这位组员与其它组员的磨合情况,并在适当的时候作出介入。
除此之外,还有一群组员不是非鼓队的成员,却又参与了中心活动达十年之久。
初部认为,这群长者应该习惯了小组的聚会模式,对这样的模式不会感到陌生。
还有一对夫妇是较少参与小组活动,有较大可能对小组产生焦虑感,而且丈夫的行动不便,工作员需特别留意。
在中心的活动情况参与非鼓队的那一群组员及参与了中心近十年组员,平日在中心较为活跃,他们积极参与中心的活动,例如义工小组及环保小组。
他们大多一大清早到街市买菜,然后便直接来到中心参与活动,完毕后便回家。
生活总是忙得很。
对于这一群组员,是次小组的介入重点会放在尝试让他们反思自己的生活模式是否平衡,以及提供一个机会让他们停下来沉淀思想。
另一方面,另一群组员正好相反,他们在中心的活跃程度不高。
即使参与活动,也是那些偏向单向接收讯息的活动,例如医疗讲座等。
这群长者有一个特点,就是他们较为关心健康的讯息,这可以从他们会积极参与健康运动及健康讲座观察出来。
除了参与这类活动外,这些组员平日在中心只会静静地看电视—由早上直至中心关门为止。
对于这一群组员,是次小组的介入重点是让他们接触不同的人,习惯小组模式,并扩阔他们的社交圈子及视野(有关健康生活的模式)。
理论架构问题介定长者的全人健康问题,可以从生理,社交,心理及认知这四方面去分析。
首先,在生理层面来说,部份组员较少运动:他们平日的运动时间只限于家中来回中心,出外午膳,以及间中进行的午间操,这样的运动量不够广度之余,亦不深入。
这可以解释为何他们经常会有一些小毛病,例如头痛。
为了促进他们的生理健康,他们实在有必要学会一套随时随地都可以做的运动,而且这项运动应有一定的广度和深度—广度是指尽量使每一处肌肉都能运动;而深度是指做完运动后,身体能够得到深层次的舒缓。
其次,在社交层面来说,有些组员的社交圈子狭窄。
他们到来中心,不会主动结识其它会员,大部份时间都是自顾自的看电视或看报纸。
同时,这些组员亦较少参与中心活动,可见他们的社交生活少之又少,这是极不健康的。
在心理层面来说,组员每天都要面对不同的生活压力,例如独居的压力以及因与家人相处不来而产生的压力等。
每个人应付压力的能力(capacity)不尽相同,有些组员可以应付过来,可是有些却未能应付。
这些未能应付的长者,心理状况并不稳定,他们没有良好的心理健康。
另一方面,每一位组员都会存有一些传统的观念,例如「媳妇是用来差遣的」,这些思想可能不合时代,在生活中引起冲突,这又增加长者所面对的压力。
最后,在认知层面上,大部份组员只知道要令自己晚年过得丰盛,便尽量参与中心活动,希望把自己的时间表「塞」得满满的,可是却忽略了平衡生活的重要性。
一些正确的健康生活价值观正正是他们所缺乏的。
另一方面,组员们从未接触过用艺术去调剂健康生活的知识,让他们接触这方面的知识可能会为他们带来新的生活方向。
问题成因之分析就以上的问题,工作员有以下的问题成因分析。
在生理层面上,组员较少做运动,这可能是因为他们认为运动对体力有很高的要求,而他们自己的年纪不少,不能应付。
此外,这也可能是他们情愿做一些静态活动如看报纸而不大愿意运动的缘故。
在社交层面上,组员不大与其它会员沟通,又少参与中心活动。
这可能是因为他们不懂得与人相处的技巧,不知道如何与其它人沟通,亦有可能他们不习惯与人相处的气氛。
在心理层面上,组员因生活压力而引致不同的不健康心理,这有很大原因是他们不懂得正确的舒缓之道,致使他们不能解决当前烦恼。
除此之外,一些较为保守的传统观念也可能是引起心理不健康的原因:他们用一种执着的态度去分析生活的事,致使他们有很多事都「看不过眼」,于是引起心理的郁结,使心理不健康。
在认知层面上,组员们平日多在大众传媒接触一些健康生活常识。
大众传媒传递的健康知识较为普通,至于一些较为另类的健康生活方式,组员们较少在日常生活中接触到。
这令到他们觉得要令自己达致健康,来来去去只有数种难以做到的方法。
问题的严重性根据二零零一年的香港人口普查,65岁及以上的人口约有75万,占总人口的11.5%,未来人口将持续老化,65岁的人口比例将显着上升至2031年的约24% (872万),由此可见,长者的人口数目正急速膨胀。
由于老人的人口不断上升,一旦上述问题处理不当,后果不堪设想,例如会造成沉重的医疗及老人服务的负担。
以下会就生理,社交和心理三方面分析问题的严重性:生理方面,组员长期缺乏足够运动,根据许多研究发现,这可以引起许多慢性疾病,例如心脏病和糖尿病等。
这些疾病都是慢性病,平日不容易察觉出来,可是一旦病发了,要作出补救已经是太迟了。
相反,长者如果有适量的运动,能够降低患上心血管疾病及其它俗称「老人病」的机会(Mazzeo,1998)。
社交方面,根据马斯洛(Maslow)的需要层次理论(Hierarchy of Needs),若果个人不能满足第三层的社交需要(Social needs),则他便不能追求自尊需要(Esteem needs)及自我实现(Self-actualization)。
对长者而言,他们已经处于人生的尾段,剩下的日子去追求最高的自我实现实在不多。
可是,如果他们连社交需要也满足不了,那么他们更没有时间去追求更多。
这对他们的追求完满发展实在是一大障碍。
心理方面,长期的不健康心理情况会引起不同的严重后果。
根据George(1991)认为,有三种社会环境因素可能导致老人的忧郁﹕(一)生活事件,例如失去伴侣;(二)慢性压力,例如健康受威胁;(三)生活琐事,例如与邻居口角。
组员正面对着不同的生活压力,而压力的累积会诱发其它情绪病,例如忧郁。
另外,压力事件产生失落或威胁(包括实际发生的生理疾病)是诱使虑病症发作的原因之一(U’Ren,1990)。
加上研究证实长期性压力会多方面破坏身体的功能及抗体。
由此可见,压力己成了疾病的前导车(Newsweek,1999/6/14)。
最后,根据《老人自杀资料汇编》一书,长者身处暮年,面对各方面生活的转变、退休生活的适应及生理的退化等,心理的压力可不少。
若然在心理上或适应上出现问题,而未能获得适当的辅导或治疗,部份长者可能会患上抑郁症;若严重,这种精神问题可导致自杀的发生。
小组活动设计理论小组的设计是根据世界卫生组织的对全人健康的定义—包括三方面:生理、社交及心理而设计出来的。
就解决生理健康方面的问题,小组会教授组员一套「深切肌肉松弛法」。
这套松弛法要求组员松弛身上的肌肉,完成后亦可使身心得到舒缓,顾及了深与广的问题。
就社交方面,小组提供了一个良好的气氛让组员互相交流,也可让那些较少参与小组活动的组员适应气氛,减低他们对社交的焦虑。
至于心理方面,每一节聚会都会有一个心灵故事跟组员一起分享。
这些故事的讯息十分正面,蕴含很多人生哲理。
组员在每一次聚会都会听到故事,这可以说是起了潜移默化的作用,工作员希望他们能从中学会豁达的人生态度。
此外,工作员鼓励组员尝试讲故事,目的是希望组员能够增强他们的自信心,亦可为其它组员带来一个相类经验(vicarious experience),提升他们的信心。
其它的环节则是以传授组员知识的认知层面为主。
介入点为长者设计一个小组活动,向他们介绍正面的人生态度,令他们明白一个健康生活的方式其实十分简单,可由自己出发。
工作员首先会让他们明白健康的生活元素,然后会让他们体验不同的健康生活小秘诀,例如每天做肌肉松弛练习及选择听一些令自己愉快的音乐。
藉此让他们学会自己帮助自己,促进全人健康。
介入理论活动学习法介入策略由于小组的对象为长者,新的知识或观念应建基于其现有的知识或观念(Zemke & Zemke,1995)。
透过让组员在过程中参与小组分享、游戏和讨论等活动,小组提供机会让他们主动地反省自己的生活模式。
另外,工作员亦采取促进诱导的方式(facilitation),并配合不同的官感的刺激(听觉,触觉及视觉),协助小组建立开放和互相接纳的气氛,并就组员的说话给予回馈(feedback)。
