二、交材料地点:校本部、大自考学员:在通知规定时间内将齐备的电子材料和书面材料一起提交到川农雅安新校区第七办公楼4-7(教务办)校外学习中心学员:在通知规定时间内将齐备的电子材料和书面材料一起提交到毕业前所在学习中心,由中心统一汇总整理,上报学院。
特别提醒:学位申报为学校统一为学员代收代报,均按照四川省人民政府学位委员会办公室文件要求提交材料,书面材料和电子材料缺一不可,未按照要求提供材料(包括填写的电子材料不规范)导致未通过审核的,根据省办要求不能再次申报。

9月7日至17日,报名者通过互联网登录报名网站,填写提交报名信息,获取并记住自己的报名编号。
(二)现场确认
9月25—27日(8:00—18:00),必须是完成网上报名的考生到郑州大学南校区图书馆二楼大厅报名点进行现场确认。否则,网上报名无效。
四、所有报名点只能接收本校的学生参加报名考试。省外各类教育机构在我省办班,其学生参加本次考试,须由学校所在省学位办出具委托函,报我办批准,并在指定报名点办理报名手续。报名点在办理此类报名时须在考生备注栏中注明省市以及办学机构名称,以便核发成绩,各报名点不得私自接受本报名点以外的考生报名。
五、网报结束后,省学位办审查考点报名信息。各考点于9月20—21日通过网上报名系统自行下载本考点考生信息。各报名点于10月9—10日将报名汇总信息送至河南省成人高等教育本科生申请学士学位外国语统一考试办公室(河南工业大学嵩山路校区中心实验楼九楼南侧)。各报名点报送的材料包括:①由报名系统生成的带有准考证号的考生信息数据库和考生图像信息光盘一式两份;②由报名系统打印出的报名情况汇总表一份;③河南省成人本科生申请学士学位外国语水平考试试卷申请单一份(详见附件3)。各报名点在送交上述材料前应仔细检查、核对,保证各项内容准确无误。所有送交的文字材料必须加盖报名点学校公章。
七、准考证发放及有关事宜省学位办委托各现场报名点发放准考证,准考证的发放时间由考点根据各自安排通知考生到报名点领取。我省成人高等教育本科毕业生申请学士学位外国语水平考试继续使用全国部分省级学位委员会办公室联合编写组编写,由中国人民大学出版社出版的《成人高等教育学士学位英语水平考试大纲(非英语专业)》、《成人高等教育学士学位日语水平考试大纲(非日语专业)》、《成人高等教育学士学位俄语水平考试大纲(非俄语专业)》。学校或考生可在省内新华书店购买。省考务办电话:0371—67756391、67756372,联系人:荣华。
四、报名费标准
按照河南省发展和改革委员会、河南省财政厅《关于确定我省成人高等教育本科毕业生申请学士学位进行外国语水平考试报名考务费收费标准的函》(豫发改收费函〔2004〕577号)规定,成人高等教育本科毕业生申请学士学位报名考试费标准为每人70元。
五、准考证发放及有关事宜
省学位办委托各现场报名点发放准考证,11月12—13日考生到郑州大学南校区图书馆二楼大厅领取准考证。
附件1 报名程序及要求
1、报名者登录网上报名网站,按要求填写并提交报考信息,获取并牢记自己的报名编号。①资格审查。9月24日前由考生到就读学校进行资格审查,具体时间由各校自定。考生报名表由考生网上报名成功后打印。②9月25—27日,完成网上报名考生持有效身份证件和报名系统打印出的报名信息表,到指定的报名点进行现场确认、照相、缴纳报名费,逾期不予补报。

六、报名费标准按照省发展和改革委员会、省财政厅《关于确定我省成人高等教育本科毕业生申请学士学位进行外国语水平考试报名考务费收费标准的函》(豫发改收费函〔2004〕577号)规定,成人本科毕业生申请学士学位报名考试费标准为每人70元。各校在报名时要公布收费项目和收费标准,严格按规定项目、标准、范围收费,不得擅自增加收费项目,提高收费标准或搭车收取其它费用,并自觉接受价格、财政、审计部门和社会的监督。请各报名点于10月10日前按每位报考者35元考试费用汇至河南省教育厅,开户行:交行政二支,账号:411060200018170024370。汇款时必须在汇款单上注明“成人学士学位考试费”。
二、报名对象
我校已注册的成人高等教育在读本科生(包括:函授、业余、脱产、网络或其他),我校高等教育自学考试主考专业的在读本科生、2009年6月以后(含6月)毕业的自学考试本科生,拟申请学士学位者,均需参加本次考试。拟申请第二学士学位者,须再次参加本考试。

根据《豫学位办〔2010〕6号》文件精神,现将2010年我校成人高等教育本科毕业生申请学士学位外国语水平统一考试报名工作的有关事宜通知如下:
一、考试时间及语种
考试时间:2010年11月14日(星期日)上午9:00至11:00进行。
考试语种:英语、日语、俄语。本次考试不考听力部分。
六、考试大纲
我省成人高等教育本科毕业生申请学士学位进行外国语水平考试继续使用全国部分省级学位委员会办公室联合编写组编写,由中国人民大学出版社出版的《成人高等教育学士学位英语水平考试大纲(非英语专业)》、《成人高等教育学士学位日语水平考试大纲(非日语专业)》、《成人高等教育学士学位俄语水平考试大纲(非俄语专业)》。考生可自行到省内新华书店购买。
2、准考证编制说明
河南省成人本科毕业生申请学士学位外国语水平考试准考证号由15位阿拉伯数字组成,其组成顺序为:省市代码2位+学校代码3位+校区1位+年度2位+考试次数1位+语种代码1位+考场号3位+座位号2位,共计15位。例如:准考证号410011091100101此例中依次,“41”代表河南;“001”代表郑州大学考点;“1”代表校区(各学校校区均设定为‘1’);“09”代表2009年度;“1”代表第一次考试;“1”代表英语;“001”代表第一考场;“01”代表第一考场的第一位考生。3、语种代码说明外国语水平考试语种中,“1”代表英语;“2”代表日语;“3”代表俄语。
报名者须在规定时间内由本人持有效身份证件、学生证、报名成功后打印的考生报名登记表[表上须粘贴考生照片并加盖考生所在院(系)或函授站公章]、网报编号到郑州大学南校区图书馆二楼大厅报名点,按规定缴纳报名考试费、照相、确认报名信息等,逾期不予办理。
本报名点只接受郑州大学符合条件的学生报名考试。
2、考生当场核对打印出的考生信息校对单(一式两份,一份签名留报名点备查,一份考生本人留存),认真检查并阅读确认,报名正确与否由考生本人负责。现场确认结束后不得再修改。
