整形外科新进展与发展设想

[摘要]　目的　综述国内外整形外科研究现状和发展动态,提出未来我军整形外科的发展方向、目标和重点。方法　在PubMed、M edline和中文生物医学期刊文献数据库(CMCC)中检索近5年发表的相关文献并进行综述,同时对军队整形外科近5年来取得的成就进行统计调查。结果　近5年来,整形外科在基础研究和临床治疗方面都取得了可喜进展,在战创伤早期救治中的作用和地位也得到重视和认可;我军整形外科在慢性创面修复、皮肤软组织爆炸伤及海水浸泡伤的损伤机制和防治方法、战创伤面部畸形救治新技术、同种异体面部复合组织移植等方面取得了令人瞩目的成就

。结论　未来我军整形外科的发展应以战创伤组织缺损和畸形的修复与重建作为主要研究方向,重点开展组织工程、同种异体复合组织移植、干细胞治疗、异常瘢痕形成机制等科研工作,以解决严重面部毁损伤、广泛性胸腹壁全层缺损、慢性溃疡以及异常瘢痕等临床治疗难题,同时应注重发挥专业特长,积极参与战创伤的早期救治。

M ethods　Relevant articles published in the last five years were retrieved through a search in PubMed,Medline,and CMCC.A statistical survey was conducted to summarize the achievements obtained by the Chinese military plastic surgery unit in the last five years.Results　Considerable prog ress has been achieved in both clinical treatment and basic research of plastic surgery in the past five years.I ts important role in the early treatment of combat injury and trauma has been recog nized and emphasized.Chinese military plastic surgery has achieved considerable accomplishments in the last five years,especially in chronic wound repair;mechanism,prevention,and treatment of explosive soft tissue injuries and seawater immersion wounds;and new remedies of maxillofacial traumatic deformity,composite facial tissue allograf t,and so on.C onclusions　The repair and reconstructio n of tissue defect and defo rmity caused by war injury and trauma will be the future majo r research direction of military plastic surgery.Research work should focus on tissue engineering,composite tissue allog raft, stem cell therapy,mechanism of abnormal scar formation,amo ng others,to solve the clinical problems of destructive facial injuries, ex tensive thora-abdominal wall defects,chronic ulcer,abnormal scars,and so o n.Furthermo re,plastic surgeons should fully utilize their special skills and take active par t in the early treatment of war injury and trauma.

现代整形外科的形成和发展与两次世界大战密不可分,可以说战争是整形外科的“催化剂”,整形外科是战火中飞出的“金凤凰”。第二次世界大战后,尤其是20世纪60年代以来,整形外科进入了快速发展阶段,本文拟就近年来国内外本专业的新进展和新动向进行简要回顾,并对军队整形外科未来的发展思路、主要研究方向和建设重点提出设想。

1　整形外科“十一五”以来国内外的新进展

1.1　皮瓣移植发展趋势与进展　皮瓣移植是整形外科最基本和最常用的治疗手段之一,在修残补缺、再造器官等方面具有不可替代的作用。近年来,发现新皮瓣的热潮已经逐渐消退,而对既往应用过的皮瓣进行精选和改良,以实现皮瓣移植技术的最优化、标准化,达到治疗效果的最佳化和供区损害的最小化,已成为皮瓣研究的主要方向。
