劳动合同补充协议

Supplement Agreement of Labor Contract

甲乙双方通过平等协商，一致同意将以下内容作为已签署劳动合同的补充

协议，与劳动合同具有同等法律效力。

Through equal consultation, party A and party B agreed to sign the below announcement as the supplement agreement of the signed labor contract, it has the same legal effect as the labor contract.

一、甲方的权利义务：

The right and obligations of part A

1、甲方考虑到乙方身体原因，不强制要求乙方来厂工作。甲、乙双方无事实

劳动关系。

Considering the health condition of party B, party B needn’t to work regularly for party A, there are no actual labor relations between A and B.

2、甲方每月按劳动合同的约定支付乙方的工资。每月10日前，将工资转至乙

方提供的银行账户内。

Party A pay the salary for party B before 10th monthly as the labor contract agreed.

3、甲方为乙方购买广州市的社会保险，甲方承担单位应缴部分，乙方承担个

人应扣缴部分。甲方不再为乙方购买住房公积金。

Party A purchase Guangzhou social insurance for party B, and party A will

undertake the part of company and party B will undertake the personnel part.

Housing fund is not contained.

二、乙方的权利义务：

The right and obligations of part B

1、在合同期内，乙方有要求甲方支付工资的权利。除每月享受合同约定的工

资外，乙方不再享受甲方为职工提供的其他福利待遇。

Party B have the right to require party A pay the salary in time within contract

period. Except the salary party B will gain no more other welfare which party A

afford for the staffs.

2、乙方除享受甲方每月支付工资待遇外，任何意外事故、疾病等事项不与甲

方相关。

Except salary, every accident and disaster of party B will have no business with

party A.

3、乙方必须于每年5月份之前配合甲方完成当年度的残疾人年审，提交的资

料包括但不限于：

➢身份证原件、复印件；

➢残疾人证原件、复印件；

➢发放工资的工商银行转账流水

Party B must coordinate party A complete the disabled annual verification before May. The documents contain but not limited,

➢ID card and the copy.

➢Disability Certificate and the copy

➢The bank transfer records of the monthly salary.

4、乙方在劳动合同期限内，不得再用残疾人证与其他用人单位建立劳动关

系，一旦被甲方发现，或影响甲方办理残疾人年审，甲方有权要求乙方返还已经支付的工资并赔偿相关损失。

If party B sign another labor contract with other company during labor contract period and affect party A get through the disabled annual verification, party A

have the right to require party B pay back the payed salary and compensate for

the related loss.

三、本补充协议的有效期与劳动合同一致。若甲、乙双方有一方要提前解除劳

动合同，需提前一个月告知。本协议一式两份，甲、乙双方各持一份。

This supplementary agreement shall be valid and consistent with the labor

contract. If either party is to terminate the labor contract in advance, it should be informed one month in advance. This agreement is in duplicate, with each party holding one copy.
