干细胞在心肌内存活、分化不足使干细胞移植疗法停滞不前。

因此,如何增加干细胞在心肌内存活、分化,提高干细胞移植治疗心肌梗死疗效是目前急需解决的主要问题。

1 心肌梗死后心肌内环境变化心肌内环境对干细胞在心肌内存活、分化起决定性作用。

心肌内环境是心肌细胞赖以生存的环境,心肌细胞、间质细胞、成纤维细胞产生或分泌的细胞外基质成分及某些生物活性分子,对心肌细胞发挥支持、连接、营养和保护等作用[5]。

急性心肌梗死发生后,继发于心肌细胞坏死,外周血中性粒细胞和单核细胞短时间内募集到受损坏死心肌组织。

心肌梗死急性期心肌组织内微环境突出表现为大量中性粒细胞、单核细胞浸润和肿瘤坏死因子α、白细胞介素(interleukin,IL)1β、IL-6等促炎因子显著增加。

其次,WST-8产生的formazan 比XTT 和MTS 产生的formazan 更易溶解。

再次,WST-8比XTT 和MTS 更加稳定,使实验结果更加稳定。

另外,WST-8和MTT 、XTT 等相比线性范围更宽,灵敏度更高。

WST-8和WST-1相比,检测灵敏度更高,更易溶解,并且更加稳定。

本试剂盒可以用于细胞因子等诱导的细胞增殖检测,也可以用于抗癌药物等对细胞有毒试剂诱导的细胞毒性检测,或一些药物诱导的细胞生长抑制检测。

关键词:膝骨关节炎;间充质干细胞;临床研究;自体;同种异体前言膝骨关节炎(Knee Osteoarthritis, KOA)是一种以退行性病理改变为基础的疾患,常见于中老年人群。

该疾病初期症状较轻,临床表现多为膝盖部位肿胀、酸痛、行动不适,坐立姿势改变时疼痛、弹响等,严重时出现活动受限、积液、关节畸形,不及时得到有效治疗病情可能发展为残疾[1]。

KOA常由长期姿势僵化、劳累、外伤、或其它关节退行性病变如软骨退化、半月板磨损等原因导致。

因此该病的治疗关键在于保护软骨,防止进一步受损,同时尽可能使其自我修复和再生。

但由于膝关节部位软骨组织无血管,因此软骨部位自我修复和再生的能力很差,且关节软骨缺损的再生方法很少[2]。

KOA一般采用综合治疗,包括病人教育,药物治疗,理疗或外科手术治疗,现有的治疗方式包括软骨保护剂硫酸氨基葡萄糖、透明质酸关节腔注射、中医针灸推拿、膝关节置换以及间充质干细胞治疗等,其远期效果大多并不理想,而近年来随着对间充质干细胞的研究逐步深入,人们发现其可促进软骨再生的特点,因而以间充质干细胞移植为主的治疗方法,在国内外逐渐兴起,并开始进入临床实验阶段。

本综述将主要就间充质干细胞治疗KOA的原理、以及相关临床研究进行汇总,并为KOA相关的研究和间充质干细胞的应用提供参考。

方法选择成年雄性蒙古沙鼠(60-80 克),沙鼠腹腔注射戊巴比妥钠1% (v/w)麻醉,夹闭双侧颈总动脉5分钟诱导缺血,后松解恢复血流。

不同组采用SPM促脑发育训练手段,最终分别检测各组的组织形态、蛋白及凋亡检测等相关数据。

结果 SPM促脑发育训练可以增加剪切力,继而作用于PECAM-1/eNOS/NO通路影响脑血管内皮功能,内皮功能改善有助于缺氧缺血性脑病的康复。

结论 SPM促脑发育训练增加脑剪切力,剪切力通过PECAM-1/eNOS/NO通路改善血管内皮功能,继而改善缺氧缺血性脑病介导的脑损伤。

关键词:缺氧缺血性脑病;PECAM-1/eNOS/NO;剪切力;脑损伤SPM brain development training mediates the effects of brain shear stress pathway on the mechanism of hypoxic ischemic encephalopathyAbstract: Objective To explore the function of cerebral vascular endothelium in the pathological mechanism of neonatal hypoxic ischemic encephalopathy, which may be related to the regulation of PECAM-1/eNOS/NO pathway mediated by SPM promoting brain development training and increasing cerebral vascular shear force, and ultimately achieve brain protection. Methods Adult male Mongolian gerbils (60-80g) were selected and anesthetized by intraperitoneal injection ofpentobarbital sodium 1% (v/w). Bilateral common carotid arteries wereoccluded for 5 minutes to induce ischemia, and then released torestore blood flow. Different groups were trained with SPM to promote brain development, and finally relevant data such as tissue morphology, protein, and apoptosis detection were detected in each group. Results SPM brain development training can increase shear stress, which inturn acts on the PECAM-1/eNOS/NO pathway and affects the endothelial function of cerebral blood vessels. The improvement of endothelial function is conducive to the rehabilitation of hypoxic ischemic encephalopathy. When used in hematopoietic stem cell transplantation, umbilical cord blood offers several distinct advantages over bone marrow or peripheral stem cells. However, umbilical cord blood collection is not part of routine obstetric care and is not medically indicated. Umbilical cord blood collection should not compromise obstetric or neonatal care or alter routine practice for the timing of umbilical cord clamping. If a patient requests information on umbilical cord blood banking, balanced and accurate information regarding the advantages and disadvantages of public and private umbilical cord blood bank-ing should be provided. Biologically, a greater degree of human leukocyte antigen mismatch is tolerated by the recipient and the incidence of acute graft-versus-host reaction is decreased w hen umbilical cord blood is used compared with unrelated donor bone marrow (1, 2). The predominant disadvantage of umbili-cal cord blood use is that there is often a low yield of stem cells acquired per unit. Only 8–12% of umbilical cord blood units have sufficient cell volume for transplant to a person weighing 80 kg (176 lb) (3). However, the use of combined units of umbilical cord blood allows for the expansion of umbilical cord blood volume (and increased number of stem cells) to be used for adult hematopoi-etic transplants. Since the first successful umbilical cord blood transplant in 1988, it has been estimated that more than 30,000 transplants have been performed in children and adults for the correction of inborn errors of metabolism, hematopoietic malignancies, and genetic disorders of the blood and immune system (4). Umbilical cord blood stem cells also are being studied in the areas of regenerative medicine and infectious disease (www. ).Public Versus Private Umbilical Cord Blood BankingTwo types of banks have emerged for the collection and storage of umbilical cord blood: 1) public banks and 2) private banks. The first public bank w as established at the New York Blood Center in 1991 and other public banks have since been established in various regions of the country. In December 2005, federal legislation, the C.W. Bill Young Cell Transplantation Act, w as enacted that provides funding for continued growth of a national umbilical cord blood registry in the United States. Some states have passed legislation requiring physicians to inform their patients about umbilical cord blood banking options. Obstetrician–gynecologists and other obstetric care providers should consult their state medical associa-tions for more information regarding state laws.Public banks promote allogeneic (related or unre-lated) donation, analogous to the current collection of whole blood units in the United States. These banks typi-cally are associated with a local network of obstetric hos-pitals that send their units of blood to a central processing facility. A minority of public banks w ill accept units through shipment by an overnight express courier (5).A list of participating hospitals is maintained by the National Marrow Donor Program (6). Public banks are supported through government grants, private dona-tions, and compensation for cord blood units used for transplant. Units of umbilical cord blood collected for public banks must meet rigorous standards of donor screening and infectious disease testing as outlined by the U.S. Food and Drug Administration. As of October 20, 2011, every unrelated donor cord blood unit to be transplanted in the United States must be either licensed or covered under an investigational new drug application approved by the U.S. Food and Drug Administration (7). Initial human leukocyte antigen typing of these units allows them to be entered into computerized registries so that when the need arises, a specific unit can be rapidly located for a patient.Private for-profit banks w ere initially developed to store stem cells from umbilical cord blood for autologous use (taken from an individual for subsequent use by the same individual) if the child develops disease later in life or for use by other family members. Private banks adver-tise directly to consumers often encouraging parents to bank their infants’ cord blood as a form of “biological insurance.” The routine storage of umbilical cord blood as biological insurance against future disease is not rec-ommended by the American Academy of Pediatrics, given the lack of scientific data to support its use and availability of allogeneic transplantation (8). Physicians or other professionals who recruit pregnant women and their families for for-profit umbilical cord blood banking should disclose any financial interests or other potential conflicts of interest.Considerations for Patients Regarding Umbilical Cord Blood BankingIf a patient requests information about umbilical cord blood banking, balanced and accurate information regarding the advantages and disadvantages of public and private banking should be provided. 在有些医疗机构,脐血气测定已经成为常规。

美国妇产科学院(xx)推荐在新生儿Apgar评分低的时候使用。

尽管脐血气无论是对即刻和远期的神经系统损伤均不具有良好的预测价值,但是对于理解导致酸中毒的产中以及分娩事件是有帮助的。

表D-1列出了足月和早产儿脐血气动脉血的正常值(均值±标准差)。

脐带血的收集在分娩后立即用2把钳子钳夹胎儿侧的脐带,另外2把钳子钳夹胎盘侧,切断后,留取10~20cm长度的脐带。

把血从脐动脉里面留到一个1~2ml肝素化了的注射器里面,加上针帽,注射器放到一个加有碎冰的塑料盒里面,立即转运到实验室。

胎儿酸中毒的生理胎儿可以通过胎盘迅速清除CO2,如果CO2不能快速得到清除,H2CO3(碳酸)在体内蓄积,就会导致呼吸性酸中毒。

由机体厌氧代谢产生的有机酸,包括乳酸和β羟基丁酸从胎儿血中清除缓慢,蓄积造成了代谢性酸中毒。

随着代谢性酸中毒的发展,碳酸氢根(HCO3)因与有机酸缓冲结合而下降。

随着H2CO3和有机酸(表现为HCO3下降)的增加,表现为混合性呼吸性——代谢性酸中毒。

为了临床的目的,HCO3代表的是代谢性的成分,以mEq/L报告,H2CO3浓度代表的是呼吸性的成分,以PCO2来报告,单位为mmHg。

碱剩余是用来衡量HCO3的缓冲能力的,譬如,随着代谢性酸中毒的加重,HCO3将下降以维持正常的pH值。

碱不足是指在HCO3浓度下降至正常水平以下,碱过多是指HCO3高于正常。

呼吸性酸中毒呼吸性酸中毒通常是在胎盘气体交换急性中断时发生的,继发CO2潴留。

脐带一过性受压是发生胎儿呼吸性酸中毒的最常见事件。

通常情况下,呼吸性酸中毒不会对胎儿产生损害,表D-2显示了呼吸性酸中毒时脐动脉血每一成分的阈值。

代谢性酸中毒代谢性酸中毒是在氧气供应无法满足胎儿细胞能量需要的厌氧代谢所需要的时间和幅度时产生的,代谢性酸中毒和缺血缺氧性脑病以及新生儿的功能障碍发生有关,但是即便是重度的代谢性酸中毒也不能预测继发的脑瘫。

截止到1998年,美国纽约中心冻存CB已超过8 000份,提供临床应用600余份[2]。

CB已成为骨髓和外周血后的第3种造血干细胞来源,日益受到人们的重视,CBB在全世界范围内广泛建立起来。

成为近十年来造血干细胞移植领域中的重要进展之一。

CBB的建立涉及到脐血的采集、分离、冷冻、复苏、检测、建库等诸多环节。

1997年至2000年5月,我们已冻存经HLA检测的CB 3 744份。

CB采集量是CBB的重要指标之一。

CBB量多,造血干细胞含量多,移植效果好。

伦敦CBB 1998年采集量为1 000份样本,平均666份CB采集量为(70±23) ml(40~181 ml)[3]。

本研究根据国内外文献建立了一套CB采集的分离技术。

通常CB的采集分为胎盘娩出本论文由无忧论文网整理提供前和娩出后采集,有文献报道胎盘娩出后所采集的CB有核细胞数低于胎盘娩前所采集到的有核细胞数[4]。

本研究采用的是胎盘娩前采集的方法,平均采集量为(93±22) ml(45~198 ml),平均采集到的有核细胞数为11.2×108,且采集过程对母亲和新生儿均无影响。

同时我们也观察到采集的CB中的有核细胞数与CB量之间存在线性相关,r=0.67,P<0.01。

因此,尽可能多的采集CB量可增加采集到的有核细胞数。

方法收集200例产妇作为入选对象,其中100例为水中分娩组,对应100例常规分娩作为对照组。

收集两种不同分娩方式的脐带血,比较两者脐带血采集的质量。

结果水中分娩方式采集的脐带血采集量、有核细胞数、细菌污染率和CD34+造血干细胞数及活性均优于常规分娩方式采集的脐带血。

结论分娩方式是影响脐带血质量的重要因素,水中分娩是提高脐带血采集质量的有效分娩方法,可为临床移植提供更高质量的脐带血造血干细胞。

Methods Total of 200 pregnant women were studied and divided into two groups: water delivery group (n=100) and conventional delivery group(n=100). Humanumbilical cord blood was collected from both groups and analyzed in quality. Results The amount of collected cord blood, the number of nucleated cells, the rate of bacterial contamination,the activity and number of CD34+ hematopoietic stem cells from water delivery group were over to those from conventional delivery group. Conclusion Delivery style is an important factor that affects the quality of umbilical cord blood. 方法 回顾1例因高危急性髓系白血病接受非血缘脐血移植、其后出现顽固性皮疹的患儿的临床资料及诊治过程,以“造血干细胞移植”“免疫重建”和“急性移植物抗宿主病”的中英文为检索词,对以下数据库的相关论文进行检索:PubMed、Web of Science、CNKI、万方数据知识服务平台,收集检索到的病例资料并进行分析。

结果 该例1岁9月龄女性患儿成功获得造血重建,移植后22 d患儿头部、后颈部出现密集红色丘疹(面积约19%)、瘙痒明显,口服他克莫司、外用激素类药物后皮疹无好转(面积>90%),此时监测CD3+细胞、CD8+细胞、CD3+CD69+细胞、CD3+HLA-DR+细胞比例明显升高,调节性T细胞(Treg)比例下降。

免疫指标支持T淋巴细胞活化,考虑Ⅱ度aGVHD(皮肤3级),遂予加强免疫抑制治疗方案。

期间患儿皮疹有消退,但仍反复,并伴皮肤明显脱屑,复查CD3+细胞、CD8+细胞、CD3+HLA-DR+细胞比例仍偏高,遂再次调整治疗方案、加强抗排斥,患儿皮肤aGVHD好转。

定期监测原发病完全缓解、植入比例100%、免疫重建稳定,随访至移植后32个月,患儿无病存活。

检索到相关文献9 篇,均论证了早期CD3+细胞、CD8+细胞、活化T淋巴细胞比例升高以及Treg下调与aGVHD发生相关。

结论 对异基因造血干细胞移植术后患者的免疫状态进行动态监测,有利于评估其免疫重建情况及疾病状态(如aGVHD),有助于制定合理的免疫抑制治疗方案,使患者获得良好预后。

Methods Clinical data and diagnosis and treatment of a child with high-risk acute myeloid leukemia who developed intractable rash after receiving unrelated umbilical cord blood transplantation were retrospectively analyzed. Using the keywords of “hematopoietic stem cell transplantation”“immune reconstitution” and “acute graft-versus-host disease”, relevant literatures were searched from PubMed, Web of Science, CNKI and Wanfang databases, and relevant cases were collected and analyzed. Results Hematopoietic reconstruction was successfully achieved in this 1 year 9 months old girl. On day 22 after transplantation, the skin of head and back neck of the patient showed dense red papules (approximately 19% in area), with obvious itching. Under the treatment of oral tacrolimus and topical hormone drugs, the rash was not alleviated (area>90%). Meanwhile, the proportion of CD3+ cells, CD8+ cells, CD3+CD69+cells and CD3+HLA-DR+cells was signi fi cantly increased, and the proportion of Treg cells was decreased. The immune indexes supported T lymphocyte activation, then diagnosis of gradeⅡaGVHD (skin grade 3) was made, and the immunosuppressive regimen was strengthened. During this period,the skin rash of the child subsided, but recurred, and accompanied by obvious skin desquamation. The proportion of CD3+ cells, CD8+ cells, and CD3+HLA-DR+cells remains relatively high, supporting the diagnosis of aGVHD. Therefore, the anti-GVHD regimen was adjusted and strengthened, and the aGVHD of the child’s skin was mitigated. Regular monitoring showed that the primary disease was completely relieved, the implantation rate was 100%, and the immune reconstitution was stable. During 32-month follow-up after transplantation, the child obtained disease-free survival. EU-MSCs-CM、MSCs-CM处理经脂多糖(LPS)诱导的THP-1巨噬细胞,流式细胞术检测THP-1巨噬细胞表面标志物CD11b、CD86、CD206的表达情况,qPCR检测THP-1巨噬细胞促炎基因TNF-α、IL-1β、IL-6的表达情况。

【结果】 MTT法结果显示在0、7.5、15 µg/mL浓度处理细胞24 h、48 h 后,细胞活力保持在90%以上;体外划痕显示,丁香酚处理可以增强HUC-MSCs迁移能力。

WB结果显示,与MSCs-CM处理相比,EU-MSCs-CM处理对活化状态的HSCs的α-SMA、COL1A1、Smad2/3、p-Smad2/3表达抑制作用更加显著。

方法:将符合入组标准的200例孕妇均分为观察组(妊娠32~36周每日口服益生菌制剂1袋)及对照组(不予干预),再依据分娩方式分为剖宫产组与经阴道分娩组,收集各组孕妇一般资料[孕妇年龄、孕期增重、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、血红蛋白(HGB)含量及肌酐水平]及新生儿一般资料[新生儿性别比例、出生孕周、体质量、身长、阿氏(Apgar)评分]、新生儿脐静脉血总胆红素(TB)、结合胆红素(CB)、未结合胆红素(UCB),比较新生儿出生后24、48及72h的经皮胆红素(TCB)值。

结果:各组孕妇及新生儿一般情况比较,差异无统计学意义(P>0 05);观察组中剖宫产及经阴道分娩新生儿脐带血TB、UCB均低于对照组相同分娩方式新生儿,差异有统计学意义(P<0 05);观察组中剖宫产及经阴道分娩新生儿出生后24、48及72h的TCB均低于对照组相同分娩方式新生儿,48h与72h的TCB比较,差异有统计学意义(P<0 05);2组新生儿中,经阴道分娩新生儿脐带血TB、UCB及CB均低于剖宫产新生儿,但差异无统计学意义(P>0 05);观察组经阴道分娩新生儿出生后24、48及72h的TCB均低于本组剖宫产新生儿,48h与72h的TCB比较,差异有统计学意义(P<0 05);对照组经阴道分娩新生儿48及72h的TCB低于本组剖宫产新生儿,差异有统计学意义(P<0 05)。

结论:孕母妊娠32~36周补充益生菌制剂,可促进新生儿胆红素的代谢,降低新生儿胆红素水平;且经阴道分娩方式可以促进新生儿胆红素代谢。

Flow-cytometry and immunoturbidimetry (ITM) were used to detect the content of T lymphocyticsubgroup and immune globulin and the alexinic in plasma of all the patients prior and post-transplantation.RESULT AND CONXLUSION: Compare with pre-transplantation, the content of CD3+, IgA and IgG was decreased and thedifferences were significant; The content of CD4+, CD8+, CD4+/CD8+, IgM, C3 and C4 was also changed and the differences werenot significant. It indicates that UCB-MSCs transplantation could not activate the immunological response of cell and humoralimmunity, so the transplantation is safety. 1 新生儿脐带血血气分析的操作与意义
脐带血血气分析指的是胎儿分娩出以后,快速的采集脐 带以及检测血气,临床上主要用来分娩出但是无法建立自主 呼 吸 的 胎 儿 中,在 直 视 的 情 况 下,在 胎 儿 的 一 侧 长 度 大 约 为 10 厘米的脐带处,用两把消毒止血钳钳夹主外侧脐带,并剪 断;再应用肝素化注射器对脐动脉或静脉血进行穿刺,密闭封 好后送检,得出血气分析值 [3]。一般正常情况下,胎儿体内的 酸 碱 处 于 平 衡 状 态;胎 儿 若 是 处 于 缺 氧 状 态 的 话,胎 盘 和 血 管内可能出现缓冲的情况,从而产生过度的① CO2、②乳酸堆 积,直至酸碱平衡失去代偿能力,进而表现出酸中毒的现象。 脐动脉的血流是从胎儿流出到胎盘绒毛间隙,可以有效的将 胎儿具体情况反映出来;脐静脉血流主要是从胎盘流向胎儿, 可以有效的将产妇的酸碱平衡与胎盘功能反映出来 [4]。因此, 可以通过检测动脉血气分析值,监测新生儿是否有窒息现象
存在,以及其他不良预后情况存在。
2 新生儿脐带血血气分析正常范围参考值
因研究对象不同,再加上脐带血气分值参数容易受到诸 多因素的影响,例如①胎龄、②分娩方式等,导致各参数正常 值,目前未能形成统一的参考范围。以往有研究数据显示:正 常新生儿脐带血血气分析正常值的测值范围为:①动脉血 SO2 平均值为:24.3%;② PH 平均值为:7.25;③ BE 值为:-4.3mol; ④ PO2 值 为:16mmHg;⑤ PCO2 值 为:50mmHg[5]。 新 生 儿 脐 动脉血气指标研究写作组,经期研究脐动脉血 pH 值与 BE 值 的统计学参考范围分别为 (7.20±0.20) 与 (-7.64±10.02);新 生儿窒息脐动脉血,pH 临床校正正常范围约为 >7.00~>7.20, BE 分布范围为 >-10~>-18[6]。

人脐血干细胞(HUCBC)移植则是干细胞移植中的热点。

虽然相关研究多为基础研究[1-7],但临床效果也不容忽视。

1.1 入院时情况患者男性,56岁,因“反复发作胸闷、胸痛7个月加”于2013年8月5日入院。

患者7个月前劳累后出现胸闷、胸痛,呈持续性,就诊于“沈阳军区总医院”,诊断为“急性下壁心肌梗死”,予以急诊行冠状动脉造影检查示:左主干(LM)(-),左前降支(LAD)近中段多处狭窄50%~60%,冠状动脉血流(TIMI血流)3级,回旋支(LCX)近中段狭窄70%,TIMI血流3级,右冠状动脉(RCA)近段80%狭窄,中段100%闭塞,TIMI血流0级,右优势型。

干预RCA,于RCA置入2枚支架,术中患者出现持续性血压下降,最低至80/60mmHg,予以升压药对症治疗(0.9%生理盐水500mL,多巴胺40mg,阿拉明40mg25mL/ h静脉泵入),并予以IABP支持治疗。

术后患者胸闷、胸痛症状缓解,病情好转出院。

出院后一直系统内科药物治疗。

1个月前无明显诱因再次出现胸闷、气短症状,伴有周身水肿、夜间不能平卧,遂来本院就诊,门诊以“冠心病、心力衰竭”收入院。

既往史:有糖尿病病史5年,自行应用胰岛素治疗,血糖控制欠佳。

入院查体:血压98/55mmHg,心率109次/min,双肺可闻及湿性啰音及少量干鸣音,心界向两侧扩大,心音低钝,律齐,双侧上、下肢及阴囊水肿明显。

辅助检查:心电图示:Ⅱ、Ⅲ、AVF 可见病理性Q波,伴有ST段抬高0.1~0.2mV。

方法方法:选择要求行分娩镇痛的足月单胎初产妇90例陆续入组观察,采用随机数字表法将研究对象产妇随机分为PIEB组和持续性硬膜外输注(CEI)组,每组45例。

2组产妇均给予首次剂量(0.08g·L-1罗哌卡因+0.4mg·L-1右美托咪定)10mL。

PIEB组产妇注入首次剂量后1h连接PIEB镇痛泵,脉冲剂量8mL·h-1,速度7mL·min-1。

CEI组产妇注入首次剂量后立即连接CEI镇痛泵,持续背景剂量8mL·h-1。

比较2组产妇镇痛前(T0)、镇痛后30min(T1)、镇痛后1h(T2)、镇痛后2h(T3)和宫口开全时(T4)的视觉模拟评分(VAS)。

比较2组T0~T3的胎儿脐动脉波动指数(PI)、脐动脉阻力指数(RI)和脐动脉血流速度收缩峰值与舒张峰值比值(S/D);比较2组胎儿娩出即刻的脐动脉血气pH值、剩余碱(BE)值和新生儿Apgar评分;比较2组产妇第一产程时间、第二产程时间、满意度和不良反应。

结果结果:2组产妇在T0、T1、T2和T3的VAS评分比较差异无统计学意义(P>0.05);PIEB组T4时间点的VAS评分低于CEI组(P<0.05)。

Fasting blood glucose, HbA1c and C peptide levelswere significantly improved in stem cell therapy group compared with control group. Li Bo, Zhao Hong-guang, Zhong Hong, Liu Rui-jun, Ma Nan, Shan Gen-fa, Mei Ju, Zhang Fu-xian,ct BACKGROUND: Endothelial progenitor cells are the cells that can form new blood vessels in the way of angiogenesis in the body, which updates the conventional theory of angiogenesis, vascular damage and repair after birth and provides new ideas for research and treatment of ischemic diseases. OBJECTIVE: To investigate the effects of dog umbilical cord blood endothelial progenitor cell (UCB-EPC) transplantation on angiogenesis after myocardial infarction. DESIGN, TIME AND SETTING: An in vivo cytological experiment was performed at the Laboratory Center of Xinhua Hospital between May 2006 and March 2007. MATERIALS: One full-term pregnant hybrid dog was included for preparation of UCB-EPCs. Thirty-six adult dogs were randomly divided into a cell transplantation group (n = 18) and a model control group (n = 18). METHODS: Acute myocardial infarction model was established in each group by ligation of anterior descending coronary artery. In the cell transplantation group, 2 mL physiological saline containing 5×106 BrdU-labeled EPCs was injected into the coronary artery, while in the model control group, simple physiological saline of the same amount was given. At 1, 4, and 8 weeks after transplantation, dogs were sacrificed for harvesting myocardial tissue. MAIN OUTCOME MEASURES: Myocardial infarction was confirmed by hematoxylin-eosin staining. Myocardial angiogenesis was observed by BrdU immunohistochemical staining. The number of infarcted myocardial vessels was calculated by von Willebrand (vW) factor staining. RESULTS: There was plenty of scar tissue, fibroblasts, and small vessels in the myocardial infarction region. In the cell transplantation group, brown yellow particles (BrdU-positive expression) appeared in some nuclei in small vessels from infarcted myocardium. Newly formed vessels were not found in the model control group. In the cell transplantation group, brown yellow particles (vW factor-positive expression) appeared in the cytoplasm of the vascular endothelial cells in the myocardial ischemia and infarction regions. vW factors were not expressed in the model control group. At 1, 4, and 8 weeks after myocardial infarction, there was no significant difference in vessel counts no matter in myocardial ischemia region or in myocardial infarction region between the cell transplantation and model control groups. CONCLUSION: EPCs derived from UCB of pregnant dog can participate in the formation of blood vessels but can not promote angiogenesis after acute myocardial infarction.

Effects of umbilical cord blood endothelial progenitor cell transplantation on angiogenesis following myocardial infarction*★
INTRODUCTION
Myocardial infarction and angiogenesis related to stem cell transplantation have become a hot spot. Stem cell transplantation is to transfer stem cells into infarcted myocardium through direct injection or coronary artery infusion to promote myocardial regeneration and revascularization, which has been considered a method of promotion of cardiac and vascular regeneration [1]. It has been confirmed that stem cell transplantation into myocardium can achieve the purpose of myocardial angiogenesis [2]. Endothelial progenitor cells (EPCs) are the key cells to promote regeneration of blood vessels. In this study, EPCs derived from umbilical cord blood (UCB) of pregnant dog were infused into infarcted myocardium via coronary artery to investigate the effects of UCB-EPCs on angiogenesis.
Methods UCB collection [3] The pregnant dog was anesthetized by intramuscular injection of 5 mL atropine, 10 mL ketamine, and 0.1 mL/kg sumianxin. Under sterile condition, a median incision was made at the abdominal layers. The uterus was cut open to separate the fetal membrane and umbilical cord vessels were taken to collect some UCB and placed into a centrifuge tube in which heparin anti-freezing liquid was pre-added.
MATERIALS AND METHODS
Design An in vitro cytological experiment.
Time and setting This study was performed at the Laboratory Center of Xinhua Hospital between May 2006 and March 2007.
Materials One full-term pregnant hybrid dog was included for preparation of UCB-EPCs. Thirty-six adult dogs, equal numbers in male and female, were provided by Laboratory Animal Center, Xinhua Hospital [certification No. SYXK (hu) 2003-0031] and were randomly divided into a cell transplantation group (n = 18) and a model control group (n = 18). All experimental protocols were performed in accordance with animal’s ethical standard.
