发声障碍评定技术康复治疗操作规范一、发声的客观评定1、定义:发声障碍又称嗓音障碍,通过应用仪器设备对语音参数进行定量化分析,以求达到较客观的描述语音的方法。
2、适应证与禁忌证(1)适应证:各种发声障碍患者。
(2)禁忌证:严重精神异常或极不合作者。
3、设备与用具电声门图仪、计算机辅助的共鸣障碍诊治系统或相应设备、计算机辅助的构音功能评定与训练系统或相应设备。
4、操作方法及步骤(1)鼻流量的检查:使用共鸣障碍诊治系统阅读特定的材料,分别采集口腔和鼻腔声音,共鸣障碍诊治系统分析计算口腔和鼻腔的声压级,从而计算出鼻流量。
(2)电声门图仪检测:将两电极置于甲状软骨水平的位置,要求患者发“æ”,电声门图仪可根据声门不同状态时电阻的大小,描计出电声门图,检测发声障碍的参数变化。
(3)嗓音检测:使用计算机辅助的构音功能评定与训练系统,按照系统要求发出目标音,系统会自动生成基频、语音强度、共振峰、最大发声时间、语音微扰、谐噪比等语音学参数。
5、评定标准(1)鼻流量检查时,参考正常人鼻流量值,系统可作出相应判断。
(2)电声门图仪检测时,正常的电声门图为一随时间变化、光滑有规律的类似正弦的曲线,声带振动或运动的异常会导致电声门图的波幅、波形和频率周期的改变,以此来判断声带的病变。
(3)嗓音检测:参考语音参数分析发生障碍的语音变化。
6、注意事项评定中要求隔音室或相对安静的环境,注意麦克风与患者的距离。
二、声质的评定1、定义声质是指声音的独特品质和色彩,声质评定常用来评价个体的声音是否圆润,是否动听及是否音色饱满。
临床上指的是声音的个性,是对听者的主观感受而言。
当声带振动以及共鸣器官变化时会造成个体声质的异常,因此音质评定对声带及共鸣器官的器质性疾病及功能有重要的提示。
2、适应证与禁忌证(1)适应证:各种发声障碍患者。
(2)禁忌证:严重精神异常或极不合作者。
3、设备与用具GRBAS评价工具。
4、操作方法及步骤GRBAS评价方法包含5个描述参数:声音嘶哑总分度G(overall grade degree,G)、粗糙声R(rough,R)、气息声B(breath B)、无力声A(asthenia,A)、紧张声S(strathy,S)。

消融针针头材质为LNCONEL625镍铬合金(适用MRI设备)或AIS1316L不锈钢(适用非MRI设备)。
与低温冷冻手术系统配合使用实施冷冻消融术。

(二)钛网修复体:用于神经外科领域的颅骨修补。

电声测试仪知识一、概述(一)用途电声测试仪是用于测量电话机、电声器件的电声性能的测量仪器,现在多由台式个人计算机配合相应的电路和软件及电声测试专用配件(仿真嘴、仿真耳等)组成智能化测试仪器。
只需更换软件,就可按有关国家标准测试电话机和各种电声器件,如扬声器、受话器、耳机、送话器、传声器、音头、咪头等的电声特性,直接在彩色显示器上显示电话机的发送、接收、侧音之响度评定值及频响曲线、回损、失真度等指标,以及电声器件的灵敏度、平均灵敏度、频响、阻抗频率特性、谐波失真等指标。

(二)分类与特点电声测试仪按功能来分:分为专用于电话机测试的电话电声性能测试仪、专用于电声器件测试的电声器件测试仪和作为通用电声性能测试的通用电声测试仪。

耳鼻咽喉科嗓音功能评价法一、喉动态镜检查喉动态镜是利用频闪光源照射来观察声带振动特征的检查仪器。
StanIPer首先发明了机械式喉动态镜。
KalIer利用电子闪光管原理研制了电子喉动态镜。
钟子良在国内首次应用电子喉动态镜。
近年喉动态镜有很大发展,将其与支撑喉镜、手术显微镜连接,在进行喉显微外科手术时能同步观察声带振动状况,更有利于提高手术效果;将喉动态镜与摄像系统及电子计算机连接,可在检查的同时将声带振动情况摄像、显示并存储、打印。
(一)检查原理正常发声时声带振动非常迅速,为100〜250次∕s,而人的视觉只能辨别每秒不超过16次的振动,故用肉眼无法观察高速的声带振动,因此,必须借助于某种方法使声带这种快速振动相对减慢。
喉动态镜即是利用快速闪烁的光源照射使声带形成一种似乎静止或缓慢活动的光学幻影,当光源闪烁频率与声带振动频率同步时,声带则好像不运动(静相);当闪光频率与声带振动频率有差别时,则可看到声带似乎缓慢振动(动相),所见到的振动频率是声带实际振动频率与闪光频率之差。
喉动态镜由频闪光源系统、接触麦克风、脚踏开关、喉内镜、摄像系统及显示器等构成。
工作时声带振动频率通过接触麦克风、声频放大器传至差频产生器,由差频产生器根据声带振动频率调节频闪光源的频率(脚踏开关亦可调节频闪光源的频率),频闪光源通过硬管或软管喉镜照射在声带上,使肉眼观察到的声带振动速度相对变慢或静止。
(二)检查方法检查前用1%丁卡因液行咽部黏膜表面麻醉。
检查时受试者取坐位,将接触麦克风固定于颈前喉体部,头稍后仰,张口伸舌,检查者一手用纱布包裹舌前部向外轻轻牵拉,另一手将喉镜伸入口咽,嘱受试者发“i”音,脚踏开关控制闪光频率,观察不同音调(高音、低音)、音强(强音、弱音)、声区(真声、假声)的声带振动情况(静相或动相)。
(三)观察内容L基频声带振动固有频率可白喉动态镜仪器上显示出。
基频与年龄、性别有关,声带病理情况下基频可下降或升高。

目前临床上应用的电声门图检测系统多采用美国Tiger Electronics(上海)公司生产的Tiger DRS. Inc 电声门图仪进行测试,采用Dr. SpeechScienc电声门图是一种无创性喉功能检测手段,弥补喉内镜检查的不足。
目前临床上应用的电声门图检测系统多采用美国Tiger Electronics(上海)公司生产的Tiger DRS. Inc 电声门图仪进行测试,采用Dr. SpeechScience forwindows 4.0 软件进行分析。
检测时将金属电极板放置于双侧甲状软骨板上,受检者发自然胸声区稳态元音/.//i/,持续3 秒,记录受声带振动调节的电极间阻抗变化,在体表描记出声门开闭曲线,可描记闭合相(CP)、开放相(OP),闭合相又分为渐闭(CCP)、渐开相(COP),OP相反映声带振动时被声门下气流吹开的瞬间,CCP、COP、与OP三者有周期性的衔接,正常电声门图波形呈规律性正弦曲线。
EGG 可测量的参数及临床意义:1、接触率(CQ):测量声带振动时声门闭合程度,即接触时间与周期之比,主要反映声带水平方向上的开闭,发声频率提高,声带被拉长,双侧声带接触面积减小,闭合度降低,CQ 值下降;2、接触幂(CI):测量声带振动时渐闭相与渐开相的对称度,在一定程度上体现了声带开闭运动在垂直面上相位差,该参数对声带麻痹非常敏感;3、接触商微扰(CQP):测量相邻振动周期间CQ 振动度;4、接触幂微扰(CIP):测量相邻振动周期间CI 扰动度。
如果声带的关闭和开放有规律,微扰量就低,即接触率微扰和接触幂微扰的值较小;5、测试声带粘膜波的接触性,反映粘膜波运动是否规则;6、测试方便无创,不受上气道干扰,更符合声带振动测量的要求,适合儿童等各种不宜做喉镜检查的病人;7、电声门图波形异常的类型与声带病变的位置和大小有关,通过与正常波形进行比较之后,能客观地获得病人声带的信息,特别能够捕捉间接喉镜检查时易遗漏的声带下缘或前联合的病变,尤其能够提供声带麻痹的证据。
