脓毒血症模型大鼠单次大剂量应用甲泼尼龙对罗库溴铵肌松效应的影响研究

摘 要 目的:研究脓毒血症模型大鼠单次大剂量应用甲泼尼龙对罗库溴铵肌松效应的影响。方法:取大鼠随机分为 8 组,每组 8 只,分别为药物早、晚期组,预处理早、晚期组,假手术早、晚期组,预处理对照组和药物对照组,除 2 对照组外盲肠结扎穿孔 6、16 h 建立脓毒血症早、晚期模型,假手术组行手术但不结扎穿孔。所有大鼠静脉注射罗库溴铵 3.81 mg·kg-1,预处理组给药前 30 min 单次静脉注射甲泼尼龙 190.28 mg·kg-1。记录注射罗库溴铵前、后大鼠平均动脉压(MAP)和心率(HR),并考察其药效学参数。 结果:与药物对照组比较,药物晚期组和预处理晚期组 MAP 明显降低,药物早期组和预处理早期组 HR 明显加快(P<0.05 或 P< 0.01);此外药物晚期组和预处理早、晚期组起效时间明显延长(P<0.05),临床肌松时间明显缩短且恢复指数明显降低(P< 0.01)。与药物早期组比较,预处理早、晚期组临床肌松时间明显缩短(P<0.05 或 P<0.01);与药物晚期组比较,预处理晚期组临 床肌松时间明显缩短且恢复指数明显降低(P<0.05 或 P<0.01)。结论:单次大剂量应用甲泼尼龙可强化脓毒血症对罗库溴铵肌 松效应的减弱作用。 关键词 罗库溴铵;脓毒血症;甲泼尼龙;肌松效应;药效学

ABSTRACT OBJECTIVE:To study effects of single large-dose methylprednisolone on rocuronium-induced muscle-relaxing in septic rats. METHODS:64 SD rats were randomly divided into 8 groups(n=8):drug early-therapy group,drug advanced-therapy group,pretreatment early-stage group,pretreatment advanced-stage group,sham-operation early-stage group,sham-operation advanced-stage group,pretreatment control group and drug control group. The cecum ligation and puncture operation were performed in those groups besides two control groups and 6 h and 16 h had passed to establish early or advanced septic model. The cecum ligation and puncture operation were also not performed in sham-operation group. All rats were given intravenous injection of rocuronium 3.81 mg·kg-1. Pretreatment groups were administrated with methylprednisolone 190.28 mg·kg-1 intravenously 30 min before administration. Mean arterial pressure(MAP)and heart rate(HR)of rats were recorded before and after intravenous injection of rocuronium,and pharmacodynamic parameters of rocuronium were investigated. RESULTS:Compared with drug control group, MAP of drug advanced-therapy group and pretreatment advanced-stage group decreased significantly and HR of drug early-therapy group and pretreatment early-stage group increased significantly(P<0.05 or P<0.01). Compared with drug control group,onset time of rocuronium was significantly extended in drug advanced-therapy group and pretreatment groups(P<0.05),clinical duration time and recovery index significantly cut down(P<0.01). Compared with drug early-therapy group,clinical duration time of pretreatment early-stage group and pretreatment advanced-stage group was shortened significantly(P<0.05 or P<0.01). Compared with drug advanced-therapy group,clinical duration time and recovery index significantly cut down in pre-treatment advanced-stage group (P<0.05 or P<0.01). CONCLUSION:Single large-dose methylprednisolone attenuates the weakening effect on rocuronium-induced muscle-relaxing response in septic rats.
