右心室功能障碍是PVOD的一种常见并发症。在描述中,该病人需要不间断的血管活性药物的支持。PVOD被诊断之前，PAH的相关疗法(吸入一氧化氮、西地那非和前列环素)要开始依次使用。在前列环素开始使用后，病人肺循环和体循环压力都下降了，但她仍然有严重的的低氧血症，肺动脉压力仍然高于正常。当再次确认病理报告检查结果并确认PVOD后，因为但心在PVOD患者接受血管舒张药治疗后造成肺水肿，前列环素便停止使用。但这又会相继导致系统性和肺动脉压力。在这期间，逐步增加肺动脉压力与右心室功能障碍又会导致心指数下降 (图2)。病人抱怨住院期间其呼吸短促没有改善并且她继续有极高的氧气需求。
Discussion
临床上对PVOD的疑诊应该重视。大约10%的患者被认为IPAH但实际上是PVOD。一系列的病例分析发现，那些临床诊断为PAH但对药物治疗无效的患者，有14位经过肺活检后就发现有12位是PVOD。肺动脉高压的诊断和治疗指南推荐：对于疑似PVOD的病人尽量使用无创检查。除了DLCO会降低，PVOD患者的肺功能测试通常在正常范围内。对于假定的PAH患者，当DLCO < 55%时，诊断为PVOD的敏感性为64.3%,特异性为89.5%。高分辨率CT通常显示小叶中心部位有模糊影，增厚的小叶间隔线和纵隔淋巴结肿大。支气管肺泡灌洗，虽然在所有接受PAH评估的患者中不是必要的，但是这项检查能发现充满含铁血黄素的巨噬细胞肺泡所占百分比增加，这一发现更加支持PVOD的诊断。
