中文版临床学习环境量表的信效度评定目的:翻译、修订中文版护生临床学习环境评估量表,初步评价其效度及信度,为评估我国护生临床学习环境质量,进一步提高临床教学质量提供一种科学、有效的工具。
方法:通过直译、回译、专家咨询和预试验对CLES+T进行翻译和文化调试,形成中文版CLES+T。
采取方便抽样对太原市内4所三级甲等医院抽取的220名护生进行调查,分析评价量表的信、效度。
通过项目分析、验证性因素分析检验量表的项目鉴别力和结构效度,对40名护生间隔两周重测,评定重测信度;Cronbach’s α系数计算内部一致性信度。
结果:(1)中文版量表各条目与总得分的相关系数r=0.31~0.70,P<0.01;(2)量表总Cronbach’s α系数为0.942,各因子的Cronbach’s α系数为0.689~0.872,重测信度和分半信度分别为0.922和0.883;(3)量表的内容效度比为0.96;经过验证性因子分析结果显示,各拟合指数均在可接受范围内。
结论:调试后的CLES+T具有良好的信效度,适合中国文化背景下护生临床学习环境的评估与测量。
但今后还需要进一步的研究对量表进行评估。
标签:临床学习环境评估量表;跨文化调适;临床学习环境;信度;效度【Abstract】Objective:To translate and develop the Chinese version of clinical learning environment,supervision and nurse teacher evaluation scale(CLES+T)and to test its reliability and validity in order to provide a scientific and effective tool for assessing the quality of nursing students’clinical learning environment and improving the quality of clinical teaching further.Method:The original version of the CLES+T was translated into Chinese,back translated and culturally modified.Data were collected among nursing students from 4 grade A class Ⅲ-hospital in Taiyuan City,with 200 students completing the questionnaire.The item analysis and confirmatory factor analysis were performed to test the discrimination of items and structure validity.40 nursing students were retested 2 weeks later for the test-retest reliability.Internal consistency reliability was tested using Cronbach’s alpha.Result:(1)The correlation coefficient of item and total ranged from 0.31 to 0.70.(2)The total Cronbach’s alpha of the scale was 0.942 and the Cronbach’s alpha of the factors ranged from 0.689 to 0.872.The test-retest correlation and split-half reliability were 0.922 and 0.883.(3)The content validity ratio (CVR)of the overall scale was 0.96.Confirmatory factor analysis indicated that the fitting indexes were all in acceptable level.Conclusion:The CLES+T has good validity and reliability.It can be used as a valid tool for the measurement and assessment of nursing students’clinical learning environment in Chinese cultural background.However,further investigation is required to develop and evaluate the questionnaire.【Key words】Clinical learning environment,supervision and nurse teacher evaluation scale;Cross-culture adaptation;Clinical learning environment;Reliability;Validity护理是以实践为基础的学科,强调从业人员的临床工作能力。

Introduction
Dementia is a serious and growing public health problem in the United States. As the prevalence of dementia increases with age, and as our population ages, an increasing number of people will be at risk for the various disorders which cause dementia. While there are more than 70 diseases that are thought to manifest in dementia, it is estimated that Alzheimer's Disease accounts for at least half of these cases, x The informal network of an individual suffering from dementia, usually the family, provides the majority of care in the early stages of disability. As the illness progresses, the behavior of a cognitively impaired person may include confusion, wandering, and outbursts of aggression. At some point, the stress and burden of providing care become overwhelming, and an alternative arrangement is sought by the caregiver(s). Often this alternative is institutionalization in a nursing home setting. 57% to as high as 78% of nursing home residents are estimated to have some type of dementia. 2 In recent years, there have been concerns and complaints by consumer/advocate groups regarding the quality and appropriateness of care provided for individuals with dementia in nursing homes. Many nursing homes have responded by segregating some of the residents suffering from dementia into Special Care Units (SCUs). The intent of SCUs is to optimize the quality of life for all residents in the facility: the cognitively intact as well as the cognitively impaired.

Advantages of SCUs
Special care units have grown from the belief that nursing home residents with dementia require specialized care that has not been routinely available in nursing homes. Essentially, the rationale for SCUs has been to provide an environment which enhances individualized care and behavioral approaches to managing residents with difficult behaviors. Characteristics of these dementia SCUs include admission of residents with dementia; specially selected, trained and supervised staff; activities specifically designed for the cognitively impaired; family involvement; and a physical environment designed for the enjoyment, safety and segregation of residents with dementia.4~ Advantages as well as disadvantages of special care units have been identified.4,6-8 One of the benefits of special care units is the ability to recruit and train professionals and direct care staff for the special unit and to reduce staff turnover and bum-out. Families anticipate that better care will be provided for their demented relative due to the specialized nature and expertise of a special care unit. SCUs meet the emotional needs of family members often through family support groups, coaching on behavioral management techniques, and participation in plan of care meetings. Residents of SCUs benefit from a coherent and consistent program in an environment which is personalized, dignified, safe, and predictable. Resident social interaction and autonomy are encouraged. Behavioral problems are recognized as a response to cognitive deficits, not as "acting out," and are managed with programmatic and preventive interventions. Special care units also can be problematic. Some families resist special care units because they deny the severity of a family member's illness. Families may be concerned that SCUs actually will provide less competent care. Other families may be concerned about the additional expense that will be incurred through placement in a special care unit. Many believe that SCU placement assumes a relentless progression of deterioration. Others believe that segregation of residents with dementia may deprive these residents of the social and physical stimulation with cognitively intact residents. Staff may limit their expectations of residents, and recruitment of staff may be difficult because of their working only with severely disturbed residents. Another difficulty is establishing admission and discharge criteria for a special care unit because of the large percentage of nursing home residents who have some level of cognitive impairment and may not be assigned to the SCU. There are also accusations that some special care units are specialized in name only with little attempt to provide appropriate programs and services.2
